The effect of a single closed-circuit rebreather decompression dive in extremely cold water to cardiac function.

PURPOSE: Dive-induced cardiac and hemodynamic changes are caused by various mechanisms, and they are aggravated by cold water. Therefore, aging divers with pre-existing cardiovascular conditions may be at risk of acute myocardial infarction, heart failure, or arrhythmias while diving. The aim of this study was to assess the effect of a single decompression CCR dive in arctic cold water on cardiac function in Finnish technical divers. METHODS: Thirty-nine divers performed one identical 45 mfw CCR dive in 2-4 °C water. Hydration and cardiac functions were assessed before and after the dive. Det…

Dive medicine capability at Rothera Research Station (British Antarctic Survey), Adelaide Island, Antarctica.

Rothera is a British Antarctic Survey research station located on Adelaide Island adjacent, to the Antarctic Peninsula. Diving is vital to support a long-standing marine science programme but poses challenges due to the extreme and remote environment in which it is undertaken. We summarise the diving undertaken and describe the medical measures in place to mitigate the risk to divers. These include pre-deployment training in the management of emergency presentations and assessing fitness to dive, an on-site hyperbaric chamber and communication links to contact experts in the United Kingdom for…

Clinical utility of dipstick urinalysis in assessing fitness to dive in military divers, submariners, and hyperbaric personnel.

INTRODUCTION: Routine dipstick urinalysis is part of many dive medical assessment protocols. However, this has a significant chance of producing false-positive or false-negative results in asymptomatic and healthy individuals. Studies evaluating the value of urinalysis in dive medical assessments are limited. METHODS: All results from urinalysis as part of dive medical assessments of divers, submarines, and hyperbaric personnel of the Royal Netherlands Navy from 2013 to 2023 were included in this study. Additionally, any information regarding additional testing, referral, or test results conc

Whole-transcriptome sequencing in advanced gastric or gastroesophageal cancer: A deep dive into its clinical potential.

Advanced gastric and gastroesophageal junction cancers (GC/GEJCs) harbor diverse molecular signatures, highlighting the need for intricate evaluations to identify potential therapeutic targets. Although whole-transcriptome sequencing (WTS) has emerged as a useful tool for understanding these molecular intricacies, its clinical implications have yet to be fully elucidated. This study evaluated the correlation between immunohistochemistry (IHC) and WTS, compared their clinical significance, and identified potential therapeutic targets undetectable through IHC alone. We enrolled 140 patients with

Diagrammatical scale to quantify herbivory severity on feijoa leaf caused by Paraulaca dives (Coleoptera: Chrysomelidae).

The aims of this work were to characterize the damage caused by Paraulaca dives in feijoa (Acca sellowiana) and develop a diagrammatic scale to evaluate the severity of herbivory. The evaluations were carried out in a feijoa progenies orchard with eight-year-old. The beetles caused damage mainly on leaves, from October to December (spring season). The distribution of beetles in the orchard occurred randomly, not following a defined pattern of incidence. The diagrammatic scale of herbivory severity was represented by seven levels (1%, 3%, 5%, 7%, 15%, 32%, and 55% of the leaf area consumed). Th

The water-exiting behavior and survival of predaceous diving beetles in responses to lambda-cyhalothrin, chlorantraniliprole, and thiamethoxam.

Predaceous diving beetles (dytiscids) are important top insect predators in various natural, agricultural, and artificial water bodies. How they respond to human disturbances such as insecticide contamination to their habitats has been understudied. In this study, we investigated the lethal effects of lambda-cyhalothrin, chlorantraniliprole, and thiamethoxam at nominal field concentrations in 3 cm-deep water in a hypothetical paddy field (Ci,3) on adult Dytiscus sinensis Feng (Coleoptera: Dytiscidae). Lambda-cyhalothrin exhibited significant lethal effects on D. sinensis adults with its Ci,3 e…

Pulmonary Barotrauma in a BUD/S Candidate Following Shallow Dives Using the MK 25 Rebreather.

Pulmonary barotrauma of ascent is a well-recognized risk of compressed air diving in the civilian and military diving communities. Chest imaging is a beneficial adjunct to clinical evaluation in screening select individuals for fitness to dive, evaluating dive-related injuries, and safely returning individuals to diving duty. We present a case of a 26-year-old male U.S. Navy Ensign and Basic Underwater Demolition/SEAL (BUD/S) candidate who experienced pulmonary barotrauma following two shallow dives to a maximum depth of 18 feet of seawater using the MK-25 100% oxygen rebreather. A chest radio…

Diving into the optoelectronic properties of Cu(II) and Zn(II) curcumin complexes: a DFT and wavefunction benchmark.

CONTEXT: Curcumin is a popular food additive around the world whose medicinal properties have been known since ancient times. The literature has recently highlighted several biological properties, but besides the health-related usages, its natural yellowish color may also be helpful for light-harvesting applications. This research aims to close a knowledge gap regarding the photophysical description of curcumin and its metallic complexes. METHODS: We conducted benchmark experiments comparing NEVPT calculations with several DFT functionals (B3LYP, M06-L, M06-2X, CAM-B3LYP, and ωB97X-D) for des

Blood Adenosine Increase During Apnea in Spearfishermen Reinforces the Efficiency of the Cardiovascular Component of the Diving Reflex.

The physiopathology consequences of hypoxia during breath-hold diving are a matter of debate. Adenosine (AD), an ATP derivative, is suspected to be implicated in the adaptive cardiovascular response to apnea, because of its vasodilating and bradycardic properties, two clinical manifestations observed during voluntary apnea. The aim of this study was to evaluate the adenosine response to apnea-induced hypoxia in trained spearfishermen (SFM) who are used to perform repetitive dives for 4-5 h. Twelve SFM (11 men and 1 woman, mean age 41 ± 3 years, apnea experience: 18 ± 9 years) and 10 control (C…

A deep dive into UV-based phototherapy: Mechanisms of action and emerging molecular targets in inflammation and cancer.

UV-based phototherapy (including psoralen plus UVA (PUVA), UVB and UVA1) has a long, successful history in the management of numerous cutaneous disorders. Photoresponsive diseases are etiologically diverse, but most involve disturbances in local (and occasionally systemic) inflammatory cells and/or abnormalities in keratinocytes that trigger inflammation. UV-based phototherapy works by regulating the inflammatory component and inducing apoptosis of pathogenic cells. This results in a fascinating and complex network of simultaneous events-immediate transcriptional changes in keratinocytes, immu

Appropriateness of intravenous immunoglobulin use in immune thrombocytopenia (ITP): A Canadian centre deep dive audit.

OBJECTIVES: Assess the appropriateness of the use of intravenous immunoglobulin (IVIG) for immune thrombocytopenia (ITP). BACKGROUND: IVIG is suggested for ITP when a rapid increase in platelet count is desired or as first line therapy if corticosteroids are contraindicated. A recent Canadian audit of IVIG requests found a lack of compliance with provincial requirements and inadequate documentation of efficacy which led the authors to conclude that the use of IVIG was broadly inappropriate for all treated diseases. METHODS: Retrospective review of patients with ITP who received IVIG at our i

Cardiac Arrest and Death Attributable to the "Diving Response" Triggered During Incision and Debridement of an Abscess of the Forehead.

The authors discuss about a patient who, while undergoing a routine procedure to drain a subcutaneous abscess within his forehead, suffered cardiac arrest that we conclude was caused by an activation of the diving response. This reflex affects homeostatic function which alters respiration and preferentially distributes oxygen stores to the heart and brain. Under some conditions, however, this reflex can also trigger cardiovascular collapse and death. The diving reflex is can begin with triggering receptors that are sensitive to cold water, submersion, or pressure within the nasal cavity and ot…

Diving and exercise: the interaction of trigeminal receptors and muscle metaboreceptors on muscle sympathetic nerve activity in humans.

Swimming involves muscular activity and submersion, creating a conflict of autonomic reflexes elicited by the trigeminal receptors and skeletal muscle afferents. We sought to determine the autonomic cardiovascular responses to separate and concurrent stimulation of the trigeminal cutaneous receptors and metabolically sensitive skeletal muscle afferents (muscle metaboreflex). In eight healthy men (30 ± 2 yr) muscle sympathetic nerve activity (MSNA; microneurography), mean arterial pressure (MAP; Finometer), femoral artery blood flow (duplex Doppler ultrasonography), and femoral vascular conduct

[Exostoses of the external auditory canal in professional diving. A case for the German Social Accident Insurance Institution].

Exostoses of the external auditory canal are often diagnosed incidentally but may also cause complications, e.g., conductive hearing loss and/or recurrent inflammations of the external ear canal due to stenosis. This paper presents current scientific data on the pathogenesis of ear canal exostoses, which obviously focus on the cold water hypothesis. We present a case of an expert opinion on occupational illness and discuss whether the legal preconditions for recognition "like an occupational disease" according to article 9(2) Social Code Book VII are met. A systematic reporting of suspected ca

The oxygen-conserving dive reflex re-examined as the principal contributory factor in sudden infant death.

This hypothesis and overview of sudden infant death syndrome (SIDS) literature is an extension of a previously published hypothesis by the author, which linked the prone sleeping position and other triggers to the oxygen-conserving dive reflex as a primary cause of SIDS. Recent prospective epidemiologic and biochemical studies, together with other data, seem to lend support to the theory that this reflex, either induced or occurring spontaneously, may lead to an irreversible pathophysiologic mechanism resulting in respiratory failure and cardiac arrest. It will also be argued that SIDS is mult
