Foot Shape in Women With and Without Hallux Valgus Determined Using 3-Dimensional Foot Scanning.

PURPOSE: Critically ill patients with hematologic malignant (HM) diseases are a heterogeneous but vulnerable group, where intensive care (ICU) is associated with high mortality. The impact of ICU care on long-term survival is less well described. This retrospective national cohort study investigates the long-term survival outcomes of hospitalized HM patients, comparing ICU survivors with non-ICU patients. In addition, focused analyses were performed on the ICU cohort to further investigate survival patterns. METHODS: Adult patients diagnosed with HM and admitted to hospital between May 6, 201

Segmentation and Clustering Analysis of Scanned Foot Models for Intelligent Diabetic Foot Ulcer Offloading Boots.

Diabetic foot ulcer (DFU) is a common and severe complication of diabetes that leads to amputation if not effectively managed. Intelligent offloading or rehabilitation devices or boots are used to manage or observe the ulcer and the process of treatment which requires the segmentation and clustering analysis of 3D scanned foot models. This study examines the effectiveness of two widely used clustering algorithms, K-Means and Gaussian Mixture Models (GMM), in segmenting scanned foot models to use in rehabilitation boots. The performance of K-Means and GMM was compared across 98 foot models. GMM…

Recurrent diabetic foot ulcers: Results of a maximal multidisciplinary approach including reconstructive foot/ankle surgery.

The diabetic foot ulcer (DFU) and Charcot Neuroarthropathy (CN) are serious complications of diabetes mellitus in which wound closure is complex to achieve. Treating recurrent DFU in patients with a combination of infection, ischemia, and deformities is extremely challenging and this group of patients has a very poor outcome. This case series describes the outcomes of patients with a recurrent DFU and CN, with a mean SINBAD score of 4 and of which 40% had a TCS of D3, using a multidisciplinary protocol that includes reconstructive foot and ankle surgery. In 24/35 (69%) of patients, wound closu…

Association Between Middle Facet Subluxation and Foot and Ankle Offset in Progressive Collapsing Foot Deformity.

BACKGROUND: Subtalar middle facet (MF) subluxation was recognized as a reliable marker for progressive collapsing foot deformity (PCFD) diagnosis. Foot and Ankle Offset (FAO) is an established measurement, predictive of malalignment severity. The objective of this study was to assess the potential association between MF subluxation and FAO in PCFD patients. METHODS: 56 individuals with flexible PCFD (74 feet) were assessed. Two blinded foot and ankle surgeons calculated MF uncoverage, MF incongruence, and FAO. Agreement was quantified using intraclass correlation coefficient (ICC). A multivar…

Foot salvage using microsurgical free muscle flaps in severely crushed foot with soft tissue defects.

OBJECTIVE: Management of complex foot injuries including skin, tendons, vessels, bone, with soft tissue defects is considered as an orthopedic challenge. Microsurgical free muscle flaps provide the best solution in such cases. PATIENTS AND METHODS: Thirty two consecutive cases of complex ankle and foot injuries were treated by skin grafted muscle free flap in one stage procedure after radical debridement. Twenty four were males and 8 were females. Right foot was affected in 26 and left was involved in 6 patients. The average age was 22 years (range 6-33 years). The most common cause of injury…

From flat foot to fat foot: structure, ontogeny, function, and evolution of elephant "sixth toes".

Several groups of tetrapods have expanded sesamoid (small, tendon-anchoring) bones into digit-like structures ("predigits"), such as pandas' … arsals that are visible in fossils. Our survey shows that basal proboscideans were relatively "flat-footed" (plantigrade), whereas early elephantiforms evolved the more derived "tip-toed" (subunguligrade) morphology, including the predigits and fat pad, of extant elephants. Thus, elephants co-opted sesamoid bones into a role as false digits and used th …

Diabetic foot infections in the elderly: primary amputation versus 'foot-sparing surgery'. A case report.

Renal failure diabetic patients who present with lower extremity gangrene represent one of the most difficult problems encountered in a typical vascular practice. We report the hospital course and management of a 74-year-old male patient with such comorbidities, affected by a non healing ulcer that progressed into a large plantar abscess. Our case unfortunately mirrors a common method of evaluation and therapy of patients with such comorbidities and sets up the stage for a very controversial subject.

A new hip-knee-ankle-foot sling: kinematic comparison with a traditional ankle-foot orthosis.

In this study, we performed a kinematic analysis of a new, low-cost sling for the lower limb, compared to a common ankle-foot orthosis (AFO). Gait with no orthosis, with the AFO, and with the new sling was analyzed in one hemiplegic subject. Both the AFO and the sling reduced the mean angle and ROM (range of movement) of the ankle and the vertical displacement of the center of mass. The sling, but not the AFO, restored the normal sequence heel-strike, forefoot contact of the affected side. The sling, but not the AFO, reduced the affected limb stance and stride duration, increased stride length…

A methodological framework for detecting ulcers' risk in diabetic foot subjects by combining gait analysis, a new musculoskeletal foot model and a foot finite element model.

Diabetic foot is one of the most debilitating complications of diabetes and may lead to plantar ulcers. In the last decade, gait analysis, musculoskeletal modelling (MSM) and finite element modelling (FEM) have shown their ability to contribute to diabetic foot prevention and suggested that the origin of the plantar ulcers is in deeper tissue layers rather than on the plantar surface. Hence the aim of the current work is to develop a methodology that improves FEM-derived foot internal stresses prediction, for diabetic foot prevention applications. A 3D foot FEM was combined with MSM derived fo…

Overuse Injuries of the Ankle and Foot.

Overuse injuries of the foot and ankle typically result from repetitive microtrauma rather than from a sudden sprain or a single impact, in which the cumulative load on tendons, fascia, bones, and surrounding soft tissues exceeds the tissues' reparative capacity. Repetitive lower extremity loading and complex movement patterns in sports such as soccer, basketball, gymnastics, and running increase the risk of overuse injuries. Common symptoms include swelling, pain, and limited mobility, which tend to worsen with continued activity. While conservative treatment is often sufficient, persistent p…

Orthobiologics for Foot and Ankle Tendinopathy.

Foot and ankle tendinopathies are common problems for which there exists a "treatment gap" between surgery and traditional conservative management. Orthobiologics have potential to help fill that gap, aiming to use molecular biology to steer host cell responses, with the goal of improving tissue homeostasis and function. They are primarily used for chronic tendon degeneration and angiofibroblastic disrepair-"tendinosis"-rather than acute inflammation-"tendonitis." As the technology, techniques, and clinical evidence for orthobiologics continue to develop and advance, several biologic therapies…

Congenital infantile fibrosarcoma of the foot.

Infantile fibrosarcoma (IFS) is a malignant soft tissue tumour and rarely presents in the newborn period. Congenital IFS poses a particular diagnostic challenge, as they may clinically and radiologically mimic vascular anomalies, such as haemangiomata. We present a case of a baby boy who, at birth, was noted to have a smooth mass of approximately 4 cm×4 cm on the sole of his right foot. Newborn examination was otherwise unremarkable. Atypical haemangioma was considered the most likely diagnosis. However, as the mass had grown very rapidly, an urgent MRI and biopsy were performed, confirming the …

RECOMMENDATIONS FOR THE MANAGEMENT OF DIABETIC FOOT.

Diabetic foot disease remains a leading cause of non-traumatic lower-extremity amputations worldwide, contributing significantly to morbidity, mortality, and healthcare costs. This article synthesizes evidence-based recommendations from the International Working Group on the Diabetic Foot (IWGDF), International Diabetes Federation (IDF), and American Diabetes Association (ADA) to provide a comprehensive approach to prevention, classification, and management. Core principles include early identification of at-risk feet, patient education, biomechanical offloading, restoration of perfusion, infe…

Antibiotic Therapy in the Diabetic Foot.

Empiric antimicrobial therapy for diabetic foot infections must be selected with attention to wound severity and patient-specific factors including comorbidities, antibiotic allergies, prior cultures, risk factors for antibiotic-resistant organisms and the local antibiogram. Subsequent therapy can be tailored in spectrum by culture results and its duration informed by surgical approach and pathologic data. After initial clinical improvement, most patients can be transitioned to oral step-down therapy. The duration of treatment for soft tissue infection (STI) is based on the severity of STI and…

Foot and Ankle Injuries in Baseball.

Foot and ankle injuries in baseball provide unique challenges to the provider. Most foot and ankle injuries (ie, ankle sprains and contusions) can be managed non-operatively using a similar approach as in other athletic populations, but a few unique mechanisms of injury in baseball are important to be aware of. While upper extremity injuries such as ulnar collateral ligament (UCL) tears and labral tears are extensively reported on in the literature, there is a need for further baseball-specific research on foot and ankle injuries, especially given the increasing prevalence of these injuries ov…

Onychomycosis in Foot and Toe Malformations.

Introduction: It has long been accepted that trauma is one of the most important and frequent predisposing factors for onychomycoses. However, the role of direct trauma in the pathogenesis of fungal nail infections has only recently been elucidated in a series of 32 cases of post-traumatic single-digit onychomycosis. The importance of repeated trauma due to foot and toe abnormalities was rarely investigated. Aimof the study: This is a multicenter single-author observational study over a period of 6 years performed at specialized nail clinics in three countries. All patient photographs taken by…

Orthotic Devices for the Foot and Ankle.

Millions of Americans wear ankle-foot orthotic devices for protection, pain relief, and deformity correction. Inquiries about off-the-shelf and custom devices are a common reason for evaluation with a foot and ankle surgeon or general orthopaedic surgeon. Despite limited high-quality evidence for their use, these devices can have a notable clinical impact on physical function. An up-to-date understanding of orthotic device options and their appropriate use in managing musculoskeletal pathologies applies to all orthopaedic providers. This review aims to categorize orthosis types and provide spe…

Osteochondral Lesions of the Ankle and Foot.

Osteochondral lesions (OCLs) in the ankle are more common than OCLs of the foot, but both share a similar imaging appearance. Knowledge of the various imaging modalities, as well as available surgical techniques, is important for radiologists. We discuss radiographs, ultrasonography, computed tomography, single-photon emission computed tomography/computed tomography, and magnetic resonance imaging to evaluate OCLs. In addition, various surgical techniques used to treat OCLs-debridement, retrograde drilling, microfracture, micronized cartilage-augmented microfracture, autografts, and allografts…
