Low accuracy of the national reporting system of acute hepatitis C infection in Taiwan, 1995-2004.

BACKGROUND AND AIM: This study attempted to clarify accuracy of acute hepatitis C (AHC) and its clinical characteristics. METHODS: We reviewed 632 reported cases from national surveillance data of the Taiwan Center for Disease Control between 1995 and 2004, and reclassified diagnoses. A definite case was defined as alanine aminotransferase (ALT) > 10 x the upper limit of normal (ULN) with seroconversion of anti-hepatitis C virus antibody (anti-HCV). A probable case was defined as (i) seroconversion of anti-HCV and/or elevated ALT levels; or (ii) anti-HCV(+) but titers increased (from < 40 S/C

Understanding physicians' risk stratification of acute coronary syndromes: insights from the Canadian ACS 2 Registry.

BACKGROUND: An important treatment-risk paradox exists in the management of acute coronary syndromes (ACSs). However, the process of risk stratification by physicians and its relationship to the management of ACS have not been well studied. Our objective was to examine patient risk assessment by physicians in relation to treatment and objective risk score evaluation and the underlying patient characteristics that physicians consider to indicate high risk. METHODS: The prospective Canadian ACS 2 Registry recruited 1956 patients admitted for non-ST-segment elevation ACS in 36 hospitals in Octob

Sampling strategies in nested case-control studies.

A stratified version of nested case-control sampling which we call "countermatching" is presented. This design uses data available for all cohort members to obtain a sample for collecting additional information in a case-control substudy. Hitherto the only stratified sampling design for such studies has involved matching of controls to cases with respect to confounding variables. However, in some situations, rather than sampling to make controls as similar as possible to cases, we might wish to make them as different as possible. This is achieved by the counter-matched design. Statistical anal

Update in clinical science: Autoimmune hepatitis.

Autoimmune hepatitis (AIH) is an enigmatic, relatively rare disease with a variable spectrum of presentation whose pathogenesis, diagnosis and management remain a major challenge. We have performed a review of recent developments in basic science, epidemiology, clinical science, therapeutics, and regulatory science, evaluating the challenges associated with the application of translational research and clinical trial design to a condition that is a chameleon in nature, where outcomes range from relatively benign disease through cirrhosis and acute liver failure. This review is focused on devel

Peripheral nerve vulnerability as risk factor for multiple sclerosis: Possible insights from Neurofibromatosis type 2 (NF2).

Despite enormous research efforts, the disease mechanisms causing multiple sclerosis (MS) are still incompletely understood, suggesting a complex and multifaceted pathogenesis. Here, we report the clinical observation that in a designated German center for Neurofibromatosis type 2 (NF2), the number of MS cases among NF2 patients is higher than in the general population. Epidemiological studies investigating a connection between NF2 and MS are difficult to perform due to the rarity of NF2 disease. However, based on the current pathophysiological concepts, we hypothesize that genetically determi

Yellow fever risk assessment in the Central African Republic.

Yellow fever still causes high burden in several areas of sub-Saharan Africa and Latin America. There are few well-designed epidemiological studies and limited data about yellow fever in Africa. Staples et al., in a recently published paper in Transactions of the Royal Society of Tropical Medicine & Hygiene, performed a nationwide study in the Central African Republic (CAR) assessing infection risk and the operational impact of preventive measures. The rapid assessment of human, non-human and mosquito data call attention to the potential risk of future yellow fever outbreaks in the CAR and els

CoxBase: an Online Platform for Epidemiological Surveillance, Visualization, Analysis, and Typing of Coxiella burnetii Genomic Sequences.

Q (query) fever is an infectious zoonotic disease caused by the Gram-negative bacterium Coxiella burnetii. Although the disease has been studied for decades, it still represents a threat due to sporadic outbreaks across farms in Europe. The absence of a central platform for Coxiella typing data management is an important epidemiological gap that is relevant in the case of an outbreak. To fill this gap, we have designed and implemented an online, open-source, web-based platform called CoxBase (https://coxbase.q-gaps.de). This platform includes a database that holds genotyping information on mor

Aligning valid research outcomes with stakeholder values-what do they need for decision-making?

This paper is derived from a presentation given by the first author at the 2024 Symposium for the Calvin Schwabe Award, presented to Dr. Jan Sargeant for Lifetime Achievement in Veterinary Epidemiology and Preventive Medicine. Researchers must work toward ensuring validity throughout the research process, but we also should ensure that our resulting outcomes are specified to appropriately inform and enable decision-making by the end-users. Given the scope and diversity of topics addressed by veterinary researchers, the potential beneficiaries or stakeholders of our research also varies. Stakeh

Serum high-density lipoprotein cholesterol, metabolic profile, and breast cancer risk.

BACKGROUND: The prevalence of metabolic syndrome (obesity, glucose intolerance, low serum high-density lipoprotein cholesterol [HDL-C], high serum triglycerides, hypertension) is high and increasing in parallel with an increasing breast cancer incidence worldwide. HDL-C represents an important aspect of the syndrome, yet its role in breast cancer is still undefined. METHODS: In two population-based screening surveys during 1977-1983 and 1985-1987, serum HDL-C was assayed enzymatically among 38,823 Norwegian women aged 17-54 years at entry. Height, weight, blood pressure, serum lipids, fat and

The Relationships Between the Eco-Bio-Social Determinants of Dengue Epidemiology in Latin America and the Caribbean: A Scoping Review of the Literature.

The factors influencing dengue virus (DENV) transmission by the Aedes aegypti mosquito vector in Latin America and the Caribbean (LAC) are complex and related. The use of a systems thinking, Ecohealth approach for understanding the dynamic linkages between all the ecological, biological, and social (eco-bio-social) factors associated with DENV epidemiology is imperative to better understand the DENV epidemiological system (episystem). The aim of this scoping review was to summarize the evidence for the relationships between the eco-bio-social factors influencing DENV vector dynamics, transmiss

Cutaneous leishmaniasis: aetiopathogenesis, epidemiology, clinical features, management and research gaps.

Cutaneous leishmaniasis (CL) is a protozoan parasitic infection caused by Leishmania species and transmitted through the bite of infected sandflies. The disease is endemic in tropical, subtropical and Mediterranean regions, where it presents as localized skin ulcers, often leaving scars that can lead to significant morbidity and social stigma. This review focuses on the aetiopathogenesis, epidemiology, clinical features, management strategies and research gaps in CL. Understanding the complex immune response involved in the pathogenesis of CL, as well as the environmental and socio-economic fa

Understanding the Intransigence of Malaria in Malawi.

Despite the scale-up of interventions against malaria over the past decade, this disease remains a leading threat to health in Malawi. To evaluate the epidemiology of both Plasmodium falciparum infection and malaria disease, the Malawi International Center of Excellence for Malaria Research (ICEMR) has developed and implemented diverse and robust surveillance and research projects. Descriptive studies in ICEMR Phase 1 increased our understanding of the declining effectiveness of long-lasting insecticidal nets (LLINs), the role of school-age children in malaria parasite transmission, and the co

More Than Seventy Years of Research (1948-November 2021) on Toxoplasma gondii in Iran: A Narrative Review.

In this review, we intend to provide a summary of the activities of researchers in the field of Toxoplasma gondii in Iran, during the past 70 years. Most studies have been limited to epidemiological studies (mostly using ELISA and IFA methods). Designing a standard and reliable method using the specific antigens of this parasite is essential. So far, studies in the field of drug effects have not been able to introduce an effective drug with few side effects. Various types of vaccines have been developed, such as recombinant and DNA vaccines. However, none of them had a good efficacy. The use o

Pb enamel biomarker: deposition of pre- and postnatal Pb isotope injection in reconstructed time points along rat enamel transect.

Exposure to lead (Pb) as well as other heavy metals in the environment is still a matter of public health concern. The development of the enamel biomarker for heavy metal exposure assessment is designed to improve studies of dose-effect relationships to developmental anomalies, particularly embryonic dysfunctions, and to provide a time-specific recount of past exposures. The work presented in this paper demonstrates maternal transfer across the placental barrier of the enriched isotope (206)Pb tracer to the enamel of the rat pup. Likewise, injections of (204)Pb-enriched tracer in the neonate r
