Comparing the Efficacy for Pulse Versus Continuous Dose Terbinafine Therapy in Patients With Onychomycosis.

Recently, treatment outcomes in patients with toenail onychomycosis have improved considerably due to more effective oral antifungal medications such as terbinafine and itraconazole. These medications can either be used continuously for several weeks at a lower dose or intermittently (pulsed) at a higher dose. Previous literature comparing pulse and continuous therapy has generated mixed results.  Our study aims to compare the efficacy, in terms of clinical cure rate, of continuous vs pulse dose terbinafine regimens for toenail onychomycosis. Sixty patients with onychomycosis of Fitzpatri

Preliminary Evidence of the Efficacy of Nitric Acid Treatment in Onychomycosis.

UNLABELLED: Onychomycosis is the main cause of toenail disorders and is produced by a fungal infection. It is becoming more prevalent because of new lifestyles and immunosuppression statuses. The therapeutic approach to onychomycosis is under considerable study because of the lengthy treatments that require strong patient commitment, the limited efficacy of treatments, the inclusion of active substances that can be hepatotoxic and cause pharmacological interactions, and/or the questionable efficacy of treatments due to a lack of clinical trials. This study responds to the demand for rapid trea

Erysipelas-like presentation of Wells' syndrome (eosinophilic cellulitis).

Wells' syndrome, also called eosinophilic cellulitis, is a rare eosinophilic dermatosis characterized by an unspecific inflammatory erythematous eruption often associated with systemic symptoms. Here we report the case of a 57-year-old female with bilateral painful pitting and pruritic feet progressive for two weeks despite one week of oral antibiotics. Skin biopsy was performed showing dermal eosinophilic infiltration. The patient showed a spontaneous progressive improvement of the condition. The presented case demonstrates both clinical and histologic presence of lesions of Wells' syndrome i

Actinomycetoma Caused by Actinomadura mexicana, A Neglected Entity in the Caribbean.

Mycetoma is a chronic infection that is slow to develop and heal. It can be caused by fungi (eumycetoma) or bacteria (actinomycetoma). We describe a case of actinomycetoma caused by Actinomadura mexicana in the Caribbean region.

Macrophomina phaseolina: an overlooked cutaneous infection, seed rot disease in humans.

Rare and emerging opportunistic fungal pathogens are a serious and growing concern for immunosuppressed patients. Macrophomina phaseolina, an important soilborne plant pathogen in tropical and subtropical soil, should be considered in this category. It may be evident as a localized cutaneous cellulitis or as an ocular keratitis. We describe a 42-year-old diabetic male originally from India in whom a soft tissue mass of the foot proved to be an M. phaseolina cellulitis and review the literature. To our knowledge, only four individuals have been documented with infections with this organism, thr

Diagnosis and Management of Onychomycosis Perspectives from a Joint Podiatric Medicine-Dermatology Roundtable.

Onychomycosis is a fungal infection, and, as such, one of the goals of treatment should be eradication of the infective agent. Despite this, in contrast to dermatologists, many podiatric physicians do not include antifungals in their onychomycosis treatment plans. Before initiating treatment, confirmation of mycologic status via laboratory testing (eg, microscopy with potassium hydroxide preparation, histopathology with periodic acid-Schiff staining, fungal culture, and polymerase chain reaction) is important; however, more podiatric physicians rely solely on clinical signs than do dermatologi

Luliconazole: a review of a new antifungal agent for the topical treatment of onychomycosis.

Luliconazole is a novel, broad-spectrum, imidazole antifungal under development in the USA as a treatment for dermatophytic skin and nail infections. In vitro, luliconazole is one of the most potent antifungal agents against filamentous fungi including dermatophytes. Luliconazole has been formulated in a 10% solution with unique molecular properties, which allow it to penetrate the nail plate and rapidly achieve fungicidal levels in the nail unit. These properties make luliconazole a potent compound in the treatment of onychomycosis. This article reviews the development of luliconazole solutio

Efinaconazole 10% nail solution: a new topical treatment with broad antifungal activity for onychomycosis monotherapy.

BACKGROUND: Topical therapies for onychomycosis are associated with less adverse events than systemic therapies, but poor nail penetration limits their efficacy. Consequently, an efinaconazole 10% nail solution was developed. OBJECTIVE: To review the evidence supporting the usefulness of efinaconazole monotherapy in onychomycosis management. METHODS: PubMed and clinicaltrials.gov databases and abstracts from the 2013 annual meeting of the American Academy of Dermatology were searched in April 2013 using the terms "efinaconazole," "IDP-108," and "KP-103." RESULTS: In vitro, efinaconazole pos

Localized xanthomas associated with primary lymphedema.

Cutaneous xanthomas arising in chronic lymphedema are rare. We present a case of verruciform xanthoma involving the left foot and toes of a 10-year-old boy who had developed a primary lymphedema (lymphedema praecox) in the left lower extremity. Laboratory studies demonstrated a normal lipid profile.

Paracoccidioidomycosis: chronic adult unifocal form.

Paracoccidioidomycosis is a highly prevalent systemic mycosis in Brazil. The primary cutaneous form is rare and occurs in the absence of pulmonary involvement. Lesions located in the distal regions of the lower limbs are not uncommon and particularly in the feet may go unnoticed or even be confused with infectious or neoplastic lesions of different etiologies. We report a case of paracoccidioidomycosis affecting the left great toe of a man, leading to complete destruction of the nail plate. Treatment was done with the sulfamethoxazole plus trimethoprim with excellent results.

A new classification system for grading the severity of onychomycosis: Onychomycosis Severity Index.

OBJECTIVE: To establish and validate a new system to define the severity of onychomycosis. The Onychomycosis Severity Index (OSI) score is obtained by multiplying the score for the area of involvement (range, 0-5) by the score for the proximity of disease to the matrix (range, 1-5). Ten points are added for the presence of a longitudinal streak or a patch (dermatophytoma) or for greater than 2 mm of subungual hyperkeratosis. Mild onychomycosis corresponds to a score of 1 through 5; moderate, 6 through 15; and severe, 16 through 35. DESIGN: Consensus conference. SETTING: Teleconference. PART
