Percutaneous needle decompression (PND) can be a successful alternative to open fasciotomies for acute compartment syndrome (ACS). We present the case of a 45-year-old male patient who survived a road traffic accident and developed ACS following his open fracture of the tibia and fibula. He was treated by performing PND on all compartments of the affected leg using a 24 gauge needle thus avoiding the complications of a double incision fasciotomy.

We have reviewed 484 open fractures in patients aged ≥65 years over a 15-year period and compared them with 1902 open fractures in patients <65 years treated in the same period. 33 patients treated operatively for plasma cell disease were analysed. There were 21 men and 12 women with an average age of 54 years. There was an undefined bone tumour in 23 cases, and a pathologic fracture in 10 cases. In only 6 cases was the diagnosis known before the operation. The primary tumour localisations were: vertebral column in 13, pelvis in 7, femur in 6, humerus in 2, rib in 1, tibia in 1, fibula in 1, scapula in 1 and olecranon in 1 case. 16 diagnostic biopsies were taken. Vertebral tumours were mainly evacuated or decompressed, combined with a stabilising procedure in 8 cases.

CASE: A 20-year-old man presented after a fall from an 8-foot ladder. Radiographs revealed pronation external rotation injury. Closed reduction and splinting was performed in the emergency department. Surgery was planned 6 days later to include open reduction and internal fixation of the fibula and syndesmosis, and open repair of deltoid ligament. Intraoperatively, a myotendinous rupture of the posterior tibial tendon (PTT) was identified and repaired.

Periosteal proprioceptive nerve receptors may act as mechanoreceptors of long bones during adaptive remodeling after fracture. They may also contribute to the mechanisms of coordinated functional activity of fractured limbs and thus inhibit harmful overloading of fracture callus. The area of proprioceptive nerve receptors around the distal part of the rat fibula was stripped surgically, and a standard fracture of the fibular shaft was produced. Animals failed to unite their fractures and developed mainly atrophic nonunions. Atrophy of ununited fragments was due to osteoclastic bone resorption.
