Rescue therapies for seizure emergencies: New modes of administration.

A subgroup of patients with drug-resistant epilepsy have seizure clusters, which are a part of the continuum of seizure emergencies that includes prolonged episodes and status epilepticus. When the patient or caregiver can identify the beginning of a cluster, the condition is amenable to certain treatments, an approach known as rescue therapy. Intravenous drug administration offers the fastest onset of action, but this route is usually not an option because most seizure clusters occur outside of a medical facility. Alternate routes of administration have been used or are proposed including rec…

Dermatology emergencies: handy hints for the acute medical team.

This article will help the general physician recognize and manage acute dermatology presentations. This can be challenging for non-dermatology doctors owing partly to the difficulty in providing an extensive dermatology undergraduate education and the lack of exposure to dermatology patients. The problem is further compounded at many hospital trusts because of the lack of on-site dermatology 'on-call'. The general physician must be able to recognize dermatology emergencies in order to provide initial management and maintain appropriate referrals to acute dermatology services. The emergency pre…

Multiple Electrolyte and Metabolic Emergencies in a Single Patient.

While some electrolyte disturbances are immediately life-threatening and must be emergently treated, others may be delayed without immediate adverse consequences. We discuss a patient with alcoholism and diabetes mellitus type 2 who presented with volume depletion and multiple life-threatening electrolyte and metabolic derangements including severe hyponatremia (serum sodium concentration [SNa] 107 mEq/L), hypophosphatemia ("undetectable," <1.0 mg/dL), and hypokalemia (2.2 mEq/L), moderate diabetic ketoacidosis ([DKA], pH 7.21, serum anion gap [SAG] 37) and hypocalcemia (ionized calcium 4.0 mg

Highlighting Indication of extracorporeal membrane oxygenation in endocrine emergencies.

Extracorporeal membrane oxygenation (ECMO) has been repeatedly used to rescue patients with cardiopulmonary arrest. However, its clinical utility in endocrine emergencies remains unclear. Herein, we describe a case series of 12 patients presenting with refractory shock secondary to endocrine emergencies who were rescued by ECMO support. Patients were identified between 2005 and 2012 from our ECMO registry. The diagnostic distribution was as follows: pheochromocytoma crisis (n = 4), thyroid storm (n = 5), and diabetic ketoacidosis (n = 3). The initial presentation of pheochromocytoma crisis was…

Cross-sectional imaging of nontraumatic emergencies of the spleen.

Multiple nontraumatic splenic emergencies are encountered during the imaging of patients in emergency room. Occasionally, patients are investigated for symptoms of suspected splenic pathology, such as abscess, infarct, symptomatic splenic artery aneurysm and pseudoaneurysm, splenic torsion, or rupture. More often, however, splenic emergencies, such as splenic masses and splenic vein thrombosis, are detected in patients in the emergency room during the evaluation of nonspecific abdominal pain. It is essential for radiologists to be vigilant in the identification of nontraumatic splenic emergenc…

Oncological emergencies: syndrome of inappropriate antidiuretic hormone secretion (SIADH).

Excessive secretion of vasopressin in the course of Syndrome of Inappropriate Antidiuretic Hormone Secretion is a common cause of hyponatremia in cancer patients. Clinical symptoms depend on the cause, rate of change of sodium level and their absolute values. Treatment options include fluid restrictions, intravenous administration of hypertonic sodium chloride solutions, loop diuretics and vaptans. The sodium level should not be adjusted too fast, because it may lead to irreversible brain damage. The article presents pathophysiology, diagnostics and recommendations of management of this oncolo

Cross-sectional imaging of nontraumatic peritoneal and mesenteric emergencies.

Multiple nontraumatic peritoneal and mesenteric emergencies are encountered at imaging of patients in the emergency department. Peritoneal and mesenteric emergencies are usually detected in patients in the emergency department during evaluation of nonspecific abdominal pain. A high index of suspicion is required for the establishment of early diagnosis and aversion of life-threatening complications in cases of peritoneal carcinomatosis, nontraumatic hemoperitoneum, and peritonitis. A correct diagnosis of omental infarction, mesenteric adenitis, and mesenteric panniculitis helps patients primar…

Spine and spinal cord emergencies: vascular and infectious causes.

This content presents infectious and vascular spinal emergencies, including epidural abscess, nontraumatic epidural hematoma, vascular malformations, and spinal cord infarction. The spine is subjected to multiple potential insults, such as trauma, infection, ischemia, hemorrhage, tumor, inflammation, and degeneration. All of these processes can lead to the sudden onset of neurologic symptoms, such as motor weaknesses, bowel and bladder incontinence, and sensory changes. Therefore, prompt recognition of these entities is important to reverse or minimize potential neurologic injury. The authors …

Brazilian dentists' attitudes about medical emergencies during dental treatment.

The main objective of this study was to evaluate the prevalence of emergencies in dental practices and the prepared-ness and the training experience in cardiopulmonary resuscitation (CPR) of Brazilian dentists in dealing with emergencies. The volunteer participants in the study were 498 Brazilian dentists who were present at the 27th International Congress of Dentistry in São Paulo. The most prevalent emergency was presyncope (reported by 54.20 percent of respondents), followed by orthostatic hypotension (44.37 percent), moderate allergic reactions (16.86 percent), hypertensive crisis (15.06 p…

Renal and urologic emergencies in the HIV-infected patient.

Antiretroviral therapy has revolutionized the care of individuals infected with the human immunodeficiency virus (HIV) and has fundamentally altered the scope of the disease. Acute renal failure and chronic kidney disease from medication toxicity and comorbid noninfectious illnesses are just as likely today as end-organ injury from the virus itself. Chronic immunosuppression renders HIV-infected patients vulnerable to any of several unique urological infections not frequently seen in immunocompetent patients. A deeper understanding of renal and urological emergencies in the context of the HIV-…

Emergencies in diabetic patients in the primary care setting.

The diabetic patient poses special problems in the primary care setting. Symptoms that are relatively unimpressive on initial presentation, such as polyuria or dizziness, may actually be the beginning of serious medical complications. With careful evaluation and follow-up, some patients, such as those who have mild hypo- and hyperglycemia and certain infections, can be managed as an outpatients; however, many cardiovascular conditions, such as cardiac ischemia or limb-threatening peripheral vascular disease, require immediate transfer to an acute care facility. In all situations, close monitor

Peri-operative anaesthetic management of neonates for surgical emergencies.

Surgery of neonates can be fraught with hazards unless the anaesthesiologist is conversant with the physiologic and pharmacologic responses under anaesthesia and surgery. The principles of anaesthetic management for surgery of critically ill neonates with common congenital malformations are briefly discussed in this article.

Infectious disease emergencies: role of the infectious disease specialist.

The importance of infections for public health has become obvious during the last decades. Examples are emerging infections such as HIV/AIDS and severe acute respiratory syndrome, deliberate release of microorganisms, such as the anthrax episode in the USA, the increasing problems with organisms resistant to antimicrobial treatment, such as methicillin-resistant Staphylococcus aureus, and the threat of a new influenza pandemic with a case fatality rate similar to that in the 1918 outbreak. An effective response to infectious disease emergencies requires careful planning and establishment of re…
