The treatment of fractures in osteopetrosis can be complicated and difficult. We describe the use of an industrial grade tungsten carbide drill bit in the treatment of one of these complex fractures. An industrial grade tungsten carbide drill bit was used to fashion a medullary canal in the surgical treatment of a left peri-implant fracture of the neck of femur in a patient with osteopetrosis. The patient was successfully treated with a hemiarthroplasty with good functional outcomes. A tungsten carbide drill bit serves as an effective and safe option in the treatment of osteopetrotic femoral n

Femur neck fractures are among the most common presentations seen in trauma centers. Surgical fixation of the fracture versus hip joint arthroplasty is the standard modality of treatment. The appropriate treatment modality is decided based on the patient's fracture pattern, activity level prior to the injury, fitness for surgery, and availability of resources. Total or hemi hip arthroplasties are effective and well-optimized procedures. However, they do not come without the risk of complications, some of which can cause significant morbidity to the patient. We present a case of a vascular comp…

BACKGROUND: Few studies have investigated the differences in proximal femoral geometry and risk factors between patients with different types of hip fracture, especially in elderly Chinese. This study aimed to assess the differences in proximal femoral geometry parameters between patients with femoral neck fractures and patients with intertrochanteric fractures to provide guidance for individualized customized prosthesis and accurate reconstruction of proximal femurs in elderly Chinese patients.
METHODS: We retrospectively studied the electronic medical records of 198 elderly patients over 65

OBJECTIVES: Clinically, it is very difficult to prevent pathological fracture caused by high recurrence rate of osteolytic disease of proximal femur in children. At present, there is no consensus in clinical studies of which internal fixation method can significantly reduce the probability of recurrence of pathological fracture. The study aims to research the mechanical properties of different internal fixations in the treatment of osteolytic lesions of proximal femur in children by finite element analysis, and to find out the optimal treatment.
METHODS: Based on finite element analysis, the

BACKGROUND: Comparison of 4 cannulated lag screws (3 inverted triangular cannulated screws + anti-rotating screws;4 CLS), dynamic hip screws + derotational screws (DHS + DS), and femoral neck fixation system (FNS) in the treatment of Biomechanical properties of middle-aged Pauwels type III femoral neck fractures.
METHODS: The femur CT data of a healthy young volunteer was selected and imported into Mimics software to construct a three-dimensional model of a normal femur. Pauwels type III femoral neck fractures were simulated according to the 70° fracture line. Use Geomagic and SolidWorks soft

Many theories have been advanced to explain the final neck shaft angle in the developing proximal femur. Muscle spasticity and delayed weight-bearing are the postulated causes of the development of the valgus deformity in patients with neuromuscular disease. Three patients with coxa vara exhibited some degree of spasticity and delayed weight-bearing. In spite of this, each child developed a varus rather than a valgus hip deformity. Existing theories seem not to adequately explain the development of the final neck shaft angle.

Loss of architecture within the femoral neck due to ageing follows a distinct pattern. This may serve as an index of physiologic ageing of bone. In pathologic osteoporosis (e.g. Cushing's disease, thyreotoxicosis and idiopathic osteoporosis) this systematic reduction of trabecular structure cannot be seen in the femoral neck although the vertebral bodies show all the signs of severe osteoporosis. Radiographic comparison between the proximal end of femur and the spine may, therefore, serve as a useful means of differentiation between pathologic and age-osteoporosis.

Although ceramic prostheses have been successfully used in conventional total hip arthroplasty (THA) for many decades, ceramic materials have not yet been applied for hip resurfacing (HR) surgeries. The objective of this study is to investigate the mechanical reliability of silicon nitride as a new ceramic material in HR prostheses. A finite element analysis (FEA) was performed to study the effects of two different designs of prostheses on the stress distribution in the femur-neck area. A metallic (cobalt-chromium-alloy) Birmingham hip resurfacing (BHR) prosthesis and our newly designed cerami

The position of the femur's greater trochanter is crucial to produce the required hip abduction moment during gait. Clinical parameters describing the shape of the femur, such as femoral neck anteversion and neck shaft angles, influence the lever-arm of abductor muscles. In children with cerebral palsy, abnormal lower limb bone shapes may impair function, highlighting the importance of clinical gait analysis for surgical decision. Here, we re-examined a dataset on the correlation between 3D lower limb bony morphology and hip rotation kinematics during gait. This study was a secondary analysis

OBJECTIVE: To determine whether femoral osteotomies that change frontal plane alignment without affecting torsion influence anteversion and inclination.
METHODS: Femurs without deformity were scanned to create three-dimensional reconstructions. The femoral head-neck axis was identified by placement of a virtual intramedullary pin. A proximal osteotomy was simulated to create three conditions while keeping torsion constant: Normal, Coxa Valga (neck-shaft angle increased by 12°), and Coxa Vara (neck-shaft angle decreased by 12°). Femoral anteversion was measured from an axial image in all three

Abnormal hip development causes one-quarter to one-half of all hip disease. Dysplastic hips typically share characteristic anatomic abnormalities. The dysplastic acetabulum is typically shallow, lateralized, and anteverted with insufficient coverage anteriorly, superiorly, and laterally. The dysplastic proximal femur has a small femoral head with excessive femoral neck anteversion and a short neck with an increased neck shaft angle. These characteristic changes result in intraarticular pathology leading to hip arthritis. A variety of treatment options exist based on the degree of dysplasia and

A new method of recording the three-dimensional anatomy of the proximal femur from a single anteroposterior radiograph is described. This technique shows that in Perthes' disease the femoral head and neck are in significant anteversion and true varus. This anatomical configuration may be important in the pathogenesis and treatment of this disorder.

CONCLUSIONS: Surgical treatment of solitary proximal end of femur osteochondromas is mandatory, a …

CASE: A 35-year-old woman, who lived in an endemic area for echinococcosis, developed a periprosthetic fracture with loosening of a total hip arthroplasty that had been performed 9 years earlier for hydatidosis of the right proximal femur with a pathological femoral neck fracture. At that time, the patient did not have any signs of liver or lung involvement. She was successfully managed by a cemented revision hip arthroplasty without any signs of recurrence at the last follow-up of 5 years.
CONCLUSIONS: Total hip arthroplasty can offer effective management of proximal femur hydatidosis, despi

BACKGROUND: Femoral neck fracture (FNF) is a very common traumatic disorder and a major cause of blood supply disruption to the femoral head, which may lead to a severe long-term complication, osteonecrosis of femoral head (ONFH). Early prediction and evaluation of ONFH after FNF could facilitate early treatment and may prevent or reverse the development of ONFH. In this review paper, we will review all the prediction methods reported in the previous literature.
METHODS: Studies on the prediction of ONFH after FNF were included in PubMed and MEDLINE databases with articles published before Oc

Operative reductions of congenital hip dislocation are sometimes difficult when lateral incisions are used since the medial part of the acetabulum and the lower limbus are difficult to visualize. In the Ludloff technique this part of the acetabulum is seen excellent, but not the lateral part. High dislocations with inverted limbus are difficult to reduce and only with partial excision of the limbus. An inguinal incision beginning lateral at the spina ilica ventralis offers the best view and can be used for all different degrees of dislocation and age groups. The technique is described, the adv
