The clinical spectrum of autoimmune congenital heart block.

Autoimmune congenital heart block (CHB) is an immune-mediated acquired disease that is associated with the placental transference of materna … anifestation of CHB, although some babies might develop other severe cardiac complications, such as endocardial fibroelastosis or valvular insufficiency, even in the absence of cardiac block. In this Review, we discuss the epidemiology, classification and management of women whose pregnancies are affected by autoimmune CHB, with a particular focus on the autoantibodies asso …

Centronuclear myopathy and cardiomyopathy requiring heart transplant.

Centronuclear myopathy has been extremely rarely associated with cardiomyopathy, which can lead to heart failure and premature death. We rep … diomyopathy. Histologic results revealed hypertrophic myocardiocytes, focal areas of infarction and endocardial fibroelastosis, most prominently in the left ventricle. Although cardiomyopathy is commonly associated with other childhood myopathies, to our knowledge, this is the youngest patient reported with centronuclear myopathy presenting with heart failure caused by card …

The role of endomyocardial biopsy in the diagnosis of cardiac disorders in infants and children.

Endomyocardial biopsies from 26 children were studied by light and electron microscopy. Follow-up biopsies were obtained in 2 patients. The biopsy showed evidence of myocarditis in 6 patients, including 1 with features of sarcoidosis. Twelve biopsies showed features consistent with dilated cardiomyopathy. Endocardial fibroelastosis was documented in two of these biopsies. In two biopsies striking myocardial hypertrophy with prominent myofibrillar disarray suggested hypertrophic cardiomyopathy. Two biopsies from patients with clinical hypertrophic cardiomyopathy showed ultrastructural evidence …

Atrial Cardiomyopathy Through the Pathologist's Lens: From Tissue Remodeling to Clinical Expression.

Atrial cardiomyopathy (ACM) provides a useful framework for understanding structural, architectural, contractile, and electrophysiological d … pectrum includes cardiomyocyte hypertrophy and myocytolysis, interstitial and replacement fibrosis, endocardial fibroelastosis, adipose and inflammatory infiltration, vascular injury, and amyloid deposition. Structural remodeling is regionally heterogeneous, and appendage specimens may underrepresent abnormalities elsewhere in the atrium. Histologic interpretation requires …

An ALMS1 variant disrupts proximal centriole organization and promotes a myofibroblast-like phenotype that is regulated by THY1.

Primary endocardial fibroelastosis (pEFE) is characterized by accumulation of elastin-rich extracellular matrix (ECM) in the left ventricular endocardium in the absence of structural defects, but its pathophysiology remains unclear. We investigated dermal fibroblasts from a pEFE patient (proband) harboring a loss-of-function ALMS1 variant. Compared with control fibroblasts, proband cells displayed a myofibroblast-like phenotype, with increased ECM proteins and markers of fibroblast activation and endothelial-to-mesenchymal transition. Conversely, the thymus cell surface antigen (THY1) was down…

Prenatal Diagnosis of an Uncommon Form of a Hypoplastic Left Heart Syndrome Variant.

BACKGROUND: Mitral valve dysplasia syndrome (MVDS) is a rare cardiac defect requiring accurate prenatal identification and appropriate counseling, delivery planning, and postnatal management. CASE SUMMARY: A 39-year-old woman underwent fetal echocardiography at 34 weeks' gestation owing to cardiomegaly and hydrops. Biatrial enlargement, mild to moderate atrioventricular valve insufficiency, dysplastic mitral valve apparatus, biventricular dilation with systolic dysfunction, endocardial fibroelastosis, hypoplastic aortic outflow, and severely restrictive atrial septum were attributed to MVDS. …

Congenital complete heart block and maternal connective tissue disease.

Congenital complete heart block can be isolated or can occur in association with other structural heart diseases. Isolated congenital comple … cts in the fetal conduction system. Cardiac manifestations reported include dilated cardiomyopathy, endocardial fibroelastosis and mitral insufficiency. Low ventricular rate, QT prolongation and arrhythmias on monitoring are high risk features. CCHB has a mortality of 30%, and 60% of infants require pacemaker therapy. Fetal echocardiography is useful in early diagnosis. Pro …

Pitfalls in the diagnosis of left ventricular hypertrabeculation/non-compaction.

Left ventricular hypertrabeculation/non-compaction (LVHT) is a cardiac abnormality, characterised by >3 trabeculations apically to the papil … and misdiagnosed as dilated (n = 20), hypertrophic (n = 14) or restrictive cardiomyopathy (n = 2), endocardial fibroelastosis (n = 5), endomyocardial fibrosis (n = 1), myocarditis (n = 3), thrombus (n = 2), localised left ventricular hypertrophy (n = 1), left ventricular mass (n = 1) or myocardial/pericardial disease (n = 1). In 14 patients, LVHT was diagnosed only by tran …

Left ventricular dysfunction and massive intracavitary thrombus in unrepaired tetralogy of Fallot with endomyocardial fibroelastosis: a case report.

Left ventricular dysfunction and thrombosis in unrepaired tetralogy of Fallot are rarely reported. We report the case of a 9-year-old boy with unrepaired tetralogy of Fallot, severe biventricular dysfunction, large left ventricular thrombus, and diffuse subendocardial fibroelastosis. After surgical repair, recovery of ventricular function and thrombus resolution were observed. This case highlights the interplay between cyanosis, myocardial remodelling, and hypercoagulability and emphasises the role of multimodality imaging.

[Doppler-echocardiographic examination of patients with pulmonary tuberculosis].

Echocardiography of 110 newly diagnosed patients with pulmonary tuberculosis was undertaken to find anatomic changes and certain blood flow indices in the right heart. These changes were represented by an enlarged chamber of the left ventricle in over half of the patients and that of the left atrium in 1/3 of them (including myocardial hypertrophy also detected most commonly in the ventricular septum in 1/3 of the cases); fibroplastic changes in the anterior wall and ventricular septum; and induration of the cusps and semilunums of the tricuspid and pulmonary valves with subvalvular structures

Fatal Lyme carditis and endodermal heterotopia of the atrioventricular node.

A fatal case of Lyme carditis occurring in a Suffolk farmworker is reported. Post-mortem examination of the heart showed pericarditis, focal myocarditis and prominent endocardial and interstitial fibrosis. The additional finding of endodermal heterotopia ('mesothelioma') of the atrioventricular node raises the possibility that this could also be related to Lyme infection and account for the relatively frequent occurrence of atrioventricular block in this condition. Lyme disease should always be considered in a case of atrioventricular block, particularly in a young patient from a rural area. T

Management of critical valvular outflow obstruction in neonates.

Severe pulmonary or aortic valve stenosis may be lethal to the neonate if untreated. Two patients suffering from pulmonary stenosis and four with aortic stenosis were relieved surgically with the use of full cardiopulmonary bypass. Both neonates with pulmonary stenosis did well, making them the youngest recorded survivors of such treatment. Two of the four patients with aortic stenosis have done well, the other two having severe endocardial fibro-elastosis. The indications and technique for cardiopulmonary bypass in this age group are briefly described.
