Bridging phenomenon - Simplifying complex ear reconstructions.

BACKGROUND: Extirpation of noninvasive skin tumors of the anterior ear may create large defects. Various flaps, described to cover these defects, demand special knowledge without which a loss of the fine detail of the ear may result. METHODS: Healthy, exposed cartilage is deliberately excised leaving a basic framework for support, thus preserving contours and a well-vascularized recipient bed for full-thickness skin grafting. The grafts heal by revascularization and "bridging," a phenomenon whereby grafts on avascular beds (such as denuded cartilage) are revascularized. RESULTS: By marrying …

Endoscopic middle ear surgery in children.

Endoscopy is advantageous in many aspects of pediatric ear surgery, especially for tympanoplasty and cholesteatoma. The narrower pediatric meatus can restrict access for a totally endoscopic approach, but when possible, avoidance of an external incision is greatly appreciated by children. In cholesteatoma surgery, less residual disease and better hearing from ossicular preservation are achievable. Factors influencing selection of a totally endoscopic approach, or use of endoscopy as an adjunct are reviewed, along with tips to facilitate successful endoscopic surgery in children. The optimum ap…

[Eustachian tube and middle ear mechanics].

Even the most sophisticated prostheses for reconstruction of the ossicular chain do not work in the presence of Eustachian tube dysfunction. This review gives an update on the mechanisms of middle ear pressure regulation and middle ear ventilation, as well as methods for measuring the opening and closing function of the Eustachian tube. So far, in most tube function tests pressures are applied far beyond the physiological range in order to open the tube or force it to open. New methods like sonotubometry with perfect sequences (PSEQ) or the application of pressure in the nasopharynx with the E…

Ear acupuncture in unilateral migraine pain.

The needle contact test (NCT) is a diagnostic technique useful to identify, through the contact of the needle on the skin of the ear, the most efficacious points for reducing pain during a migraine attack. The aim of this study was to identify the most important auricular zones for pain control by applying the NCT in a group of 15 women during a unilateral attack of migraine without aura. We also assessed how effective the insertion of a semi-permanent needle in these zones was in reducing the migraine pain during the next 24 h. The most effective tender points in pain control were located on …

Ear reconstruction with porous polyethylene implants.

This article describes a surgical technique using porous polyethylene as the framework material for ear reconstruction. In comparison to the use of rib cartilage, porous polyethylene - first described by Berghaus in 1982 - provides better definition and projection as well as congruency with the opposite side. Hospitalization time is significantly shorter. There are less surgical interventions than with traditional microtia operations that use rib cartilage, and the patient is spared the additional procedure needed to remove the rib cartilage, with all the associated complications as well as th…

Severe visual loss after ear surgery.

Sigmoid sinus thrombosis was once one of the most common intracranial complications of otitis media, but this has become less common since the advent of antibiotics. The frequency of sigmoid sinus hypoplasia reported in the literature is about 17%. Considering the possibility of postoperative intracranial hypertension, sigmoid sinus and jugular bulb surgeries were contraindicated in this case.We present a case with sigmoid sinus thrombosis of the sinus due to compression flowing surgical injury. This is a unique case, which presents with contralateral sigmoid sinus hypoplasia. Eventually, the

Middle ear adenoma: a challenging diagnosis.

Middle ear adenomas are rare benign tumors, which can easily be mistaken for other conditions radiologically. They derive from the middle ear mucosa. We report the case of a 48-year-old man with a history of decreased left-sided hearing and intermittent pulsatile tinnitus. High-resolution CT of the temporal bones revealed a well-defined left middle ear soft-tissue attenuation abutting the head of the malleus. Surgical excision revealed a middle ear adenoma.

External ear reconstruction in hemifacial microsomia.

Restoration of a missing external ear is important for the child/adult with craniofacial deformities to achieve balance and harmony of the face and head. Ear reconstruction is not an isolated requirement, however. For 2 reasons, we believe that external ear reconstruction is an integral part of craniofacial reconstruction: (1) the auricle is located, not in the face mask, but just outside the face mask, and projects onto the posterior part of the temporal bone; and (2) isolated microtia may be an early clinical marker of hemifacial microsomia. This is the first English-language article describ…

Cutting edge of inner ear MRI.

CONCLUSION: Recent advances in clinical MR imagers, such as the 3-Tesla, multi-channel phased-array coil and novel pulse sequences, allow the evaluation of subtle alterations in the inner ear fluid environments and breakdown of the blood-labyrinthine barrier. Intratympanic injection of Gd-DTPA allows the imaging detection of endolymphatic hydrops in patients. OBJECTIVES: To describe the current status of inner ear MRI and future directions for imaging. MATERIALS AND METHODS: Based on our experiences and literature research, a brief review of the history and recent developments of inner ear M…

Do viruses cause inner ear disturbances?

The association of viral infection to inner ear disease is controversial. Experiments on animals show that several viruses are capable of causing hearing loss, if applied into the perilymph. Some of these have specific affinity to the cellular type of the inner ear, as sensory epithelia and cochlear nerve. Some viruses as adenoviruses and Coxsackie virus B have specific CAR receptors that are identified in different cell types, whereas other act by attaching onto nonspecific cellular surface receptors. Some viruses such as varicella zoster virus (VZV) do not cause disease in rodents. We assess…

Middle ear hemangiona: a case report.

A middle ear hemangioma was detected in a 51-year-old man who suffered from fullness and hearing loss in the left ear. This case is presented with a review of the relevant literature. The mass was surgically resected for diagnostic and therapeutic reasons, and pathologic examination revealed a benign hemangioma. The CT, MRI, angiography, and light microscopy findings of the tumor are documented and compared with those a glomus tumor. The significance of this diagnosis is also discussed.

Pediatric ear, nose, and throat emergencies.

Acute otitis media is the most common infection for which antibiotics are prescribed in children, resulting in more than 20 million antibiotic prescriptions annually. New practice guidelines published by the American Academy of Pediatrics and the American Academy of Family Physicians call for the judicious use of antibiotics in view of increasing antibiotic resistance and the unclear necessity of the use of antibiotics in children with uncomplicated acute otitis media. This article reviews those guidelines, several other common ear, nose, and throat entities, including sinusitis and dental eme…

Iatrogenic complications from chronic ear surgery.

Iatrogenic complications can and do occur in ear surgery. Whether the surgery is undertaken to treat chronic infectious disease or for other purposes (eg, for cochlear implants, shunts, or skull base approaches), the key to avoiding iatrogenic injuries and untoward events is attention to detail. Knowledge of what to do when the unexpected happens is the key to minimizing any negative outcome for the patient.

[Ear canal narrowing: a case report].

BACKGROUND: ear canal narrowing is the thinning or closing of the ear canal. This can damage the sound transmition from the external to the internal ear, which in turn may cause a false hearing loss. AIM: to alert the professionals of the area on how to avoid a miss diagnosis. METHOD: a 47 year old woman, submitted to two audiologic evaluations: one without avoiding ear canal closing and one avoiding it. RESULTS: a decrease of up to 40 dB was observed in the results of the second evaluation. CONCLUSION: in order to obtain reliable results it is extremely important to verify the presence of…
