[Estimation of the transfer of ESBL-producing Escherichia coli to humans in Germany].

In 2011 EFSA has evaluated the risk for the consumer caused by ESBL-/AmpC-producing bacteria in food of animal origin and in livestock animals. Human-to-human transfer in hospitals.and in the community was considered as the most relevant route of transmission for ESBL-producing E. coli. ESBL-/AmpC-producing E. coli are in Germany, as in many other Member States of the European Union, widely spread in food of animal origin and in livestock animals. In an assessment of the relevance of livestock animals as reservoir for ESBL-/AmpC-producing E. coli as well as for ESBL-coding resistance genes the…

Background paper to the recommendation for routine rotavirus vaccination of infants in Germany.

Two rotavirus (RV) vaccines were introduced to the European market in 2006. To support the decision-making process of the German Standing Committee on Vaccination ("Ständige Impfkommission", STIKO) regarding adoption of routine RV vaccination into the national vaccination schedule in Germany relevant scientific background was reviewed. According to STIKO’s Standard Operating Procedures for the development of evidence-based vaccination recommendations, a set of key questions was addressed and systematic reviews were performed with a focus on the efficacy, effectiveness, impact and safety of RV …

The effectiveness of varicella vaccination in children in Germany: a case-control study.

BACKGROUND: Effectiveness of 1 dose of varicella vaccination was estimated to be 85-88% against clinical varicella of any severity in case-control studies in non-European countries, but lower effectiveness has been demonstrated in outbreaks. METHODS: A prospective, age- and practice-matched case-control study was conducted in Germany to assess the effectiveness of 1 dose of OKA/GSK varicella vaccine (derived from the OKA strain, a Japanese clinical isolate) and of any varicella vaccine (including OKA/GSK, OKA/Merck and MMR-OKA/GSK) against polymerase chain reaction (PCR)-confirmed varicella u…

[Current status of vertebroplasty and kyphoplasty in Germany: an analysis of surgical disciplines].

OBJECTIVE: This study was performed to provide an overview of the current practice of balloon kyphoplasty (BKP) and percutaneous vertebroplasty (PVP) in Germany. All surgical departments were evaluated for indications, experience and expertise, and the complications. METHODS: Questionnaires were mailed to 1330 surgical departments. RESULTS: A total of 582 (43.76%) surveys were returned; 257 departments perform neither BKP nor PVP. Osteoporotic compression fractures and to a lesser extent hemangiomas and metastasis were treated. About 30% of the BKP users consider traumatic vertebral fracture…

[Varicella vaccination in Germany. A provisional appraisal in the context of MMR vaccination].

In 2004, a general varicella immunization was introduced in Germany for infants from the age of 11 months, followed by the subsequent recommendation in 2009 of a second vaccine dose. The vaccination is carried out at the same time as the immunization against measles, mumps, and rubella (MMR). Results of the nationwide sentinel surveillance of varicella and herpes zoster implemented by the Varicella Working Group (Arbeitsgemeinschaft Varizellen, AGV) show that the defined goals for varicella immunization (reduction of varicella-related morbidity, complications and hospitalizations) have been re…

[Hantaviruses in Germany: threat for zoo, pet, companion and farm animals?].

Hantaviruses are so-called "emerging" and "re-emerging" viruses because of the new and sudden nature of their appearance. Human infections can lead to two distinct disease patterns, the Haemorrhagic Fever with Renal Syndrome and the Hantavirus Cardiopulmonary Syndrome. All known human pathogenic hantaviruses are transmitted through rodent hosts. There are three rodent-associated hantaviruses in Germany. The bank vole-associated Puumala virus (PUUV) is responsible for most of the human hantavirus infections. The Dobrava-Belgrade virus (DOBV) associated with the striped field mouse is causing ha…

[Vulval hypoplasia in an alpaca cria - first case report in Germany].

This is the first German case report of an alpaca cria suffering from a congenital vulvahypoplasia. Symptoms, diagnostics and therapy are described. The 24-hours-old cria was referred to the hospital due to reduced general condition and colic. The existent vulvahypoplasia led to impaired urination, causing further complications such as urovagina and urinary stasis up to the renal pelvis. Surgical construction of the rima vulvae cured the cria. As indicated by several cases published on the clinical picture of vulvahypoplasia during recent years, this congenital deformation could become an incr

Elimination of terrestrial rabies in Germany using oral vaccination of foxes.

Oral rabies vaccination (ORV) has become the method of choice in fox rabies control in Europe. During the past three decades fox-mediated rabies virtually disappeared from Western and Central Europe. Following Switzerland, Germany was the second European country to launch ORV field trials on its territory in 1983. This paper provides a historical overview on the emergence of fox rabies in Germany; describing the basic principles and milestones of the German rabies eradication programme and presenting results of two decades of efforts to control the disease in foxes. Also, setbacks as well as c…

[Costs of guideline-based treatment of hepatitis B patients in Germany].

The costs of guideline-based treatment are unknown for most diseases. This also applies for economically significant infectious diseases lik … ors. On the assumption that only 25 % of the infected patients are diagnosed, 5-year total costs in Germany account for more than 2.5 billion Euros. Therefore, chronic hepatitis B is a disease with a very high economic burden. The aim of a guideline treatment is to prevent the development of cirrhosis with all its complications as well as the development …

The sylvatic Trichinella cycle and its implications for Trichinella control in Germany.

Trichinellosis is a food-borne, zoonotic disease caused by a parasitic organism. Pork containing muscle larvae represents the most important source of human trichinellosis. In Germany, each slaughtered domestic swine is systematically sampled and examined for Trichinella spp. European Union legislation (EC (No.) 2075/2005) condones the approach of a risk-oriented meat inspection for Trichinella in pigs which is accompanied by monitoring programmes for pig holdings and reservoir animals. Here we discuss the current epidemiological situation of Trichinella in the sylvatic cycle in Germany and th…

[Is the training and continuing education for urologists in Germany still up to date?].

Besides its role in bladder and kidney cancer, urology plays a leading part in oncology particularly with regard to prostate cancer, the most frequent malignant tumor found in men. The multitude of hereditary anomalies of the urogenital tract and the resultant medical conditions, the importance of urinary tract infections including the still deadly urosepsis, urolithiasis which has become as widespread a condition as diabetes mellitus, and urinary incontinence as an increasing problem of a continuously aging population play such a large role in routine practice that every practicing physician

Radiometric dating of the type-site for Homo heidelbergensis at Mauer, Germany.

The Mauer mandible, holotype of Homo heidelbergensis, was found in 1907 in fluvial sands deposited by the Neckar River 10 km southeast of Heidelberg, Germany. The fossil is an important key to understanding early human occupation of Europe north of the Alps. Given the associated mammal fauna and the geological context, the find layer has been placed in the early Middle Pleistocene, but confirmatory chronometric evidence has hitherto been missing. Here we show that two independent techniques, the combined electron spin resonance/U-series method used with mammal teeth and infrared radiofluoresce…

West Nile virus monitoring of migratory and resident birds in Germany.

West Nile virus (WNV) is a mosquito-borne flavivirus naturally circulating in wild bird populations. The virus is also capable to infect a broad range of vertebrate species. Humans and equines are highly susceptible and can develop mild flu-like illnesses as well as severe encephalitis leading to fatalities. Most recently, WNV was found to circulate in countries close to Germany, such as France, Czech Republic, Italy, Austria, and Hungary. Given this epidemiological situation its spread to Germany cannot be ruled out. As no data on the WNV situation were available for Germany for the most rece…

[Tick-borne encephalitis virus in humans and ticks in Northeastern Germany].

Tick-borne encephalitis (TBE) was known to have occurred in humans in the area of Mecklenburg-Western Pomerania in Germany, until 1985. Between 1992 and 2004 more than 16,000 ticks were tested and found to be negative for TBE virus in that area of Germany, wich was therefore thought to be free of TBE. But after 19 years three autochthonous cases of human TBE-infections were identified between 2004 and 2006. We subsequently collected ticks from the three areas where the infection had been acquired and tested them for the presence of TBE-virus RNA with a nested reverse transcription polymerase c…

Current surveys on the prevalence and distribution of Dirofilaria spp. in dogs in Germany.

The Central Upper-Rhine (Baden-Württemberg, Germany) is one of the warmest regions in Germany and also harbours abundant numbers of mosquitoes. Case reports on presumably autochthonous occurrence of Dirofilaria spp. were reported previously and were a reason for a further investigation into the occurrence of vector-borne pathogens. For this purpose, 44 hunting dogs from the Central Upper-Rhine region were tested between 4(th) and 29(th) June 2007. The blood samples were tested using the Knott's test and IDEXX SNAP 4Dx test. The Knott's test revealed unsheathed microfilaria identified as Dirofi…

[Caterpillar dermatitis. An increasing dermatologic problem in warmer regions of Germany].

Caterpillar dermatitis or lepidopterism (Lepidoptera = butterflies) is a toxic-irritant, or rarely allergic, reaction triggered by the release of histamine, thaumetopoein and other kinins from the hairs of butterflies and caterpillars. In Central Europe, the two main causes of caterpillar dermatitis are the oak and pine processionary caterpillar. In addition to cutaneous reactions, patients may develop conjunctivitis, bronchitis and even anaphylactic reactions. We describe the cutaneous aspects of caterpillar dermatitis based on two case reports.

[Practice, problems and perspectives of opioid substitution treatment (OST) in Germany].

Opioid dependency is a complex and chronically relapsing disease with high risks of morbidity and mortality. Frequent relapses and in most of the cases a long process of maturing out characterize this disease. Opioid substitution programs with methadone, buprenorphine and other opioids are a suitable intervention and form the first choice in the treatment of this disease. In Germany, compared to its neighboring countries, this treatment was introduced relatively late. However, in the last five years, the number of patients in substitution treatment has increased significantly to more than 70,0…

Indigenous hepatitis E virus infection of a plasma donor in Germany.

BACKGROUND: Although Europe is supposed to be non-endemic for hepatitis E virus (HEV), locally acquired human cases are registered, and a relatively high prevalence for anti-HEV was found in blood donors in some European countries. Transfusion-transmitted infections by contaminated blood products were reported in Japan and sporadically in Europe. MATERIALS AND METHODS: Several samples from a plasma donor were screened with a highly sensitive quantitative HEV real-time polymerase chain reaction and the full-length genome was generated. Serology was performed with two different commercially ava
