[Management of voiding dysfunction in geriatric general hospital: clinical experience of 315 cases with indwelling urethral catheter or diapers].

In a geriatric general hospital, 157 patients with an indwelt urethral catheter and 158 patients using diapers were evaluated by detailed interview, urinalysis, measurement of residual urine, and cystometry during a period of 16 months from July, 1988, when the Department of Urology was established. As a result of therapeutic modalities, 94% of the patients were freed from the catheter or diapers. The indwelling catheter was found to be unnecessary in one-third of the patients. In addition, among the patients wearing diapers, one-third were capable of urinating only with the appropriate assist

Lost in Translation: Interprofessional Variability in Understanding Pro Re Nata (PRN) Prescribing in Geriatric Practice-A Prospective Observational Study.

Background/Objectives: Medication safety is a critical concern in older adults due to age-related vulnerability, multimorbidity, and polypharmacy. Pro Re Nata (PRN, "as needed") prescribing is common in geriatric care but may be interpreted inconsistently across healthcare professionals, potentially increasing the risk of medication errors. This study aimed to evaluate PRN prescribing patterns in hospitalized older adults and assess interprofessional perceptions of the clarity and appropriateness of PRN prescribing for conditions. Methods: We conducted a prospective, single-day observational s

[Mutual guiding and following of movement - The interaction process in mobilisation based on the example of kinaesthetics: A qualitative study].

Background: Mobility impairment is often seen as a reason for needing long-term care. Thus, promoting mobility becomes increasingly significant in nursing homes. The kinaesthetic approach offers a way to support nursing home residents in using their own resources to maintain or improve their mobility. Aim: The present study intends to identify the characteristics of the interaction between nursing home residents with impaired mobility and kinaesthetic trainers during mobilisation. Methods: This secondary analysis comprises nine video sequences interpreted according to Grounded Theory-principle

Orthogeriatric co-management and incident nursing home admissions in older patients with fragility fractures other than the hip-a retrospective cohort study using insurance claims data from Germany.

BACKGROUND: Orthogeriatric co-management (OGCM) has been proposed as care model for geriatric patients with fragility fractures. However, its impact on nursing home (NH) admissions following non-hip fractures is unclear. This study aims to assess the association between OGCM and the probability of NH admissions within 6 months in older patients with fragility fractures other than the hip. METHODS: This retrospective cohort study utilized nationwide insurance claims data from Germany (from years 2014-2018), covering individuals aged 80 years or older with fractures of the humerus, forearm, pel

Malnutrition and its determinants among older adults living in foster families in Guadeloupe (French West Indies). A cross-sectional study.

BACKGROUND: Foster families may represent an alternative model for dependent older adults in many countries where nursing homes are insufficiently developed. This study aimed to assess the prevalence of malnutrition and its determinants in older adults living in foster families in Guadeloupe (French West Indies). METHODS: This cross-sectional study was gathered from the KASAF (Karukera Study of Ageing in Foster families) study (n = 107, 41M/66F, Mdn 81.8 years). Nutritional status was assessed with the Mini Nutritional Assessment Short-Form (MNA-SF). Clinical characteristics and scores on ger

We've got trouble: medications in assisted living.

The rise in assisted living facilities (ALFs), coupled with residents with increasingly complex conditions, leads to unanticipated problems linked to medication management. The purpose of this study was to describe one western state's scope of medication-related problems, the relationship of problems to facility characteristics, and the nature of the problems using state surveyors' reports from routine inspections and complaint investigations. Of the 1,335 ALFs surveyed for 2007-2008, 61.8% received a medication-related citation. Of the 1,558 allegations, 60.3% involved individual residents, w

Care Settings and Clinical Characteristics of Older Adults with Moderately Severe Dementia.

OBJECTIVES: Little population-level evidence exists to guide the development of interventions for people with dementia in non-nursing home settings. We hypothesized people living at home with moderately severe dementia would differ in social, functional, and medical characteristics from those in either residential care or nursing home settings. DESIGN: Retrospective cohort study using pooled data from the National Health and Aging Trends Study, an annual survey of a nationally representative sample of Medicare beneficiaries. SETTING: US national sample. PARTICIPANTS: Respondents newly meeti

Practices, issues and possibilities at the interface between geriatrics and palliative care (InGaP): A case study exemplifying knotworking.

INTRODUCTION: With the recognition of the need for palliative care for people with non-malignant conditions, there is an increasing emphasis on interdisciplinary working between geriatric and palliative care teams. This interdisciplinary work has evolved organically; more needs to be known about current working practices. This is of policy and clinical interest as the older patient population continues to grow. METHODS: A case study based on qualitative interviews was undertaken of end-of-life care for older in-patients in a large London NHS Trust. 30 semi-structured qualitative interviews we

The Use of Low-Dose Dopamine in the Neonatal Intensive Care Unit.

Life-space mobility (LSM) is a concept for assessing patterns of functional mobility over time. LSM is gaining traction in the research of geriatric population health. Several instruments have been developed to measure LSM, such as the University of Alabama at Birmingham Life-Space Assessment (LSA) or the Nursing Home Life-Space Diameter instrument. There has been exponential growth in the use of instruments measuring LSM in studies of older adults since the concept was introduced in 1985. In response to the increased volume of publications with clinical applicability to those working in geria

Impact of Geriatric Events on Clinical Outcomes and Resource Utilization of Acute Coronary Syndrome Hospitalizations.

Background The effect of geriatric events (GEs) on outcomes of acute coronary syndrome (ACS) admissions is poorly understood. We evaluated the prevalence and impact of GEs on clinical outcomes and resource utilization of older patients admitted with ACS. Methods Using the 2018 National (Nationwide) Inpatient Sample, we analyzed all elective hospitalizations for ACS in older adults (age ≥ 65 years) and a younger reference group (age 55-64). Nationally-weighted descriptive statistics were generated for GEs based on ACS subtypes. Multivariate logistic regression models controlling for comorbiditi

Energy and protein intake in 330 geriatric orthopaedic patients: Are the current nutrition guidelines applicable?

BACKGROUND AND AIMS: Elderly orthopaedic patients are potentially at a high nutritional risk. The reasons for this are numerous as i.e. reduced appetite, sensation of satiety after eating small amounts of food, multi-medication or immobility. This is in contrast to the increased energy and protein recommendations for geriatric orthopaedic patients. METHODS: Oral intake during hospitalization of more than 1000 geriatric orthopaedic patients aged over 80, with or without fracture, was recorded, calculated and then compared to energy and protein requirements by clinical dietitians according to i

Treatment of femoral neck fracture with a minimal invasive surgical approach for hemiarthroplasty - clinical and radiological results in 180 geriatric patients.

PURPOSE: The Direct Anterior Approach (DAA) is well established as a minimal access approach in elective orthopaedic hip surgery. For the growing number of elderly patients with femoral neck fractures treated with Bipolar Hip Hemiarthroplasty (BHH), only a few results do exist. The study shows the clinical and radiological outcome for 180 patients treated by a modified DAA with BHH. MATERIALS AND METHODS: The data of 180 geriatric patients with medial femoral neck fractures were evaluated retrospectively. The general and surgical complications, mobilisation using the Timed Up and Go test (TUG

Undermedication for pain and precipitation of delirium.

INTRODUCTION: Review of the literature revealed an association of pain and delirium in patients with hip fracture. Literature was sparse on pain and delirium in other types of patients. PURPOSE: The purpose of this study was to determine if there was a difference in the amount of analgesia received by patients in pain who developed delirium compared with those who did not develop delirium. METHOD: A sample of 43 medical-surgical patients who were hospitalized with pain and who developed delirium was compared with a matched group of 43 medical-surgical patients with pain who did not develop d

How to try this: Assessment of transient urinary incontinence in older adults.

Urinary incontinence in older adults is associated with an increased risk of institutionalization, as well as with urinary tract infections and depression. Transient urinary incontinence arises suddenly, lasts less than six months, and results from reversible causes. Many caregivers erroneously consider urinary incontinence to be inevitable in older adults, especially in hospitalized patients. Failure to identify and respond to transient urinary incontinence may lead to established incontinence and to other poor outcomes after hospital discharge. A bladder diary and a mnemonic are two methods

Defining ceilings of treatment in hip fracture patients: prevalence, prognostic value, and the role of orthogeriatric co-management.

PURPOSE: Ceilings of treatment, including do-not-resuscitate (DNR), do-not-intubate (DNI), or decisions to forgo intensive care unit (ICU) admission, are increasingly recognised in frail older adults, but evidence in hip fracture care is scarce. We aimed to (1) describe the prevalence and predictors of treatment ceilings, (2) evaluate associations with mortality and length of stay, and (3) assess the association of orthogeriatric care (OGC) with ceilings and outcomes. METHODS: This retrospective cohort study included 1,120 hip fracture patients admitted to Maastricht University Medical Centre
