External fixation in the treatment of tibial pilon fractures: comparison of two frames in torsion.

BACKGROUND: Ankle spanning external fixation has become the initial treatment of choice for complex tibial pilon fractures. Many fixator designs exist, but their biomechanical performance has not been studied extensively for this application. The goal of the present study was to compare the torsional performance of two commercially available frames, the Orthofix XCaliber and the Howmedica Hoffmann II. METHODS: The XCaliber and the Hoffmann II were each applied to six fresh cadaver lower extremities and were loaded in a materials testing machine. Strain gauges were attached to the anteromedial

Effectiveness of the lateral unilateral dynamic external fixator after elbow ligament injury.

BACKGROUND: The optimum management of ligamentous injuries of the elbow is not known. Use of dynamic external fixators has been advocated to stabilize the joint while maintaining motion, but there are no published data to corroborate their efficacy. The purpose of this study was to test the hypothesis that a laterally applied unilateral dynamic external fixator is capable of stabilizing and restoring normal kinematics to elbows with varying degrees of soft-tissue injury. METHODS: Six fresh-frozen cadaveric upper extremities, from donors who were an average of seventy-six years of age at the t…

Arthroscopic-assisted reduction and percutaneous external fixation of lateral condyle fractures of the humerus.

Lateral condyle fractures in children are the second most common fracture about the elbow. Anatomic reduction of intra-articular displacement is considered a priority. Most experts agree that the treatment of choice of mildly displaced (>2 mm) lateral condyle fractures is open reduction-internal fixation or intraoperative arthrography followed by closed reduction and percutaneous pinning. In this report we described an alternative approach using closed arthroscopic-assisted reduction. Arthroscopy was performed by use of a 30 degree, 4.5-mm arthroscope through a proximal anteromedial portal wit

Arthrodesis with external fixation in the unstable or misaligned Charcot ankle in patients with diabetes mellitus.

The unstable or misaligned Charcot ankle with or without chronic foot ulceration is a major clinical challenge. When it cannot be accommodated with an ankle foot orthosis, surgical treatment is indicated in order to avoid leg amputation. This requires extensive soft tissue release and bony resection to realign the foot and arthrodesis with internal or external fixation. The guidance in the literature favors internal fixation. This article reports results with external fixation in 11 patients (12 feet) over a period of 12 years. External fixation was chosen as the surgical option because of the

External fixation of femoral fractures in multiply injured intensive care unit patients.

We report the results of a prospective study of 42 patients with multiple injuries, including femoral fractures, who required intensive care unit (ICU) admission and whose fractures were treated by means of external fixation. The Injury Severity Score (ISS) ranged from 18 to 41 and the average Glasgow Coma Scale (GCS) on admission was 12. Seventeen fractures were open. All patients had their fractures stabilised within 6 hours from admission by means of external fixation. After a follow-up of 11 months (range 4-20), 28 fractures had healed within 6 months (range 4.5-8) and 13 developed non-uni

[Constituted adult equine from the ankle: surgical treatment by Ilizarov external fixator].

INTRODUCTION: Ilizarov's external fixator indications are classically used in children to lengthen the lower limbs. This technique is used for adults to gradually correct articular limitations, especially in the knee, ankle and foot. METHOD AND RESULTS: We report a case of a 53-year-old patient with distal microangiopathy of the lower limbs leading to amputation of the toes and a bilateral, direct, irreducible, isolated equine feature? from the back foot to 0/50/60 degrees? on retraction of the Achilles tendon. The aim of patient management was to regain walking ability. Because of vascular a

The bridge and barrel hoop plates as alternatives to external fixation techniques in the foot and ankle.

External fixation is a useful tool for treating severe traumatic injuries to the foot and ankle. External fixation, however, does have associated complications, such as inadequate fixation and pin tract infections. Temporary bridge plating of the medial, lateral, or both columns is a useful alternative to external fixation for treating multilevel foot injuries. This article describes the technique of temporary bridge plating which may be used as an alternative to external fixation.

Treatment of distal femoral nonunions by external fixation with simultaneous length and alignment correction.

The use of external fixation for management of distal femoral nonunions may minimise some of the problems frequently encountered in these patients. Fifteen patients treated by external fixation for distal femoral nonunions between 1987 and 1997 were reviewed. There were nine males and six females. The average age was 35.4 years (17-53) with an average follow up of 4.6 years (2-8). Nine followed an open fracture, five a closed fracture and one a femoral osteotomy. Five of the cases were infected nonunions. In all cases an internal fixation device was used as the initial method of treatment. All

Temporary external fixation for the management of complex intra- and periarticular fractures of the lower extremity.

Complex intra-articular and periarticular fractures of the lower extremity are challenging injuries to treat. Recently, the concept of utilizing a staged approach with temporary external fixation, a delay to allow soft-tissue healing followed by open reduction and internal fixation, has gained popularity. Historically high complication rates appear to be significantly improved with this strategy. Temporary external fixation is also useful in other settings, such as in the initial management of polytraumatized patients with multiple complex fractures, augmentation of internal fixation in osteop

[Dorsal stabilization of fractures of the thoracic and lumbar spine by external fixator--technique and outcome].

Posterior stabilization by internal fixator is used as a frequent procedure for the surgical treatment of thoracolumbar spine fractures. The technique of internal fixator stabilization and its results regarding the correction of spinal posture and spinal canal clearance are described. By transpedicular spongiosal filling of the reduced vertebral body, a complete consolidation can be achieved. Occurring correction losses of the spinal alignment are mainly attributed to the collapse of intervertebral segments, thereby suggesting insufficient anterior fusion and support after transpedicular inter
