Impact of obesity on postoperative and long-term outcomes in a general surgery population: a retrospective cohort study.

BACKGROUND: The obesity paradox has been demonstrated postoperatively in several surgical populations, but only a few studies have reported long-term survival. This study evaluates the presence of the obesity paradox in a general surgery population, reporting both postoperative and long-term survival. METHODS: This retrospective study included 10,427 patients scheduled for elective, noncardiac surgery. Patients were classified as underweight (body mass index (BMI) < 18.5 kg/m(2)); normal weight (BMI 18.5-24.9 kg/m(2)); overweight (BMI 25.0-29.9 kg/m(2)); obesity class I (BMI 30.0-34.9 kg/m(2)…

Posterior reversible encephalopathy syndrome in an untreated hypertensive patient after spinal surgery under general anesthesia -A case report-.

Posterior reversible encephalopathy syndrome (PRES) is an unfamiliar term to anesthesiologists, and this is characterized by neurologic symptoms that include mental change, headache, seizure and visual disturbance and also abnormal neuroimaging finding. A 71-year-old female patient was operated on for posterior decompression and total laminectomy under general anesthesia for the spinal stenosis. After the operation, she developed generalized tonic-clonic seizure and a stuporous mentality in the recovery room. The magnetic resonance imaging (MRI) revealed swelling and increased signal intensity

Predictors of major morbidity and mortality after pneumonectomy utilizing the Society for Thoracic Surgeons General Thoracic Surgery Database.

BACKGROUND: Pneumonectomy is associated with a significant incidence of perioperative morbidity and mortality. The purpose of this study is to identify the risk factors responsible for adverse outcomes in patients after pneumonectomy utilizing The Society of Thoracic Surgeons General Thoracic Surgery Database (STS GTDB). METHODS: All patients who had undergone pneumonectomy between January 2002 and December 2007 were identified in the STS GTDB. Among 80 participating centers, 1,267 patients were selected. Logistic regression analysis was performed on preoperative variables for major adverse o

Dose-dependent suppression of the electrically elicited stapedius reflex by general anesthetics in children undergoing cochlear implant surgery.

BACKGROUND: Cochlear implants stimulate the auditory nerve to enable hearing. Determining appropriate upper and lower limits of stimulation is essential for successful cochlear implantation. The intraoperative evoked stapedius reflex threshold (ESRT) and evoked compound action potential (ECAP) are commonly used to determine the limits of implant stimulation. In this study, we evaluated the dose-related effects of sevoflurane, desflurane, isoflurane, and propofol on the intraoperative ESRT and ECAP. METHODS: Forty-four children aged 6 mo to 17 yr undergoing cochlear implantation were recruited

Intrathecal morphine plus general anesthesia in cardiac surgery: effects on pulmonary function, postoperative analgesia, and plasma morphine concentration.

OBJECTIVES: To evaluate the effects of intrathecal morphine on pulmonary function, analgesia, and morphine plasma concentrations after cardiac surgery. INTRODUCTION: Lung dysfunction increases morbidity and mortality after cardiac surgery. Regional analgesia may improve pulmonary outcomes by reducing pain, but the occurrence of this benefit remains controversial. METHODS: Forty-two patients were randomized for general anesthesia (control group n=22) or 400 microg of intrathecal morphine followed by general anesthesia (morphine group n=20). Postoperative analgesia was accomplished with an int

[The Dutch College of General Practitioners' practice guideline 'Diagnosis of breast cancer': reaction from the field of surgery].

The new guideline is an adaptation to recent developments in the genetics of breast cancer. Hereditary and familial breast cancer may give rise to many questions in women coping with family members with the disease. Recommendations are given in a clear and succinct way. It is regrettable that breast self examination is not recommended anymore as it is not considered to be of evidence-based value. This may be true, but, particularly in young women, it is often the only way to detect breast cancer. In case of suspicion of a malignancy the patient should be referred to a surgeon who is dedicated

Prospective, multi-centre trial of mortality following general or spinal anaesthesia for hip fracture surgery in the elderly.

In a prospective randomized multi-centre study, the mortality following internal fixation surgery for fracture of the upper femur was investigated in 538 elderly patients allocated to receive subarachnoid blockade or general (narcotic-relaxant) anaesthesia. The 28-day mortality was 6.6% with subarachnoid, and 5.9% with general, anaesthesia. The difference was not significant (95% confidence limits: -3.5 to +4.8). At 1 year following surgery, the mortality was 20.4%. Increasing age, ischaemic heart disease, cardiac failure, preoperative arrhythmias and poor ASA status were all associated with i

The Environmental and Human Health Impact of Virtual General Surgery Residency Interviews With an In-Person Second Look.

PURPOSE: Health care contributes to climate change through several mechanisms, including potentially avoidable travel. A major change in academic medicine during the COVID-19 pandemic was the transition to virtual interviews and resultant reduced environmental impact. However, as travel restrictions have lifted, programs have explored interview styles incorporating both virtual and in-person aspects. This study estimated the net carbon emissions from a single general surgery residency program that offered virtual interviews paired with an optional in-person second-look event and quantified the…
