Treatment of bilateral congenital extensor tendon dislocation on multiple digits with only one sagittal band reconstruction: a case report.

Bilateral congenital dislocation of the extensor tendon in the metacarpophalangeal joint is an exceedingly rare disease and often involves multiple fingers. Surgical treatment of multiple congenital extensor tendon dislocations in both hands has been reported; however, no report has clearly stated whether all fingers should be surgically treated in patients with multiple finger involvement. We report a case in which we successfully treated bilateral congenital extensor tendon dislocation on multiple digits with only one single-loop reconstruction of the sagittal band instead of operating on al

Fractures of the phalanges and interphalangeal joints in children.

Phalangeal fractures in children are common, and conservative treatment leads frequently to a good functional outcome. Articular or displaced fractures require early re-cognition and special attention, including surgery. In children, remodeling occurs primarily in the sagittal plane, and rotational deformities are often unacceptable.

Primary flexible implant arthroplasty of the metacarpophalangeal joints during digital replantation.

Primary silicone rubber arthroplasty in a total of four metacarpophalangeal joints during digital replantation or revascularisation in two patients is reported. The results appear preferable to present alternatives and allow good function without prejudicing further management.

Correction of the claw hand.

Intrinsic paralysis can be the manifestation of a variety of pathologic entities (stroke, cerebral palsy, Charcot-Marie-Tooth, muscular dystrophy, leprosy, trauma, cervical disease, and compressive and metabolic neuropathies). Patients present with a spectrum of clinical findings dependent on the cause and severity of the disease. The 3 main problems caused by intrinsic weakness of the fingers are clawing with loss of synchronistic finger flexion, inability to abduct/adduct the digits, and weakness of grip. Clawing is defined as hyperextension of the metacarpophalangeal joints and flexion of t

The use of skeletal traction to correct the flexed PIP joint in Dupuytren's disease. A pilot study to assess the use of the Pipster.

The flexed PIP joint presents a particular problem in the treatment of advanced Dupuytren's disease. Following reports of the use of skeletal traction in the treatment of this condition, a simple device, the "Pipster" was developed to extend the PIP joint by skeletal traction before surgery. In seven fingers in five patients with severe contractures, there was a pre-operative improvement of at least 45 degrees in the flexion angle (measured as maximum achievable passive extension). The technique was effective in primary and recurrent disease. Subsequent surgery was facilitated and amputation a

Object Shape Recognition Using Sparse Soft Capacitive Tactile Sensors for Robotic Hands.

Reliable tactile object shape recognition on robotic hands is often achieved using dense sensor arrays or vision-based tactile skins, which … ative metacarpophalangeal signals are identified and removed, reducing the number of sensors at the finger joints while improving classification accuracy. A lightweight multi-layer perceptron operating on this low-dimensional input achieves 95.4% size-invariant recognition accuracy across 12 rigid objects representing four geometric primitives-cuboid, sphere, …

"Suture fixation of the fingers": an effective method for positioning burned and contracted fingers using a pulley system as a guide.

Preserving function of the hand is the aim of treatment in burned hands; appropriate splinting is one of the important measures during acute and chronic treatment. We introduce an effective safe method for positioning of fingers without violating the joints; In this method before performing skin graft for palmar finger burn or contracture release we suture tip of finger with silk 2-0 and fix it to dorsum of hand while extending the finger and for preventing slipping we insert some pulley like circles tied with silk 2-0 fixing over dorsum of mid phalanx.

A novel splint for proximal interphalangeal joint contractures: a case report.

Proximal interphalangeal (PIP) joint contractures are notoriously difficult to treat. Best results are obtained with early mobilization and splinting, though a high level of adherence is critical for a good outcome. A new roll-on splint that aims to increase motion with minimal difficulty was used. The patient described here with moderate PIP joint contractures (30°-60°) was treated successfully using this splint. The splint design and therapy protocol are described. The patient was treated for 12 weeks with good adherence to therapy and splinting. Total active motion increased by 87% in the i

Impaired proprioceptive acuity at the proximal interphalangeal joint in patients with the hypermobility syndrome.

Twelve patients with the hypermobility syndrome (> or = 4 on the Beighton score) were examined to establish whether they showed any impairment of proprioception. A position matching paradigm was used which required subjects to align a finger silhouette with the kinaesthetically perceived position of their hidden index finger. Position sense at the PIP joint was found to be significantly (P < 0.0001) impaired in hypermobile patients who made larger matching errors at all angles compared to age- and sex-matched controls. The patients also demonstrated biasing in the direction of errors made when

Common finger fractures and dislocations.

Finger fractures and dislocations are common injuries that are often managed by family physicians. A systematic physical examination is imperative to avoid complications and poor outcomes following these injuries. Radiography (commonly anteroposterior, true lateral, and oblique views) is required in the evaluation of finger fractures and dislocations. Dorsal dislocation of the proximal interphalangeal joint is the most common type of finger dislocation. Finger dislocations should be reduced as quickly as possible and concurrent soft tissue injuries treated appropriately. Referral to a hand spe

Extensor tendon injuries and repairs in the hand.

Due to their superficial course, the extensor tendons are frequently lacerated over the dorsum of the hand and fingers. Excellent functional results are obtained in repairs of simple tendon lacerations. 'Open' mallet lacerations over the distal IP joint or involving the central extensor slip over the proximal IP joint require more precise suturing methods. More proximal extensor tendon divisions near the wrist involve dissection of the retracted finger extensors or long thumb extensor in the distal forearm and more formal tendon repairs, including a possible tendon transfer to the thumb. 'Clos

Does a 'firebreak' full-thickness skin graft prevent recurrence after surgery for Dupuytren's contracture?: a prospective, randomised trial.

We randomised 79 patients (84 hands, 90 fingers) with Dupuytren's contracture of the proximal interphalangeal joint to have either a 'firebreak' skin graft (39 patients, 41 hands, 44 fingers) or a fasciectomy (40 patients, 43 hands, 46 fingers) if, after full correction, the skin over the proximal phalanx could be easily closed by a Z-plasty. Patients were reviewed after three, six, 12, 24 and 36 months to note any complications, the range of movement and recurrence. Both groups were similar in regard to age, gender and factors considered to influence the outcome such as bilateral disease, fam

Dislocation of the extensor tendons over the metacarpophalangeal joints.

Twenty-seven patients who had received treatment for dislocation of the extensor tendons over the metacarpophalangeal joint were reviewed. Sixteen patients had traumatic dislocations, seven had spontaneous dislocations, and four had congenital dislocations. The long finger was most frequently affected. The other fingers were affected almost equally. Displacement of the extensor tendon always occurred in the ulnar direction in the long and ring fingers. The index and little fingers exhibited different patterns of dislocation: two patients had ulnar dislocation of both the common and proprius te

Percutaneous Tenodermodesis for Mallet Fingers: An Office-based Procedure.

Mallet fingers are injuries to the extensor tendon at the distal interphalangeal (DIP) joint and can present as a bony avulsion or as a soft tissue injury. Nonbony mallet fingers are frequently splinted in extension between 6 and 8 weeks. If splinted correctly, most results are good with a mean DIP joint extension lag between 5 and 10 degrees. However, decreased swelling, hygienic considerations and patient compliance can lead to splint removal and a less favorable outcome. We present a percutaneous tenodermodesis using only a digital block and a 4.0 nylon suture. This office-based procedure p

Traumatic extensor tendon dislocation in a boxer: a case study.

An elite collegiate boxer developed extensor tendon subluxation in the small finger of his dominant right hand. He was thought to have a radial sagittal band disruption and was allowed to complete his season with custom padding. Surgical exploration revealed intact sagittal bands with divergent dislocation of the two extensor tendons to the small finger with underlying capsular rupture. These lesions were repaired and he successfully returned to boxing. Soft tissue injuries to the dorsal MCP joint may involve the collateral ligaments, the sagittal bands, the extensor tendons, or the joint caps

Tendon transfer or tendon graft for ruptured finger extensor tendons in rheumatoid hands.

We evaluated the clinical outcome of tendon reconstruction using tendon graft or tendon transfer and the parameters related to clinical outcome in 51 wrists of 46 patients with rheumatoid arthritis with finger extensor tendon ruptures. At a mean follow-up of 5.6 years, the mean metacarpophalangeal (MP) joint extension lag was 8 degrees (range, 0-45) and the mean visual analogue satisfaction scale was 74 (range, 10-100). Clinical outcome did not differ significantly between tendon grafting and tendon transfer. The MP joint extension lag correlated with the patient's satisfaction score, but the

Congenital defects of the flexor digitorum profundus tendon of the little finger.

The cases of two patients, a four-year-old boy and an eight-year-old boy, who had been incapable of active flexion of the little finger since birth, are presented. They were capable of active flexion of the metacarpophalangeal (MP) joint, but not of the proximal interphalangeal (PIP) and distal interphalangeal (DIP) joints. They were diagnosed with a defect of the flexor digitorum profundus (FDP) tendon of the little finger and underwent surgery. In both cases, the FDP tendon turned into fibrous tissue proximal to the palm and lost continuity on this side. Reconstruction was performed by makin

Dupuytren's contracture of the fingers: a simplified approach to the surgical treatment.

Hematoma formation, delay in healing, pain, stiffened finger joints are complications that sometimes follow classical surgical approaches to Dupuytren's contracture. A new surgical approach to the disease that can correct the contractures without the attendant morbidity is urgently needed. By treating Dupuytren's as any other scar contracture (division of the contracting soft tissue at its point of maximal tension and interposing normal free full-thickness skin) postoperative morbidity can be greatly decreased. Full return of function was achieved within 21 days following operation in 85 perce…

A preliminary assessment of Na-hyaluronate injection into "no man's land" for primary flexor tendon repair.

A special fraction of Na-hyaluronate (Healon) was used for enhancement of repair of flexor tendon lacerations in "No Man's Land" to assess its role in preventing adhesion formation. The profundus tendons of the third and fourth fingers in owl monkeys were lacerated and repaired, and the superficialis tendons were resected. Prior to closure, saline solution or Healon paste was applied around the tendon. The fingers were immobilized for 4-5 weeks with the proximal interphalangeal joint (PIP) at 90 degrees of flexion. Following immobilization, the range of motion of the PIP joint was tested in a

Operative correction of posttraumatic and congenital swanneck deformity--a new technique.

Volar plate reconstruction by using a part of the slip of the superficialis tendon is a new technique in the treatment of posttraumatic and congenital swanneck deformity. This small series represents two patients with a posttraumatic swanneck deformity due to volar plate rupture and one patient with a congenital swanneck deformity in six fingers. The results of the operation with a two years follow-up show seven fingers with a normal flexion-extension range without hyperextension. Only one finger has an extension lag of five degrees. All fingers have a normal grip strength and normal fine move
