Correlating audiological results with location of intracochlear schwannoma assessed by routine and Hydrops MRI.

BACKGROUND: Whether an intracochlear schwannoma (ICS) concomitant with endolymphatic hydrops (EH) remains unclear. OBJECTIVE: This study had two objectives: first, to correlate the location of ICS with audiological results using routine MRI; and second, to determine whether an ICS is associated with EH using Hydrops MRI. METHODS: Four cases with ICS demonstrated by routine MRI underwent an inner ear test battery, followed by 3 T Hydrops MRI. RESULTS: Audiometry revealed total deafness in 2 patients, flat-type hearing loss in one and 2 kHz dip type hearing loss in 1. Routine MRI demonstrated…

Monitoring of the glycerol test with sinusoidal harmonic acceleration in Menière's disease patients.

Changes in auditory pure tone threshold and speech discrimination and the phase and gain in the sinusoidal harmonic acceleration test were measured in subjects with endolymphatic hydrops (Meniere's disease) following the administration of glycerol. Although all subjects showed a marked improvement in auditory function, a comparable change in vestibular function, as measured by the sinusoidal harmonic acceleration test, was not observed.

Ménière's disease.

Ménière's disease affects about 1% of the population. Typically it presents as episodic vertigo, fluctuating hearing loss, tinnitus and aural fullness. The history and physical examination are critical in making the diagnosis. Laboratory tests are useful in tailoring and following up patients. Treatment options are limited and usually targeted towards reducing endolymphatic hydrops to stabilise the hearing loss and reduce the vertiginous episodes.

Magnetic resonance imaging for Ménière's disease: correlation with tone burst electrocochleography.

The newly developed use of magnetic resonance imaging of the human inner ear, on a 3 Tesla scanner with intratympanically administered gadolinium, can now reliably distinguish perilymph from endolymph and visually confirm the presence or absence of endolymphatic hydrops. Transtympanic tone burst electrocochleography is an established, and under-utilised evoked response electrophysiological test for hydrops, but it relies on a symptom score to indicate the likelihood of hydrops being present. The current diagnostic criteria for Ménière's disease make no allowance for any in vivo test, making di…

The effects of pressure on cochlear microphonics in experimentally induced hydropic ears in the guinea pig.

The effect of perilymphatic pressure on the cochlear microphonics (CM) threshold was examined in guinea pigs with experimentally induced hydrops. When a pressure of 300 mm H2O was applied to the scala tympani of the basal turn, the 500 Hz CM threshold shifts in ears during the 1st week following obliteration of the endolymphatic sac and duct were significantly smaller when compared with shifts in normal ears. Larger threshold shifts were then regained at 4-12 weeks after surgery. When acetazolamide was administered to hydropic animals, the effect of the increase of perilymphatic pressure on th

[Advances in diagnosis and treatment of Meniere's disease].

Meniere's disease (MD), a kind of common disease of otology, is based on the endolymphatic hydrops. The clinical features of MD are intermittent episodes of vertigo, fluctuating sensorineural hearing loss, tinnitus and ear fullness. With the in-depth exploration of the disease, the diagnosis and treatment of MD has made a series of research results. In this paper, the related literature and research reports in recent years were reviewed.

Shy-Drager syndrome presenting as Menière's disease.

A 59-year-old patient with a history of Menière's disease is presented. She developed signs of progressive autonomic failure suggesting a diagnosis of Shy-Drager syndrome, a rare degenerative neurologic condition of unknown etiology. The most common otolaryngologic presentation of this syndrome is with stridor attributable to abductor paralysis of the vocal cords. The association with endolymphatic hydrops has not been previously described.

Perfusion of the inner ear with streptomycin.

Any medical or surgical treatment of Meniere's disease is always suspect because the natural history is so variable and the pathologic physiology is not well-known. Even the occurrence rate in one and both ears is not known. It is certainly true that endolymphatic hydrops, caused by the failure of absorption of endolymph in the endolymphatic sac, is the principal pathology in the classical, fully developed Meniere's disease. While the endolymphatic shunt operation is correct in theory, in most ears, it does not work because the surgeon does not always locate the lumen of the sac and when locat…

Inner Ear Schwannoma Treated With Intratympanic Gentamicin for Recurrent Ménière's Disease-Like Vertigo: A Case Report.

Inner ear schwannoma (IES), also known as intralabyrinthine schwannoma, is a rare benign tumor typically presenting with hearing loss, tinnitus, and vestibular symptoms. In some cases, vestibular manifestations mimic Ménière's disease, characterized by recurrent vertigo attacks in which endolymphatic hydrops has been implicated in the underlying pathophysiology. Herein, we report the case of a 44-year-old woman with left-sided IES who developed intractable recurrent vertigo. Despite profound hearing loss in the affected ear, vestibular function testing revealed residual activity in all three s…

Changes in hearing function and intracochlear morphology after electrode array insertion in minipigs.

BACKGROUND: In temporal bone specimens from long-term cochlear implant users, foreign body response within the cochlea has been demonstrated … t four weeks postoperatively, a fibrous sheath formed around the electrode. At each time point, the endolymphatic hydrops was found; no significant changes in the morphology and packing density of the spiral ganglion neurons were observed. CONCLUSIONS AND SIGNIFICANCE: The effect of electrode array insertion on hearing and intracochlear fibrosis was significant. The p …

Effects of MRI contrast agents (Omniscan) on vestibular end organs.

CONCLUSION: These findings indicate that application of 4-16-fold diluted Omniscan could have depressant effects on the frog vestibular end organs. Based on cochlear studies on cytotoxicity using 8-fold diluted Omniscan, 16-fold diluted Omniscan is optimal for the inner ear and its application could be useful for visualization of endolymphatic hydrops as well as the control of vertiginous attacks in Meniere's disease. OBJECTIVES: Endolymphatic hydrops could be visualized recently by intratympanic injection of Omniscan diluted 8- or 16-fold with saline using three-dimensional fluid attenuated …

MR imaging of the cochlear modiolus after intratympanic administration of Gd-DTPA.

PURPOSE: We evaluated whether enhancement of the cochlear modiolus could be visualized 24 hours after intratympanic injection of gadolinium diethylenetriamine penta-acetic acid (Gd-DTPA) using a 3-dimensional real inversion recovery sequence combined with a 32-channel head coil at 3 tesla. Intratympanic injection of Gd-DTPA has been reported for visualizing endolymphatic hydrops in Meniere's disease, and its use has shown communication between the cochlear perilymph and cerebrospinal fluid in the internal auditory canal. Although the cochlear modiolus has been considered the route for this com…

Glycerol affects vestibular evoked myogenic potentials in Meniere's disease.

OBJECTIVES: to show that abnormal vestibular evoked myogenic potentials on the sternocleidomastoid muscle (SCM) in patients with unilateral Meniere's disease are caused by endolymphatic hydrops. SUBJECTS: six normal volunteers and 17 patients with unilateral Meniere's disease were examined. METHODS: click-evoked myogenic potentials were recorded with surface electrodes over each SCM. Responses evoked by clicks recorded after oral administration of glycerol (1.3 g/kg body weight) were compared with those recorded before administration. RESULTS: the change rate of the p13-n23 amplitude was ca…

Reissner's membrane in aging, Menière's disease, and profound sensorineural deafness.

The characteristics of Reissner's membrane from 47 human cochleae with mild endolymphatic hydrops, profound sensorineural deafness and normal ears were studied by light microscopy. The highest cell densities were observed in the zones adjacent to the limbus spiralis and the stria vascularis. The cell density of Reissner's in normal ears decreased with age concomitant with an increased formation of epithelial cell clusters. In hydrops, the density increased with the exception of the apical turn. However, sporadic loss of the cells in isolated areas of the membrane was observed. The ears with pr…

Clinical management of Menière's disease: Clinician perspective in 2020.

Menière's disease (MD) still presents both diagnostic and therapeutic difficulties. Today, this pathology is diagnosed only on clinical criteria. The development of high resolution magnetic resonance imaging of the inner is very promising to improve diagnostic criteria in MD. MD treatment depending on the practitioner and the clinical center, is mainly based on conservative therapies, and if this fails, non-ablative or ablative therapies. MD therefore always exposes clinicians to diagnostic uncertainties, but also to therapeutic difficulties which still lead to destructive treatments, in the a
