Diverticular disease is common in the elderly Western population and its complications are frequent clinical presentations. Despite this, the pathogenesis of the condition remains relatively poorly understood. Several theories have been developed, the most acceptable suggesting elastosis of the taeniae coli as the primary event, causing shortening of the sigmoid colon, with relative mucosal excess and subsequent mucosal herniations. A Western-type diet is implicated in the increased uptake of proline from the gut, leading to elastosis of the sigmoid colon. For pathologists, in clinical practic

Diverticular disease is one of the most gastrointestinal disorders especially in developed countries. Prevalence rises with age, about two-thirds of patients in the age of 80 years are affected. In western countries diverticulosis is predominantly located in the distal colon. Only a minority of patients with diverticulosis develops symptoms. Ultrasound studies and CT scan are most important in diagnosing diverticulitis and its complications. Patients with the first attack of uncomplicated diverticulitis are treated with broad-spectrum antibiotics and in more severe case with bowel rest. Mesala

INTRODUCTION: The colovescical fistula is one of the complications of diverticular disease. It can cause significant symptoms like pneumaturia and fecaluria affecting the quality of life and sometimes leading to death, usually secondary to sepsis. We describe two cases of colovescical fistula treated by laparoscopic approach in patients with diagnosis of complicated acute diverticulitis.
CASE REPORT: We studied two patients with clinical, radiological and endoscopic diagnosis of colovescical fistula as a consequence of diverticular disease. We performed a totally laparoscopic treatment with c

Acute diverticulitis (AD) is the most common complications of diverticular disease. AD can present with a spectrum of symptoms, ranging from mild, self-limiting inflammation to severe peritonitis. A paradigm shift in the treatment of uncomplicated diverticulitis has been noted, with cumulating evidence challenging the routine use of antibiotics. High-quality evidence suggests that antibiotics may be safely omitted in healthy patients with mild AD. Nonetheless, surgery remains the mainstay treatment of patients with generalized peritonitis or recurrent, debilitating disease. In summary, current

BACKGROUND: There are no specific recommendations regarding the optimal management of this group of patients. The World Society of Emergency Surgery suggested a nonoperative strategy with antibiotic therapy, but this was a weak recommendation. This study aims to identify the optimal management of patients with acute diverticulitis (AD) presenting with pericolic free air with or without pericolic fluid.
METHODS: A multicenter, prospective, international study of patients diagnosed with AD and pericolic-free air with or without pericolic free fluid at a computed tomography (CT) scan between May

A 66-year-old man was admitted to the hospital and diagnosed with mediastinal emphysema after an abnormal chest X-ray during physical examination. During hospitalization, an intestinal obstruction was observed to be caused by barium used during gastric cancer screening. Colonoscopy demonstrated stenosis and perforation due to sigmoid colon diverticulitis. Hartmann surgery was performed, and the patient was discharged on the 58th day. Therefore, colon perforation should be considered as a cause of mediastinal emphysema, and organic stenosis may occur with intestinal obstruction caused by barium

A 73-year-old man was admitted with sigmoid colon diverticulitis. Although a biopsy did not indicate malignancy, the sigmoid colon was completely obstructed following conservative treatment. After sigmoidectomy, the histopathological findings revealed a well-differentiated adenocarcinoma localized to the mucosal surface; invasive micropapillary carcinoma (IMPC)accounting for>95% of the tumor volume spread extensively below the submucosal layer. IMPC is highly malignant and difficult to diagnose preoperatively, possibly due to the presence of poorly differentiated histological sub-types in the

The risk of anastomotic failure in the unprepared colon is generally high. Therefore, Hartmann's operation is frequently employed in emergencies. We present an alternative technique, the exteriorized anastomosis described until now only for cases of perforation. Exteriorized anastomosis was performed in 14 patients. The preoperative diagnosis was occlusive neoplasm, perforated diverticulitis or traumatic performation of the sigmoid colon. Four patients subsequently developed partial anastomotic dehiscence, but without the consequences of intraperitoneal dehiscence. We think that exteriorized a

Colonic volvulus is one of leading causes of large bowel obstruction following colorectal cancer and diverticulitis, accounting for 5% of cases. Sigmoid volvulus is most common (75%) followed by volvulus of the caecum (15%), transverse colon (3%) and splenic flexure (2%). Synchronous volvulus of the caecum and sigmoid is very rare, with six reported cases in the literature to the best of our knowledge. We report two cases within 6 months. The key learning points include that classical radiological signs of both caecal and sigmoid volvulus may not be present, and that prompt, definitive managem

In 1923 the French surgeon Henri Hartmann described an operation for resection of a cancer in the distal sigmoid and upper rectum, resulting in a permanent sigmoid colostomy. In the subsequent years, the indications for performing the Hartmann procedure have broadened to include complicated diverticulitis, ischemic bowel, iatrogenic perforations, volvulus, and colitis. Hartmann's procedure is recognised by most colorectal surgeons to be a blunt but effective method of dealing with left-sided colonic emergencies. Hartmann procedure is a safe and efficacious option for the surgeon confronted wit

Hyaline ring (pulse granuloma) is a rare benign lesion of the oral cavity soft tissues characterized by round or oval structures with scalloped borders composed by a collagenous matrix variable associated with inflammatory and foreign bogy giant cells. Vegetable fragments are variably identified. Whereas most cases occur in the oral cavity, it has also been described in other sites such as the lung and rectum. It is thought by most to represent a reaction to foreign materials. We report two incidentally discovered cases of colonic hyaline rings associated with perforated diverticulitis. Vegeta

Colonic diverticula in reality are herniations of the mucosa through a thickened musculature of the bowel wall where it is penetrated by the vasa recta. Their prevalence increases with western food and with age. Low-residue diet and increased intraluminal pressure are considered to be etiologic factors. Diverticular disease is characterised by inflammatory and bleeding complications. Diverticulitis may lead to peridiverticulitis and pericolitis with frank peritonitis. Uncomplicated diverticulosis warrants observation only under dietary guidance. Repeated complications indicate elective operati

A 49-year-old man was seen with an unusual complication of diverticulitis--a giant sigmoid diverticulum. This patient had undergone an extensive workup during a 5-year period, ranging from multiple extensive gastroenterology evaluations and procedures to an eventual psychiatric evaluation to rule out the possibility of a functional cause for his chronic, intermittent abdominal pain. Later, an accurate diagnosis of a giant sigmoid diverticulum resulted in an en bloc resection of the sigmoid colon and cyst. The patient had an uncomplicated recovery with no return of his previous symptoms. The au

Reestablishing bowel continuity subsequent to sigma resection with terminal descendostomy and blind closure of the rectum (Hartmann procedure) by a descendo-rectostomy (DR) is well suited for a laparoscopic approach. One part of the operation is performed extracorporally, and with the laparoscopic operation there is no need to consider radical tumor surgery or staging rules. We demonstrate our techniques for laparoscopic DR. - Operational steps. - A. conventionally: complete dissection of the descendo-stoma, insertion of a 29 or 31 mm circular stapler head secured by a purse string suture and

Giant sigmoid diverticula ("giant gas cysts," "giant sigmoid cysts") are an unusual complication of diverticulitis. We have recently seen two patients with such lesions, bringing the total number of reported cases to 48. Most patients are elderly and complain of abdominal pain or the presence of an abdominal mass. Air trapping by a ball-valve mechanism may be responsible for formation of the fibrous cysts. The gas-filled structures can usually be differentiated from other conditions by contrast studies. Excision of the cyst and adjacent colon with anastomosis can usually be accomplished safely

BACKGROUND: Intra-abdominal infections (IAIs) are common and severe surgical emergencies associated with high morbidity and mortality. In recent years, there has been a worldwide increase in antimicrobial resistance associated with intra-abdominal infections, responsible for a significant increase in mortality rates. To improve the quality of treatment, it is crucial to understand the underlying local epidemiology, clinical implications, and proper management of antimicrobial resistance, for both community- and hospital-acquired infections. The IRIS study (Italian Register of Complicated Intra
