Comparison of embryological and clinical outcome in GnRH antagonist vs. GnRH agonist protocols for in vitro fertilization in PCOS non-obese patients. A prospective randomized study.

PURPOSE: Embryological and clinical efficacy of gonadotropin-releasing hormone (GnRH) antagonist and agonist stimulation protocols in non-obese women with polycystic ovarian syndrome (PCOS) were compared. METHODS: A prospective randomized study. SETTING: Medical University Hospital. PATIENTS: 70 infertile PCOS patients; 33 in GnRH antagonist and 37 in GnRH agonist group. RESULTS: Similar mature metaphase II oocyte rate (76% vs. 76%) was observed in both protocols. Optimal pronuclear morphology zygotes dominated in both groups (64% vs. 66%). Transferred embryo quality did not differ in both

Arterio-arterial malformation between a high origin radial artery and brachial artery within the cubital fossa - its clinical and embryological significance: a case report.

INTRODUCTION: Arterial variations of the upper extremity are commonly seen in the radial and ulnar arteries. Arterial variations can be damaged through iatrogenic means if not properly documented. CASE PRESENTATION: A rare arterial anomaly was found in a 61-year-old female cadaver consisting of an arterio-arterial malformation between a high-origin radial artery and the brachial artery within the cubital fossa. The high-origin radial artery arose from the axillary artery, deep to the pectoralis minor muscle. It coursed superficially through the anterior compartment of the arm, converging with

Spatial distribution of postotic crest cells defines the head/trunk interface of the vertebrate body: embryological interpretation of peripheral nerve morphology and evolution of the vertebrate head.

The migration pathways and spatial distribution of neural crest cells largely depend on the embryonic architecture. At the preotic level in the chick embryo, cephalic crest adhere to even-numbered rhombomeres proximally, and populate each pharyngeal arch distally, thus prefiguring the morphology of the branchiomeric nerves. This distribution pattern is possible because of the absence of somites in the head. In the postotic region, however, somites and pharyngeal arches coexist at the same axial level. The caudalmost cephalic crest cell population, the circumpharyngeal crest cells, are derived

Postmortem discovery of a rare five-vessel aortic arch with aberrant right subclavian artery and bilateral vertebral artery origin variants: embryological analysis and perioperative implications.

Complex aortic arch branching variants with multiple direct branches and variant vertebral artery origins are rare and typically incidental. We report a postmortem-confirmed case of a rare five-vessel aortic arch configuration identified in a 65-year-old man who underwent elective mitral and tricuspid valve repair for severe mitral regurgitation. The postoperative course was complicated by delayed neurological deterioration and fatal tonsillar herniation. Examination of the thoracic aorta revealed a five-branch aortic arch consisting of the right common carotid artery, left common carotid arte

Effect of anti-nuclear antibody positivity with negative specific anti-extractable anti-nuclear antibodies on IVF and ICSI embryological outcomes: potential role of sperm concentration.

RESEARCH QUESTION: Anti-extractable nuclear antigens (ENA) and anti-nuclear antibodies (ANA) impair oocyte and embryo quality. What are the embryo laboratory outcomes in ANA-positive but anti-ENA-negative patients? DESIGN: ANA-positive women (n = 113) undergoing their first IVF/ICSI cycle and matched ANA-negative women (n = 213) were included. Clinical characteristics and IVF/ICSI outcomes were compared between groups. Multivariable linear regression controlled for confounders to assess the relationship between ANA positivity and laboratory outcomes. Subgroup analysis explored the interaction

A rare bilateral renal arterial configuration featuring a right-sided crossed trajectory and left-sided type IIa branching: morphometric and embryological insights from cadaveric dissection.

PURPOSE: This study aimed to report and characterize bilateral renal artery (RA) variations observed during cadaveric dissection and to evaluate these findings in the context of embryological development and morphometric analysis. CASE PRESENTATION: During routine anatomical dissection of an 87-year-old Caucasian male cadaver, bilateral variations in the renal arteries were identified. On the right side, two renal arteries (RRA1 and RRA2) were observed, each giving rise to presegmental and segmental branches. Notably, the RRA1 also gave rise to the right inferior suprarenal artery. This arter

Bilateral intravesical ureterocele associated with unilateral partial duplication of the ureter and other anomalies: proposal of a new variant to the classification of ureterocles based on a perinatal autopsy, review of the literature and embryology.

The aims of this study were to demonstrate a case of bilateral intravesical ureterocele associated with megacystis and mega-ureters, unilateral partial duplication of the ureter and unilateral segmental renal dysplasia of the upper pole and an accessory spleen and to propose an addition of the new variant to the classification of ureteroceles. A perinatal necropsy was conducted on the 21-week fetus by employing the Rokitansky procedure with evisceration performed in blocks. The autopsy revealed the aforementioned abnormalities without cardiac or neural anomalies. The amniocentesis report was n

Cardinal vein isomerism: an embryological hypothesis to explain a persistent left superior vena cava draining into the roof of the left atrium in the absence of coronary sinus and atrial septal defect.

BACKGROUND: A persistent left superior vena cava (PLSVC) is a relatively frequent systemic venous anomaly associated with congenital heart defects. This anomaly has been explained with the persistence of the left superior cardinal vein. PLSVC usually drains into the right atrium, via coronary sinus, but it joins the left atrium in approximately 8% of the cases either directly in the setting of atrial isomerism, or via an unroofed coronary sinus, or through a coronary sinus type atrial septal defect. CASE REPORT: We describe a case of an adult patient with atria in the situs solitus, PLSVC dra

A novel embryological theory of autism causation involving endogenous biochemicals capable of initiating cellular gene transcription: a possible link between twelve autism risk factors and the autism 'epidemic'.

Human alpha-fetoprotein is a pregnancy-associated protein with an undetermined physiological role. As human alpha-fetoprotein binds retinoids and inhibits estrogen-dependent cancer cell proliferation, and because retinoic acid (a retinol metabolite) and estradiol (an estrogen) can both initiate cellular gene transcription, it is hypothesized here that alpha-fetoprotein functions during critical gestational periods to prevent retinoic acid and maternal estradiol from inappropriately stimulating gene expression in developing brain regions which are sensitive to these chemicals. Prenatal/maternal

Novel embryological classifications of hepatic arteries based on the relationship between aberrant right hepatic arteries and the middle hepatic artery: A retrospective study of contrast-enhanced computed tomography images.

BACKGROUND: Variations in hepatic arteries are frequently encountered during pancreatoduodenecomy. Identifying anomalies, especially the problematic aberrant right hepatic artery (aRHA), is crucial to preventing vascular-related complications. In cases where the middle hepatic artery (MHA) branches from aRHAs, their injury may lead to severe liver ischemia. Nevertheless, there has been little information on whether MHA branches from aRHAs. This study aimed to investigate the relationship between aRHAs and the MHA based on the embryological development of visceral arteries. METHODS: This retro

The guidelines issued by the European Society for Human Reproduction and Embryology and the American Society for Reproductive Medicine regarding the induction of ovulation with metformin in patients with the polycystic ovary syndrome potentially require reconsideration.

In 2007, the European Society for Human Reproduction and Embryology (ESHRE) and the American Society for Reproductive Medicine (ASRM) issued guidelines in Thessaloniki regarding the use of metformin and clomiphene for the induction of ovulation in patients with anovulatory polycystic ovary syndrome (PCOS). According to these guidelines, the use of metformin should be limited to patients with impaired glucose tolerance and should be interrupted well before the administration of clomiphene, thus restricting the use of metformin to a minority of patients with PCOS. More recent data suggest that t…
