Temporal lobectomy in bilateral temporal lobe epilepsy: A relook at factors in selection, invasive evaluation and seizure outcome.

OBJECTIVES: We sought retrospectively to assess outcomes in bilateral temporal lobe epilepsy (BTLE) patients undergoing resective temporal lobe surgery following invasive EEG lateralization in comparison to unilateral TLE (UTLE). METHODS: Patients undergoing surgical intervention for TLE (the vast majority being mesial temporal lobe epilepsy) during a 7-year period were retrospectively categorized as suspected BTLE (sBTLE) or UTLE. Temporal lobectomy was performed in the sBTLE group on the side of maximum number of intracranial EEG seizure onsets. EEG, MRI, laterality of iEEG ictal onset (in

Facial asymmetry in patients with temporal lobe epilepsy. A clinical sign useful in the lateralization of temporal epileptogenic foci.

While a clinical history is frequently sufficient for a localizing diagnosis of temporal lobe epilepsy, lateralization of the epileptogenic abnormality often is impossible on clinical grounds alone. Since we have noted facial asymmetry in such patients, 50 individuals with temporal lobe epilepsy were studied. In patients with unilateral foci, we found contralateral lower facial weakness of mild to severe degree in 73 percent while 13 percent had ipsilateral weakness, and in 13 percent the face was symmetrical. The facial weakness usually was more striking on emotional movement. In the group of

Feline hippocampal necrosis and temporal lobe epilepsy: A review of current literature and definitions.

Feline hippocampal necrosis (FHN) is a histopathological diagnosis first identified ∼25 years ago in domestic cats, and characterised by marked neuronal necrosis largely restricted to the hippocampus. FHN is often conflated with an overlapping condition, hippocampal sclerosis (HS). FHN does not represent a singular neurological condition, but rather a common endpoint of several disease entities including neoplasia, vascular disease and autoimmunity. The latter of these has been of particular interest in recent years with the discovery of limbic encephalitis and FHN associated with autoantibodi

Impact of disease duration and surgical intervention on arousal networks in temporal lobe epilepsy.

OBJECTIVE: Epilepsy is a common neurological disease affecting nearly 1% of the global population, and temporal lobe epilepsy (TLE) is the most common type. Patients experience recurrent seizures and chronic cognitive deficits that can impact their quality of life, ability to work, and independence. These cognitive deficits often extend beyond the temporal lobe and are not well understood. It has been proposed in the extended network inhibition hypothesis that repeated spread of seizure activity to the ascending reticular activating system (ARAS) may contribute to these deficits. Disease durat

Individualized Functional and Structural Language Lateralities in Temporal Lobe Epilepsy and Their Impact on Memory.

The basis and impact of functional asymmetries in the brain, particularly language lateralization, are not fully understood, and the relationship between functional and structural asymmetries remains largely untested. This study investigated the degree to which asymmetries in hemispheric language laterality are concordant with asymmetries in gray matter (GM) structure and whether the hemispheric organization of memory is influenced by functional language asymmetries. Structural and functional MR data was acquired from 261 individuals, including those with unilateral temporal lobe epilepsy (LTL

Induced Overexpression of Connexin43 in Astrocytes Attenuates the Progression of Experimental Temporal Lobe Epilepsy.

Astrocytic gap junctional communication plays a critical role in regulating neuronal activity and network synchronization, yet its precise contributions to brain function and the pathogenesis of neurological disorders remains incompletely understood. To address this, we generated a transgenic mouse line with inducible, astrocyte-specific overexpression of the gap junction protein connexin43 (Cx43). In these mice, hippocampal astrocytes exhibited markedly elevated Cx43 protein levels and a ~ 20% increase in intercellular gap junction coupling. Enhanced coupling was accompanied by a reduction in

Influence of the estrous cycle on seizure activity in a model of mesial temporal lobe epilepsy.

Catamenial epilepsy is characterized by seizure exacerbation during specific phases of the menstrual cycle and affects up to 70 % of epileptic women. These seizures are often non-responsive to medication and our understanding of the relationship between menstrual cycle and seizure generation remains limited. Previous experiments using the in vitro 4-aminopyridine model of epileptiform synchronization demonstrated that the proestrus and estrus, which are associated to high blood estrogen levels, favor seizure generation; this effect presumably results from enhanced parvalbumin (PV)-positive int

Electroencephalogram microstate analysis in temporal lobe epilepsy: A comparative study with and without anxiety.

BACKGROUND: Anxiety is a prevalent psychiatric comorbidity in temporal lobe epilepsy (TLE), however, limited attention has been given to the dynamic characteristics of TLE coexisting with anxiety. The aim of this study was to examine abnormal resting-state Electroencephalography (EEG) microstate dynamics in TLE patients with anxiety disorders compared with TLE patients without anxiety disorders and healthy controls. METHODS: Ninety participants in this study: 30 healthy controls (HC), 30 TLE patients with anxiety (PAS), and 30 TLE patients without anxiety (nPAS). The dynamic changes in the ent

Effect of levetiracetam on DNA oxidation and glutathione content in a temporal lobe epilepsy model.

Levetiracetam (LEV) is a drug commonly used as an anticonvulsant. However, recent evidence points to a possible role as an antioxidant. We previously demonstrated the antioxidant properties of LEV by significantly increasing catalase and superoxide dismutase activities and decreasing the hydrogen peroxide (H2O2) levels in the hippocampus of rats with temporal lobe epilepsy (TLE) showing scavenging properties against the hydroxyl radical. The aim of the present work was to evaluate, the effect of LEV on DNA oxidation, by determining 8‑hydroxy‑2‑deoxyguanosine (8‑OHdG) levels, and glutathione co

Topological Embedding of Human Brain Networks with Applications to Dynamics of Temporal Lobe Epilepsy.

We introduce a novel, data-driven topological data analysis (TDA) approach for embedding brain networks into a lower-dimensional space in quantifying the dynamics of temporal lobe epilepsy (TLE) obtained from resting-state functional magnetic resonance imaging (rs-fMRI). This embedding facilitates the orthogonal projection of 0D and 1D topological features, allowing for the visualization and modeling of the dynamics of functional human brain networks in a resting state. We then quantify the topological disparities between networks to determine the coordinates for embedding. This framework enab

Auditory processing in patients with temporal lobe epilepsy: A systematic review and meta-analysis.

BACKGROUND: Hearing efficiency is known to influence and interact with communication and mental health. Hearing impairment may be hidden when co-occurring with neurological disorders. PURPOSE: We performed a systematic review and meta-analysis in order to address the following questions: 1) which specific tools of auditory processing show clear deficits, separating Temporal Lobe Epilepsy (TLE) patients from normal controls,2) How well is TLE evaluated in terms of hearing and auditory processing? METHODS: The study inclusion criteria were: 1) patients diagnosed with temporal lobe epilepsy, 2)

Overcoming Diagnostic Uncertainty: A Case of Temporal Lobe Epilepsy With Normal MRI and EEG.

Temporal lobe epilepsy (TLE) can present in a variety of ways and is challenging to diagnose. While MRIs are the gold standard for detecting structural lesions and EEGs can identify epileptiform discharges, relying solely on these diagnostic methods can result in underdiagnosis or misdiagnosis of the condition. Seizures with motor components may manifest as unusual behavior; anxiety, psychosis, and other psychiatric conditions can occur in conjunction with or independently of seizure activity. In cases where diagnostic testing yields negative results but the history strongly suggests seizure-l

Altered immune pathways in patients of temporal lobe epilepsy with and without hippocampal sclerosis.

Over the past decades, the immune responses have been suspected of participating in the mechanisms for epilepsy. To assess the immune related pathway in temporal lobe epilepsy (TLE), we explored the altered immune pathways in TLE patients with and without hippocampal sclerosis (HS). We analyzed RNA-seq data from 3 TLE-HS and 3 TLE-nonHS patients, including identification of differentially expressed RNA, function pathway enrichment, the protein-protein interaction network and construction of ceRNA regulatory network. We illustrated the immune related landscape of molecules and pathways on human

Association of ictal aphasia with hypoperfusion in language areas in temporal lobe epilepsy patients.

Ictal clinical phenomenology, including aphasia, is usually associated with increased regional cerebral perfusion. We present an unusual pattern of ictal cerebral perfusion in three patients with pharmacoresistant, lesional temporal lobe epilepsy and ictal/postictal aphasia studied with prolonged video-EEG, ictal, and interictal SPECT and MRI for pre-surgical evaluation. Subtraction of ictal-interictal SPECT images co-registered with MRI (SISCOM) showed ictal hyperperfusion in the temporal epileptogenic area in all patients. In addition, hypoperfusion of Broca's area in one case, Wernicke's ar

Is Hippocampal Resection Necessary for Low-Grade Epilepsy-Associated Tumors in the Temporal Lobe?

Low-grade epilepsy-associated tumors (LEATs) are common in the temporal lobe and can cause drug-resistant epilepsy. Complete resection of LEATs is sufficient for seizure relief. However, hippocampal resection might result in postoperative cognitive impairment. This study aimed to clarify the necessity of hippocampal resection for seizure and cognitive outcomes in patients with temporal lobe LEATs and a normal hippocampus. The study included 32 patients with temporal lobe LEATs and without hippocampal abnormalities. All patients underwent gross total resection as treatment for drug-resistant ep

Mossy fiber sprouting into the hippocampal region CA2 in patients with temporal lobe epilepsy.

Hippocampal sclerosis (HS) in Temporal Lobe Epilepsy (TLE) shows neuronal death in cornu ammonis (CA)1, CA3, and CA4. It is known that granule cells and CA2 neurons survive and their axons, the mossy fibers (MF), lose their target cells in CA3 and CA4 and sprout to the granule cell layer and molecular layer. We examined in TLE patients and in a mouse epilepsy model, whether MF sprouting is directed to the dentate gyrus or extends to distant CA regions and whether sprouting is associated with death of target neurons in CA3 and CA4. In 319 TLE patients, HS was evaluated by Wyler grade and Intern
