Midfoot osteotomies.

Midfoot osteotomies are primarily used for reconstructive procedures for congenital or acquired deformities. Although used more in a younger population, they can used in most any patient population. With restoration of a more normal architecture, patients' symptoms resolve and prevention of long-term sequella can be avoided. Careful evaluation is necessary, but with proper use, midfoot osteotomies are useful procedures in the foot and ankle surgeons' repertoire.

[The painful foot].

The different causes of painful feet, most of which are obvious, but neglected, are discussed. With a good history and examination, almost all the causes can be detected and treated in a simple manner.

Forefoot Deformity in Rheumatoid Arthritis: A Comparison of Shod and Unshod Populations.

UNLABELLED: All reported rheumatoid arthritis (RA) forefoot deformities in the literature thus far have arisen from shoe wearing populations. Our aim in this study was to compare hallucal deformities seen in a shod population with an unshod population. A population comparison was undertaken in 2 specialized foot and ankle units, one in India and one in the United Kingdom. In the shod population, there was 1 hallux varus deformity, 10 without hallucal deformity, and 90 hallux valgus deformities. In contrast, in the unshod population, there were 19 hallux varus deformities and 6 hallux valgus de…

Pes cavo-valgus foot.

Collapsing pes plano valgus deformity and pes cavus deformity have been identified and treated for many years. Recently, a combination of these deformities has been recognized occurring simultaneously. This multiplanar deformity is known as the cavo-valgus foot. Treatment of this foot type requires a firm understanding of planal dominance. The authors present a case report on this subject.

Synpolydactyly of the foot in homozygotes.

In 2002, we reinvestigated a large synpolydactyly kindred first described in 1995. It was found to have expanded with an increase in number … e and syndactylized hypoplastic minor toes. From clinical and surgical perspectives, their hand and foot deformities were different from those of their parents. We surgically treated both feet of four individuals with this deformity, which we called "homozygote foot synpolydactyly." Clinically, the deformity consisted of a supinated prominent great toe, hypoplasti …

The 3-dimensional configuration of the typical foot and ankle in diastrophic dysplasia.

BACKGROUND: Surgical correction of the foot and ankle in patients with diastrophic dysplasia is extremely difficult secondary to the markedly distorted pathoanatomy. Several authors have described superficially some of the clinical and radiographic findings typical of the foot and ankle with diastrophic dysplasia; however, no description of the specific osseous deformities has been described in the literature. The purpose of this article was to provide such a model, detailing the nature of deformity of each of the bones and joints in the foot and ankle and their relationship to each other from

Basic considerations for shoes, shoe modifications, and orthoses in foot care.

It is important to remember that appropriate padding and materials for the foot should be selected on the basis of indications and functional needs of the patient. It is important to remember that foot wear, as protection and foot covering, needs to be compatible with the foot and the patient's functional requirements. The foot, orthosis, and shoe must function as a unit and as part of the patient's ambulatory system. I have attempted to identify some of the basic considerations involved in shoe selection, shoe modification, and orthotic selection for patient's rehabilitation. This article is

Current concepts of Charcot foot in diabetic patients.

The Charcot foot is an uncommon complication of neuropathy in diabetes. It is a disabling and devastating condition. The etiology of the Charcot foot is unknown, but it is characterized by acute inflammation with collapse of the foot and/or the ankle. Although the cause of this potentially debilitating condition is not known, it is generally accepted that the components of neuropathy that lead to foot complications must exist. When it is not detected early, a severe deformity will result in a secondary ulceration, infection, and amputation. Immobilization in the early stages is the key for suc

OBJECTIVE: The objectives of this present case study are to report a rare combination of a displaced talar neck fracture with a compression … ient recovered from the accident without having symptoms of pain, avascular necrosis, postoperative foot deformities or neurovascular deficits. CONCLUSIONS: The combination of a displaced talar neck fracture with a compression fracture of the calcaneocuboid joint in children usually requires correct diagnoses and early treatment with anatomic reduction and intern …

Foot ulcer risk and location in relation to prospective clinical assessment of foot shape and mobility among persons with diabetes.

AIMS: We assessed baseline clinical foot shape for 2939 feet of diabetic subjects who were monitored prospectively for foot ulceration. MET … However, there was no relationship between ulcer location and foot deformity. CONCLUSIONS: Certain foot deformities were predictive of ulceration, although there was no relationship between clinical foot deformity and ulcer location.

Associated deformities and hypermobility in hallux valgus: an investigation with weightbearing radiographs.

BACKGROUND: Hallux valgus has been reported to recur after surgical correction in patients subsequently diagnosed with hypermobility of the first ray, pronation of the foot, and pes planovalgus. An objective means of assessing the foot for these deformities preoperatively may avert a poor outcome. This investigation evaluated the efficacy of full-length weightbearing radiographs to recognize associated deformities in patients with hallux valgus before surgery. METHODS: This study compared five parameters from anteroposterior and four parameters from lateral weightbearing pedal radiographs of

An evaluation for compensated anterior cavus.

Anterior cavus is a multiplane structural deformity that can originate from different areas within the foot. Differentiation of these areas is a necessary examination in order to determine the effect of proper treatment. A depressed fourth and fifth ray or lateral column is a major deformity in anterior cavus. The lateral column deformity is a pronatory force that causes a premature medial shift of weight. The premature shift can cause a breakdown of the medial column of the foot. We have developed an examination of the compensated anterior cavus deformity that enables the practitioner to isol

[Foot surgery in Germany. Historical burden or scientific challenge?].

In Germany foot surgery was established in the late nineteenth century. To correct foot deformities due to paralytic disorders, i.e., poliomyelitis, and congenital deformities, different types of tenotomies and arthrodeses were developed to obtain plantigrade weight-bearing and to avoid external orthosis. A description of surgical techniques during the last century is presented in detail. During those years resection arthroplasties for hallux valgus were common, although some authors recommended osteotomies of the first metatarsal as joint-sparing techniques. Nowadays, further knowledge in eti…

OBJECTIVE: Little prospective research exists on risk factors for diabetic foot ulcer that considers the independent effects of multiple pot … nd type, race, smoking status, diabetes education, joint mobility, hallux blood pressure, and other foot deformities. Neuromuscular issues in cavovarus foot.

Differential muscle weakness can cause a cavus foot deformity. Presenting complaints in the hindfoot may include ankle instability, secondary arthritis, or peroneal tendonitis. Presenting complaints in the forefoot may include stress fractures, callus formation over the lateral border of the foot, claw toes, first ray overload, and metatarsalgia. More general presenting complaints can include a drop-foot gait, decreased walking tolerance, and difficulty with shoe or orthotic fitting. To surgically correct the foot shape, soft tissue contractures need to be released, bone deformity corrected, a
