A Comparison of Brain MRI Outcomes in Youth American Football versus Noncontact Sport Athletes.

OBJECTIVES: This study aimed to compare brain magnetic resonance imaging (MRI) outcomes between children who play American football versus noncontact sport controls testing the hypotheses that history (primary) and duration (secondary) of football participation would be associated with differences in cortical thickness, subcortical volume, resting-state functional connectivity, and white matter diffusivity. METHODS: This secondary analysis of cross-sectional baseline data from the Adolescent Brain Cognitive Development Study compared brain MRI outcomes between 9- and 10-year-old children who …

Mental fatigue, skill performance and activity profile in elite male Australian Football match play.

The aim of this study was to identify relationships between mental fatigue (MF), match activity profile and skill execution in elite Australian Football (AF) match play. Thirty-nine elite male AF athletes (24.6 ± 4.5y) rated their MFs on a visual analogue scale on each training day of a 25 week, 23 match seasons. Match activity profile was measured by global navigation satellite system (GNSS) and match statistics measured by official Australian Football League (AFL) statistics in 23 AFL matches. Match statistics were coded based on an offensive or defensive skill and association with positive …

Shaping Training Load, Technical-Tactical Behaviour, and Well-Being in Football: A Systematic Review.

Football performance results from the dynamic interaction between physical, tactical, technical, and psychological dimensions-each of which also influences player well-being, recovery, and readiness. However, integrated monitoring approaches remain scarce, particularly in youth and sub-elite contexts. This systematic review screened 341 records from PubMed, Scopus, and Web of Science, with 46 studies meeting the inclusion criteria (n = 1763 players; age range: 13.2-28.7 years). Physical external load was reported in 44 studies using GPS-derived metrics such as total distance and high-speed run…

Correlation of King-Devick Test and Helmet Impact Exposures Over a Youth Football Season.

BACKGROUND: The cumulative effect of repetitive subconcussive head impacts on neurocognitive function during youth contact sports remains largely unknown. There is a paucity of literature evaluating cumulative helmet forces over a season and their correlation with preseason and postseason cognitive performance tasks such as the King-Devick test (KDT). HYPOTHESIS: Higher helmet forces recorded throughout a 10-week, 10-game youth football season would correlate with slower performance on postseason KDT. STUDY DESIGN: Prospective cohort study. LEVEL OF EVIDENCE: Level 3. METHODS: A cohort of …

Characterising biomechanical and situational aspects of high magnitude subconcussive impacts in Canadian collegiate football.

The objective of this study was to characterise high magnitude subconcussive impacts in Canadian football to identify injurious relationships between player/game-based characteristics, impact magnitude, and frequency. Eighty-one male university-level football players were equipped with helmet accelerometers to capture peak linear acceleration (PLA), impact frequency, and helmet impact location. Impacts with PLA ≥ 60 g were included in this analysis. Video footage from 15 games over three seasons was analysed to characterise aspects of play including play type, position, closing distance, tackl…

Youth football helmet fit assessment and association of prior fitting, issuance and initial season.

The purpose of the study was to describe helmet fitting characteristics and influence of prior helmet fitting, issuance and initial season in a sample of youth football athletes. Helmet fit was assessed for 69 athletes aged 8-14 years using a standardized, 9-point inspection. Associated variables for improper fit consisted of previous fitting of helmet for the season, helmet issuance [team/school issued or purchased individually] and year of helmet initial season within 3 years of current season. A series of chi-square tests for association were conducted between the overall helmet fit and the…

Video analysis of head impacts in top-level female football players: a cohort study.

This study aimed at exploring the exposure of head impacts, i.e. headers and non-headers, in top-level female football players. The study included all teams (n = 10) and players (n = 238) in the Norwegian Women's Premier League during the 2023 season. Video analysis was used to register headers and non-headers in the 135 matches of the season. Potential head injuries were compared between video signs and medical reports. The header and non-header incidences were estimated per 90 match minutes with 95% confidence intervals (CI). We observed 9364 headers and 233 non-headers in 135 matches. Nine …

Smooth Pursuit Velocity After a Season of Repetitive Head Impacts in American Football Players.

IMPORTANCE: Repetitive head impacts (RHI) are blows to the head that do not elicit clinical signs or concussion symptoms. Yet, research suggests that brain integrity and functionality can be altered following a single season of contact collision sports. These deficits are untraceable by current clinical testing. OBJECTIVE: To examine the association between RHI and oculomotor control across a single division I American football season. DESIGN, SETTING, AND PARTICIPANTS: This single-center cohort study took place from July 2020 to May 2022. Data were analyzed from May 2022 to December 2024. P…

Mental health trajectory throughout high school football career: a four-year prospective cohort study.

INTRODUCTION: Repetitive head impacts may contribute to long-term neurological disorders, including chronic traumatic encephalopathy (CTE), and mental health decline often precedes cognitive symptoms. Adolescent athletes are especially vulnerable, yet prospective data on mental health trajectory across high school athletic career are limited. AIM: To examine the mental health changes of high school football players across multiple seasons of participation. METHODS: This prospective cohort study included 6 high schools across southern Indiana for 4 consecutive seasons from July 2021 to Februa…

Strengthening exercise and motor control among football players with ankle sprain: A scoping review.

INTRODUCTION: The ankles and feet of footballers are the most commonly affected areas by acute and chronic injuries, especially sprains. The durability of changes in motor control for the sprained injury strongly suggests that central motor commands have been reorganized and restructured involving the sensorimotor system. Indirectly, providing strength training improves muscular strength and benefits cardiometabolic health, coordination, sensorimotor, and motor performance. Thus, this study aimed to identify the effects of strengthening exercises on motor control among footballers with spraine…

Career duration and later-life health conditions among former professional American-style football players.

OBJECTIVES: Career duration is often used as a metric of neurotrauma exposure in studies of elite athletes. However, as a proxy metric, career length may not accurately represent causal factors, and associations with health outcomes may be susceptible to selection effects. To date, relationships between professional American-style football (ASF) career length and postcareer health remain incompletely characterised. METHODS: We conducted a survey-based cross-sectional cohort study of former professional ASF players. Flexible regression methods measured associations between self-reported career…

Antisocial behavior in football matches: Do changes in alcohol sales policy increase violent acts?

BACKGROUND: The violent behavior of football fans is constantly associated with their drinking habits. Aiming to reduce its impact, policy makers often ban the sales and consumption of alcohol beverages during matches. Nonetheless, there are few papers that empirically analyzed such relationship, and our paper aims to shed light on this question. METHODS: Out dataset comprises 4,560 matches from the first and second tiers of the Brazilian League, where 245 exhibited at least one antisocial behavior from fans. Ordered logistic regressions are used as method. RESULTS: Our empirical findings ev…

Chronic traumatic encephalopathy neuropathologic change is uncommon in men who played amateur American football.

INTRODUCTION: We examined postmortem brain tissue from men, over the age of 50, for chronic traumatic encephalopathy neuropathologic change (CTE-NC). We hypothesized that (i) a small percentage would have CTE-NC, (ii) those who played American football during their youth would be more likely to have CTE-NC than those who did not play contact or collision sports, and (iii) there would be no association between CTE-NC and suicide as a manner of death. METHODS: Brain tissue from 186 men and accompanying clinical information were obtained from the Lieber Institute for Brain Development. Manner of…

Tau Positron Emission Tomography and Neurocognitive Function Among Former Professional American-Style Football Players.

American-style football (ASF) players experience repetitive head impacts that may result in chronic traumatic encephalopathy neuropathological change (CTE-NC). At present, a definitive diagnosis of CTE-NC requires the identification of localized hyperphosphorylated Tau (p-Tau) after death via immunohistochemistry. Some studies suggest that positron emission tomography (PET) with the radiotracer [18F]-Flortaucipir (FTP) may be capable of detecting p-Tau and thus establishing a diagnosis of CTE-NC among living former ASF players. To assess associations between FTP, football exposure, and objecti…

Isolated Obturator Internus Muscle Strain Injury in a Professional Football Player: A Case Report.

The present case report concerns an isolated obturator internus muscle strain. The patient, who was a 30-year-old professional, elite-level football player, suffered the injury during the warm-up before a football match. Isolated injuries of the obturator internus are very uncommon and they are extremely rarely reported in the literature. The diagnosis was made clinically, documented by magnetic resonance imaging. The rehabilitation program was conducted conservatively with physical therapy and kinesiotherapy. The patient's return to full athletic activity took place 2 weeks after the incident…

Association between race and maladaptive concentric left ventricular hypertrophy in American-style football athletes.

OBJECTIVES: American-style football (ASF) athletes are at risk for the development of concentric left ventricular hypertrophy (C-LVH), an established cardiovascular risk factor in the general population. We sought to address whether black race is associated with acquired C-LVH in collegiate ASF athletes. METHODS: Collegiate ASF athletes from two National Collegiate Athletic Association Division-I programmes were recruited as freshmen between 2014 and 2019 and analysed over 3 years. Demographics (neighbourhood family income) and repeated clinical characteristics and echocardiography were recor…

Baseline, retest, and post-injury profiles of auditory neural function in collegiate football players.

OBJECTIVES: Recent retrospective studies report differences in auditory neurophysiology between concussed athletes and uninjured controls using the frequency-following response (FFR). Adopting a prospective design in college football players, we compared FFRs before and after a concussion and evaluated test-retest reliability in non-concussed teammates. DESIGN: Testing took place in a locker room. We analysed the FFR to the fundamental frequency (F0) (FFR-F0) of a speech stimulus, previously identified as a potential concussion biomarker. Baseline FFRs were obtained during the football pre-se…
