Arthroscopic treatment of post-traumatic elbow contracture.

The purpose of this study was to evaluate range of motion and patient-reported outcome after complete arthroscopic release of post-traumatic elbow contracture. Fourteen consecutive patients who underwent elbow arthroscopy and capsular release were reviewed retrospectively at a minimum follow-up of 1 year. Pain and range of motion were measured. Patient outcome was assessed with the American Shoulder and Elbow Surgeons Elbow Assessment Form. Mean self-reported satisfaction on a visual analog scale was 8.4 out of 10. Only 6 patients continued to have pain, with a mean maximum pain score of 4.6 o…

Flexion-type supracondylar elbow fractures in children.

Flexion-type supracondylar elbow fractures are uncommon in children. Minimally displaced fractures were treated in plaster cast in 7 children, and 22 children with a displaced fracture were treated by closed reduction and percutaneous pinning. Results after an average follow-up of 6.3 years showed that the treatment of choice yielded excellent or good results in 86.2% of the patients. All patients were satisfied with the end result and had normal use of their elbow.

Functional outcome of semiconstrained total elbow arthroplasty.

BACKGROUND: The objective of the present study was to review the results of primary total elbow arthroplasty with use of the Coonrad-Morrey prosthesis. Two hypotheses were tested: (1) the results in patients with inflammatory arthritis would be superior to those in patients with a traumatic or posttraumatic condition, and (2) the isometric extensor torque after total elbow arthroplasty would be significantly less than that of the contralateral elbow. METHODS: Forty-seven consecutive patients (fifty-one elbows) had the operation performed by one of three surgeons between November 1, 1989, and …

How I Manage Tennis and Golfer's Elbow.

Lateral and medial epicondylitis are common in sports and work activities that require repetitive gripping or exert acute forces on the forearm. Left untreated, epicondylitis may interfere with daily activities. Most patients respond to early conservative treatment consisting of activity modification, ice, anti-inflammatory medications, and rehabilitation exercises. Approximately 5% of patients will fail conservative treatment and require surgery. The outcome for most patients following surgery is excellent.

Closed elbow dislocation and brachial artery damage.

Damage to the brachial artery associated with closed elbow dislocation but without accompanying fracture is rare. Only 25 cases have been reported to date. We present a case that responded well to brachial artery vein grafting, and review the literature on this subject. Operative repair is recommended in this uncommon complication of closed dislocation of the elbow.

[Treatment principles of chronic recurrent elbow dislocation].

The chronic recurrent luxation of the elbow is a seldom event. The specific diagnostic uncovers the pathomorphology of the injury. Stabilization has to be carried out according to the pattern of injury. Clinical findings and treatment are described on the basis of three observed cases.

External oblique fasciocutaneous flap for elbow coverage.

The external oblique fasciocutaneous pedicle flap can be used for reconstruction of soft-tissue defects of the elbow. This flap has been used in five patients, and results have been good. The technique is appropriate in patients with recurrent defects of the elbow in whom local tissue has been previously used and is no longer available. With the development of local fasciocutaneous units, this method may have limited application. However, because of the relationship of this flap to the elbow, the procedure can be done easily and rapidly with minimal immobilization. It is a clinical impression …

Compound elbow dislocation and brachial artery rupture.

A case of brachial artery rupture following compound dislocation of the elbow is reported. Arterial injury is a well-known complication of a supracondylar fracture of the humerus, but is very rare in association with elbow dislocation. A reversed saphenous vein graft was used for treatment of the arterial injury and is the recommended method of management.

Painful neuromas in long below-elbow amputees.

During a five-year period, seven patients were seen who failed to wear their protheses because of neuromas in their amputation stumps. Proximal ligation of the median, radial, and ulnar nerves allowed prosthetic wear and use in every instance. The advantage of this technique used at the time of the original amputation or at an early elective time will aid in the rapid rehabilitation of the patient.

A Different Type of Tennis Elbow: Spontaneous Septic Arthritis of the Elbow in a Previously Healthy 40-Year-Old Male.

Septic arthritis of the elbow is a serious and uncommon condition, particularly in immunocompetent individuals and those without risk factors. We present the case of a 40-year-old previously healthy male who developed spontaneous elbow septic arthritis after initiating a recreational tennis regimen. He presented with progressive elbow pain, loss of extension, and swelling, but lacked fever or any systemic findings suggestive of infection on arrival. Point-of-care ultrasound (US) revealed an elbow effusion, which then prompted US-guided arthrocentesis in the emergency department. Synovial fluid…

Comparative study of open elbow arthrolysis with and without hinge external fixation for the treatment of post-traumatic elbow stiffness.

BACKGROUND: The present study aimed to compare the functional outcomes of hinged external fixators and non-external fixation in open elbow arthrolysis (OEA) for post-traumatic elbow stiffness (PTES) and to evaluate their applicability and limitations in patients with posttraumatic elbow stiffness. METHODS: The clinical data of patients with PTES treated with OEA at our hospital between March 2015 and June 2022 were retrospectively analyzed. The assessed variables were the operation time, intraoperative blood loss volume, duration of hospitalization, and treatment costs. The elbow range of mot…

CT is a feasible imaging technique for detecting lesions in horses with elbow lameness: A study of 139 elbows in 99 horses.

Computed tomography (CT)is increasingly available in veterinary referral practices; however, published studies describing CT lesions of the equine elbow are currently lacking. In this single-center, retrospective, observational study, horses undergoing elbow CT at Equitom between July 2015 and October 2018 were reviewed. Subchondral bone sclerosis; resorption of the radius, ulna, and humerus; osteophyte; and enthesophyte lesions were graded. One hundred thirty-nine elbows of 99 horses (16 with elbow pain and 123 control elbows) were included (median age, 9 years). Osseous cyst-like lesions (n …

Comparison of functional outcomes of total elbow arthroplasty vs plate fixation for distal humerus fractures in osteoporotic elbows.

Treating intra-articular fractures about the osteoporotic distal humerus poses a significant challenge. The purpose of this retrospective study was to evaluate functional outcomes for distal humeral fractures treated with total elbow arthroplasty (TEA) or open reduction and internal fixation (ORIF) in a nonarthritic elderly population with osteoporosis. We reviewed the records of all women older than age 60 who had undergone surgical treatment for intraarticular distal humerus fractures (Orthopaedic Trauma Association types 13B and 13C) by 1 of 2 surgeons. Demographic and operative data were o…

Elbow Arthroscopy: Pearls to Avoid Nerve Injuries.

Elbow arthroscopy is a useful tool that can be applied in a variety of surgical indications. However, performing the procedure safely demands a thorough understanding of the proximity of neurovascular structures around the elbow. Although nerve injuries in elbow arthroscopy are rare, complications can further be avoided by adhering to a set of principles designed to protect the surrounding neurovascular structures. Before making portals, the surgeon should palpate and mark the ulnar nerve to confirm its location in the groove. Next, the joint should be insufflated with fluid to distend the joi…

Endoscopic anterior capsulectomy for severe elbow contractures.

Elbow stiffness secondary to trauma or surgical reconstruction can sometimes result in a severe contracture with restricted joint space, and arthroscopic access to the joint is difficult. Previous surgery and severe stiffness can also alter the position of neurovascular structures and iatrogenic injury is possible with an inside-out arthroscopic approach. To overcome these technical difficulties, an endoscopic approach to the anterior capsule can be performed as an alternative to open approach. The endoscopic approach utilises the sub-brachialis space for an outside-in capsular resection under…
