Heteroresistance of tuberculosis: Mechanism, detection, and clinical treatment.

OBJECTIVE: Tuberculosis (TB), an infectious disease caused by Mycobacterium tuberculosis (Mtb) infection, has remained a persistent threat to global public health. Heteroresistance, a unique and complex form of TB drug resistance, refers to the co-existence of drug-resistant and drug-susceptible subpopulations within a host. This review aims to summarize the molecular mechanisms, detection methods, and targeted therapeutic strategies associated with TB heteroresistance. METHODS: PubMed was searched through both key terms and subject headings. The literatures were screened, assessed for the qu

Tuberculosis: An Update on Pathophysiology, Molecular Mechanisms of Drug Resistance, Newer Anti-TB Drugs, Treatment Regimens and Host- Directed Therapies.

Human tuberculosis (TB) is primarily caused by Mycobacterium tuberculosis (Mtb) that inhabits inside and amidst immune cells of the host with adapted physiology to regulate interdependent cellular functions with intact pathogenic potential. The complexity of this disease is attributed to various factors such as the reactivation of latent TB form after prolonged persistence, disease progression specifically in immunocompromised patients, advent of multi- and extensivelydrug resistant (MDR and XDR) Mtb strains, adverse effects of tailor-made regimens, and drug-drug interactions among anti-TB dru

Drug resistance, fitness and compensatory mutations in Mycobacterium tuberculosis.

For tuberculosis to be eradicated, the transmission of Multi-Drug-Resistant and eXtensively Drug Resistant strains of Mycobacterium tuberculosis (MDR and XDR-TB) must be considerably reduced. Drug resistant strains were initially thought to have reduced fitness, and the majority of resistant strains may actually have compromised fitness because they are found in only one or a few patients. In contrast, some MDR/XDR-TB strains are highly transmitted and cause large outbreaks. Most antibiotics target essential bacterial functions and the mutations that confer resistance to anti-TB drugs can incu

[The rate and pattern of drug resistance in Mycobacterium tuberculosis at various sites of the disease].

Drug resistance of Mycobacterium tuberculosis (MBT) to essential and reserve antituberculous drugs (ATD) was monitored in patients with tuberculosis of respiratory organs (ROT) and at extrapulmonary sites (EPT) in 2005-2007. The standard indirect absolute concentration technique was used to study the drug resistance of 578 MBT strains from pretreated patients with ROT and that of 159 MT strains from those with EPT. With all sites of tuberculosis, a poor trend continues to show an increase in the total frequency of MBT resistance (from 82.3 to 85.2% in ROT and from 43.7 to 59.3% in EPT) and its

The continuing evolution of antibiotic resistance in Neisseria gonorrhoeae: past, present and future threats to effective treatment.

Gonorrhoea constitutes a global public health threat. Although a range of antibiotics have been available to treat gonococcal infections for more than 80 years, Neisseria gonorrhoeae has shown remarkable versatility in its ability to develop resistance to successive classes of drugs. As a result, national and international treatment guidelines have had to be regularly updated to take account of increases in the prevalence of gonococcal strains resistant to recommended antibiotics. Even when particular antibiotics are no longer empirically used to treat gonorrhoea, N. gonorrhoeae often retains

Bacteria carrying mobile colistin resistance genes and their control measures, an updated review.

The plasmid encoded mobile colistin resistance (MCRs) enzyme poses a significant challenge to the clinical efficacy of colistin, which is frequently employed as a last resort antibiotic for treating infections caused by multidrug resistant bacteria. This transferase catalyzes the addition of positively charged phosphoethanolamine to lipid A of the outer membrane of gram-negative bacteria, thereby facilitating the acquired colistin resistance. This review aims to summarize and critically discuss recent advancements in the distribution and pathogenesis of mcr-positive bacteria, as well as the va

Systematic mapping of antibiotic cross-resistance and collateral sensitivity with chemical genetics.

By acquiring or evolving resistance to one antibiotic, bacteria can become cross-resistant to a second antibiotic, which further limits therapeutic choices. In the opposite scenario, initial resistance leads to collateral sensitivity to a second antibiotic, which can inform cycling or combinatorial treatments. Despite their clinical relevance, our knowledge of both interactions is limited. We used published chemical genetics data of the Escherichia coli single-gene deletion library in 40 antibiotics and devised a metric that discriminates between known cross-resistance and collateral-sensitivi

Genetic synergy between Acinetobacter baumannii undecaprenyl phosphate biosynthesis and the Mla system impacts cell envelope and antimicrobial resistance.

Acinetobacter baumannii is a Gram-negative bacterial pathogen that poses a major health concern due to increasing multidrug resistance. The Gram-negative cell envelope is a key barrier to antimicrobial entry and includes an inner and outer membrane. The maintenance of lipid asymmetry (Mla) system is the main homeostatic mechanism by which Gram-negative bacteria maintain outer membrane asymmetry. Loss of the Mla system in A. baumannii results in attenuated virulence and increased susceptibility to membrane stressors and some antibiotics. We recently reported two strain variants of the A. bauman

Chance Favors the Prepared Genomes: Horizontal Transfer Shapes the Emergence of Antibiotic Resistance Mutations in Core Genes.

Bacterial lineages acquire novel traits at diverse rates in part because the genetic background impacts the successful acquisition of novel genes by horizontal transfer. Yet, how horizontal transfer affects the subsequent evolution of core genes remains poorly understood. Here, we studied the evolution of resistance to quinolones in Escherichia coli accounting for population structure. We found 60 groups of genes whose gain or loss induced an increase in the probability of subsequently becoming resistant to quinolones by point mutations in the gyrase and topoisomerase genes. These groups inclu

Class 1 integron carrying qacEΔ1 gene confers resistance to disinfectant and antibiotics in Salmonella.

Salmonella has presented increasingly alarming rates of antimicrobial resistance believed to be a result of a high prevalence of integrons. It is speculated that disinfectant-resistant isolates are due to the expression of qacEΔ1, an efflux pump located in the 3' conserved sequence (3'CS) of class 1 integrons. With this concern, we tested the antibiotic and disinfectant resistance of 581 Salmonella strains collected from different sources, and characterized their integron structures. Gene expression and induction experiments were also performed. Results showed that Salmonella have high resista
