Risk of arterial and venous thromboembolism in patients with atrial fibrillation or flutter: A nationwide population-based cohort study.

BACKGROUND: Patients with atrial fibrillation or flutter (AFF) are at increased risk of ischemic stroke, but their risk of other thromboembolic events remains less clear. METHODS: During 2004-2013, we conducted a nationwide population-based cohort study using Danish medical registries. We identified all patients with first-time AFF and sampled a sex-, age-, and calendar year-matched general population comparison cohort without AFF. For myocardial infarction, peripheral embolism, ischemic stroke, hemorrhagic stroke, deep venous thrombosis, and pulmonary embolism, we computed cumulative risks a

An analysis of ischemic stroke in an urban southern California population. The University of California, San Diego, Stroke Data Bank.

BACKGROUND: Stroke databanks may provide important information regarding regional and temporal variations in the causes of stroke. METHODS: Five hundred consecutive patients presenting to the University of California, San Diego, stroke services with acute ischemic stroke were evaluated prospectively. A specific cause of stroke was assigned in each case according to predetermined diagnostic criteria. RESULTS: Relative incidences of ischemic stroke causes were as follows: lacunar, 27%; unknown cause, 23%; cardioembolic, 22%; large-vessel atherothrombotic/embolic, 18%; and miscellaneous, 10%.

Ischemic stroke occurring during intercourse in young women on oral contraceptives.

Ischemic stroke occurring during intercourse in young patients is exceedingly rare. We present 2 cases of young women taking oral contraceptives, each presenting with an ischemic stroke. Transthoracic echocardiography revealed a patent foramen ovale in one patient and an atrial septal defect in the other. The most likely cause of stroke in both patients is embolic. Despite conflicting evidence, young patients presenting with ischemic stroke and found to have a patent foramen ovale or atrial septal defect should be considered for possible device-based closure.

[Neurological diagnosis and therapeutic measures in cerebral embolism].

UNLABELLED: Stroke is caused by intracerebral or subarachnoid hemorrhage in about 15% of clinical presentations and the remaining 85% result from ischemia. About 15% of ischemic strokes are caused by emboli arising from the heart. In younger patients (18 to 50 years) with ischemic strokes or transient ischemic attacks (TIA), the incidence of cardiac embolism is increased to 23 to 36%. DIAGNOSIS: a) SYMPTOMS: Individual neurologic symptoms of stroke do not provide sensitive or specific indications of the underlying mechanism. In 25 to 82% of patients with possible embolic stroke, there is an …

The Association Between Adult Height and Stroke Incidence in Japanese Men and Women: A Population-based Case-Control Study.

BACKGROUND: No studies have examined the associations between adult height and ischemic stroke subtypes. METHODS: We conducted a population-based case-control study that included 2,451 thrombotic and 687 embolic stroke cases, as well as 1,623 intracerebral and 768 subarachnoid hemorrhage cases without history of stroke aged 40-79 years, and the same number of sex- and age-matched controls. Cases and controls were grouped according to the quintile cut-off values of height in controls, and the third quintile, which was approximately the average height group, was used as the reference group. Cerebral infarction: Surgical timing dilemma in Streptococcus agalactiae endocarditis - A case report.

Infective endocarditis (IE) is a relatively uncommon disease with a high mortality rate. Early recognition and prompt antibiotic therapy are crucial for preventing complications such as acute heart failure, uncontrolled infection, and embolic events. Embolic-related stroke events significantly increase both morbidity and mortality. Stroke may be the first clinical manifestation of IE. We report the case of a 57-year-old male who initially presented with stroke symptoms and left-sided weakness. His condition deteriorated rapidly and progressed to septic shock within 6 h. Broad-spectrum antibiot

Cardiogenic brain embolism. Cerebral Embolism Task Force.

One in six ischemic strokes is due to cardiogenic embolism. The current knowledge about the natural history, diagnosis, prevention, and treatment of cardioembolic stroke is reviewed. Nonrheumatic atrial fibrillation is the most frequent substrate for brain embolism; patients with this dysrhythmia have a fivefold increase in the risk of stroke. Ischemic heart disease (25%), rheumatic mitral stenosis (10%), and prosthetic cardiac valves (10%) are other major sources of cardioembolic stroke. Mitral annulus calcification and calcific aortic stenosis infrequently serve as cardioembolic sources. As …

Infarct pattern and clinical outcome in acute ischemic stroke following middle cerebral artery occlusion.

BACKGROUND: Cerebral arterial occlusion develops via two distinct mechanisms: thrombosis and embolism. Discrimination between thrombosis and embolism is an important aspect needed for further determining the etiology of stroke in a patient. This study evaluated infarct patterns and outcomes in acute stroke patients with relevant artery occlusions, focusing on features specific to each occlusion mechanism. METHODS: Acute ischemic stroke patients who were consecutively registered in a tertiary hospital between 2002 and 2010 with infarctions in the middle cerebral artery territory and a correspo

Cerebral Embolic Protection Devices for Transcatheter Aortic Valve Replacement: Review of the Literature and Future Perspectives.

PURPOSE OF REVIEW: Transcatheter aortic valve replacement (TAVR) has been a revolutionary therapy in the treatment of aortic valve stenosis. The risk of stroke associated with TAVR has decreased significantly since its introduction; however, it remains a devastating complication when it does occur. RECENT FINDINGS: Many of the strokes associated with TAVR occur peri-procedurally and are thought to be due to embolic debris entering the cerebrovascular circulation. A number of different cerebral embolic protection devices (CEPD) have been developed and are in various stages of testing and use.

Atrial-septal-aneurysm-associated thrombus and stroke: Demonstration with transesophageal echocardiography.

Atrial septal aneurysm is an increasingly recognized cardiac abnormality. It represents a marked bulging of the interatrial septum into one atrium or the other. A consistent association between this structure and embolic stroke has been demonstrated. Because of the high prevalence of patent foramen ovale or atrial septal defect associated with this structure, paradoxical embolism has been suggested as a mechanism for embolic events. An alternate explanation is the local association of the aneurysm with thrombus. Few data have been found to support this mechanism, however. We report a young wom…

Patent Foramen Ovale-Associated Stroke: A Neurologist's Perspective.

Paradoxic embolism through a patent foramen ovale (PFO) is a common cause of ischemic stroke, accounting for 1 in 20 of all ischemic strokes. Neurologists play the leading role in diagnosing PFO-associated stroke, determining that a cerebral infarct is embolic in distribution and excluding other potential stroke mechanisms. Among patients aged 18 to 60 years old with a PFO and an otherwise cryptogenic stroke, the PFO-Associated Stroke Causal Likelihood classification system should be used to identify the 85% of patients likely to benefit from PFO closure and the 15% of patients likely to be ha

Standard or lower intensity warfarin anticoagulation prevents embolic stroke in the elderly with a hemorrhage risk even lower than aspirin. In fact, adverse effects seem to occur more often with aspirin. Excess anticoagulation hazards are prevented with lower starting doses, stricter corrections, more frequent International …

Drug therapy of neurovascular disease.

The goal of therapy for acute stroke is immediate treatment to reduce the risk of permanent neurologic disability and death. For ischemic stroke, it is imperative to reestablish brain perfusion while at the same time trying to preserve neuronal function. Early treatment of hemorrhagic stroke, particularly subarachnoid hemorrhage, also may improve clinical outcome. Prevention of stroke still remains the most important treatment strategy. In this review, pharmacologic strategies to both prevent and treat thrombotic, embolic, and hemorrhagic strokes are discussed. Treatment modalities for other c

Clinical relevance of atrial fibrillation/flutter, stroke, pacemaker implant, and heart failure in Emery-Dreifuss muscular dystrophy: a long-term longitudinal study.

BACKGROUND AND PURPOSE: Emery-Dreifuss muscular dystrophy (EDMD) is a rare inherited disorder associated with cardiac involvement. We investigated the spectrum and relevance of the cardiac manifestations of EDMD, focusing on bradyarrhythmias and tachyarrhythmias (including atrial fibrillation/flutter), embolic stroke, and heart failure. METHODS AND RESULTS: Eighteen patients (age 42.8+/-19.6 years) with genetically confirmed X-linked (n=10, including 3 carriers) or autosomal dominant (n=8) EDMD were followed for a period ranging from 1 to 30 years in a research center for neuromuscular diseas…

Before, during, and after: An Argument for Safety and Improved Outcome of Thrombolysis in Acute Ischemic Stroke with Direct Oral Anticoagulant Treatment.

Direct oral anticoagulants are the primary stroke prevention option in patients with atrial fibrillation. Anticoagulant use before stroke, however, might inhibit clinician comfort with thrombolysis if a stroke does occur. Resuming anticoagulants after ischemic stroke is also problematic for fear of hemorrhage. We describe extensive literature showing that thrombolysis is safe after stroke with direct anticoagulant use. Early reinstitution of direct anticoagulant treatment is associated with lower risk of embolic recurrence and lower hemorrhage risk. The use of direct anticoagulants before, dur

An MRI Based Ischemic Stroke Classification - A Mechanism Oriented Approach.

Oxfordshire Community Stroke Project and Trial of Org 10172 in acute stroke treatment are the commonly used ischemic stroke classification systems at present. However, they underutilize the newer imaging technologies. Diffusion-weighted magnetic resonance imaging (DW-MRI) of the brain can detect the site and extent of infarcts accurately. From the MRI patterns, the mechanisms of ischemic stroke can be inferred. We propose to classify ischemic infarcts into the following types based on their DW-MRI appearance: cortical territorial infarcts, striatocapsular infarcts, superficial perforator infar

Cerebral embolic protection devices for transcatheter aortic valve replacement: A meta-analysis and trial sequential analysis.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) is associated with periprocedural stroke risk due to embolic debris. The efficacy and safety of cerebral embolic protection devices (CEPDs) remain uncertain, with conflicting results between trials. We performed a meta-analysis of randomized controlled trials (RCTs) of current-generation CEPDs for TAVR. METHODS: Electronic databases were searched for RCTs comparing clinical outcomes with routine CEPD use vs no CEPD use. Outcomes of interest included any stroke, disabling stroke, and all-cause mortality. Risk ratios (RR) or mean differe

[Update on antithrombotic secondary prevention of ischemic stroke].

Over the past decade innovations regarding antithrombotic treatment for secondary stroke prevention have been particularly dominated by the non-vitamin K dependent anticoagulants in patients with cardioembolic stroke; however, several studies investigating other important aspects have also recently been published. This update focuses on new trials on intensified antiplatelet therapy in the early phase of (mild) stroke and transient ischemic attacks (TIA) as well as studies on secondary prevention in patients with embolic stroke of undetermined source (ESUS). Data from these studies are critica…
