[Difficult intubation due to an unusual anomaly of the larynx].

A patient, scheduled for an ophthalmic operation could not be intubated in spite of various different attempts. The cause of the difficulties was a cartilagineous thickening of the right ary-epiglottic fold, which was lying like a second epiglottis over the entry of the glottis. The importance of ventilation-management by mask and close cooperation with a laryngologist is stressed.

Invasive Haemophilus influenzae Type b (Hib) Infection in a Fully Vaccinated Child: A Case Report.

BACKGROUND: Invasive Haemophilus influenzae type b (Hib) infection in children has become rare following the introduction of highly effective conjugate vaccines under national immunisation programmes. However, breakthrough invasive infections in fully immunised individuals can still occur. We report a case of invasive Hib infection presenting as epiglottitis and bacteraemia in a fully vaccinated 5-year-old boy in regional Queensland, Australia. CASE PRESENTATION: The child, with no history of immunodeficiency, presented with a 3-day history of fever, progressive throat pain and reduced oral i…

Presentation with gastrointestinal symptoms and high case fatality associated with group W meningococcal disease (MenW) in teenagers, England, July 2015 to January 2016.

Atypical clinical presentations associated with group W meningococcal disease (MenW) are well-described and include pneumonia, septic arthritis, endocarditis and epiglottitis/supraglottitis. Following anecdotal reports of teenagers presenting with predominantly gastrointestinal symptoms, we undertook a case review of MenW cases in 15 to 19 year-olds diagnosed in England between July 2015 and January 2016. Of the 15 cases, seven presented with a short history of nausea, vomiting and diarrhoea; five of these seven cases died within 24 hours of presentation to hospital.

[Primary HIV infection. Case histories].

More than 50% of individuals with primary HIV infection have a symptomatic febrile illness associated with seroconversion. The most common clinical picture is a mononucleosis-like or influenza-like illness but almost any organ system may be involved, and primary HIV infection is often an imitator of other acute diseases. Six cases of symptomatic primary infection are described, one with a characteristic presentation, and five cases respectively presenting with colitis, rhabdomyolysis, epiglottitis, cold agglutination haemolysis and meningitis as marked symptoms.

Hemophilus parainfluenzae prostatitis in a homosexual man with chronic lymphadenopathy syndrome and HTLV-III infection.

Hemophilus parainfluenzae, a common upper respiratory tract pathogen, has been reported to cause pharyngitis, epiglottitis, otitis media, conjunctivitis, and pneumonia. Rarely H. parainfluenzae infects the urinary tract, and is believed not to have been previously reported as a cause of prostatitis. A case of H. parainfluenzae in a young homosexual man infected with HTLV-III and chronic lymphadenopathy is described. Common clinical syndromes such as prostatitis may be associated with unusual pathogens in persons immunodeficient due to HTLV-III infection.

Invasive Haemophilus influenzae disease in adults.

In a five-year period, 29 cases of bacteremia and/or meningitis in adults caused by Haemophilus influenzae were seen in our large community hospital. There were 17 cases of bacteremic pneumonia and 12 cases of serious extrapulmonary infections. The extrapulmonary infections included cases of endocarditis, meningitis, cholecystitis, epiglottitis, tubo-ovarian abscess, and cellulitis. In contrast with the pediatric experience, H influenzae type B was the causative pathogen in only 45% of patients and only one isolate was ampicillin resistant.

Severe Haemophilus influenzae infections.

From May 1973 thorugh April 1974, a total of 83 children with severe Haemophilus influenzae infections were treated in three Denver pediatric hospitals. Although meningitis was the most common clinical manifestation (45 cases), other foci of infection were also noted (pneumonia, 12 cases; cheek cellulitis, eight cases; epiglottitis, eight cases; empyema, seven cases; pericarditis, three cases; arthritis, one case; periorbital cellulitis, one case; and abscess, one case). Nine children had positive blood cultures with H influenzae without an initial detectable focus of infection. Two patients d…

Anomalies in an infant with Nager acrofacial dysostosis.

We report on an infant with Nager acrofacial dysostosis, laryngeal and epiglottic hypoplasia, abnormal septation of the right middle lobe of the lung, hypoplastic right first rib, and dislocation of the right hip. These findings suggest the possibility that patients with the Nager syndrome may have other developmental defects in addition to the facial and acral anomalies associated with this syndrome.

Catastrophic Hemoptysis in Concurrent Laryngeal and Endobronchial Tuberculosis in an Immunocompetent Host: Survival Following Brief ECMO Support.

We present a case of a 45-year-old previously healthy Filipino domestic helper who presented with life-threatening hemoptysis due to disseminated tuberculosis (TB) involving the larynx, tracheobronchial tree, and pulmonary parenchyma. She presented with acute respiratory distress from massive hemoptysis, necessitating emergent intubation and subsequent veno-venous (V-V) extracorporeal membrane oxygenation (ECMO) support. Initial chest radiography revealed bilateral upper lobe infiltrates and right middle lobe collapse. Bronchoscopic evaluation demonstrated active haemorrhage and airway obstruc

Laryngeal features of external superior laryngeal nerve denervation: revisiting a century-old controversy.

A long-standing controversy exists regarding the laryngoscopic features associated with unilateral denervation of the external superior laryngeal nerve (ESLN). Recently, we modeled acute unilateral cricothyroid muscle paralysis by blocking the ipsilateral ESLN with lidocaine hydrochloride, and identified epiglottic petiole deviation to the side of paralysis during high-pitched voice production as a possible diagnostic sign. This study provides preliminary clinical evidence supporting the presence of petiole deviation in cases of ESLN denervation. Epiglottic petiole deviation to the side of wea

The correlation of cervical symptoms with endotracheal intubation quality and preoperative airway assessment with radiological methods: A prospective observational study.

Airway management can be challenging for patients undergoing cervical spinal surgery. This study aimed to evaluate how the duration of the cervical pathology associated with neck pain and numbness affects airway anatomy and airway management, and to correlate radiological measurements. This was a single-center prospective observational study. Eighty-two patients with American Society of Anesthesiologists I to III were selected from those undergoing surgery for cervical pathology. The patients' symptoms and duration of cervical disease were assessed. Skin-epiglottis, skin-hyoid, epiglottis-voca

Aspecific membranous laryngitis after infectious mononucleosis.

Aspecific membranous laryngitis is an unusual but very serious complication of viral infections. Here, we report the uncommon finding of infectious mononucleosis characterized by aspecific membranous laryngitis with fever, dysphonia and severe dyspnea in a 12-year-old girl. Endoscopy showed mucopus and sloughed epithelium forming a pseudomembrane covering almost all the supraglottal region and a supraglottal swelling including the epiglottis and arytenoids. The importance of suspecting diphtheria, epiglottitis, viral or bacterial croup and laringo-tracheo-bronchitis and including them in the d…

A rudimentary epiglottis associated with Pierre Robin sequence.

Epiglottis anomaly associated with Pierre Robin sequence (PRS) is a rare occurrence. To the knowledge of the authors, this is the first reported case of epiglottic anomaly associated with PRS. Doctors should remain aware of this atypical presentation in a PRS patient, and the presented case highlights the clinical challenges that must be met to ensure effective treatment of the defect in terms of compromised respiration, swallowing and feeding.
