Along the way to developing a theory of the program: a re-examination of the conceptual framework as an organizing strategy.

PURPOSE: Conceptual frameworks (CF) have historically been used to develop program theory. We re-examine the literature about the role of CF in this context, specifically how they can be used to create descriptive and prescriptive theories, as building blocks for a program theory. Using a case example of colorectal cancer screening intervention development, we describe the process of developing our initial CF, the methods used to explore the constructs in the framework and revise the framework for intervention development. METHODS: We present seven steps that guided the development of our CF:

"We are used to this": a qualitative assessment of the perceptions of and attitudes towards air pollution amongst slum residents in Nairobi.

BACKGROUND: People's perceptions of and attitudes towards pollution are critical for reducing exposure among people and can also influence the response to interventions that are aimed at encouraging behaviour change. This study assessed the perceptions and attitudes of residents in two slums in Nairobi regarding air pollution. METHODS: We conducted focus group discussions with residents aged 18 years and above using an emergent design in the formulation of the study guide. A thematic approach was used in data analysis. RESULTS: The discussions revealed that the two communities experience air

'We don't have recipes; we just have loads of ingredients': explanations of evidence and clinical decision making by speech and language therapists.

RATIONALE, AIMS AND OBJECTIVES: Research findings consistently suggest that speech and language therapists (SLTs) are failing to draw effectively on research-based evidence to guide clinical practice. This study aimed to examine what constitutes the reasoning provided by SLTs for treatment choices and whether science plays a part in those decisions. METHOD: This study, based in Ireland, reports on the qualitative phase of a mixed-methods study, which examined attitudes underpinning treatment choices and the therapy process. SLTs were recruited from community, hospital and disability work sett

Teachers' knowledge, attitudes and experience in sexual abuse prevention education in El Salvador.

Research on how to prevent child sexual abuse (CSA) in developing nations is practically non-existent. We sought to determine Salvadoran teachers' knowledge, attitudes, experience and barriers to CSA detection and reporting to assess the need for a CSA prevention programme and to assess barriers in conducting such a programme. One hundred teachers completed a questionnaire while they visited the Tin Marin Children's Museum in San Salvador. Nineteen of these teachers also participated in a focus group. We found that 89% of teachers reported at least two signs and symptoms of child abuse. One hu

Alloimmunisation during pregnancy in Greece: need for nationwide HDFN prevention programme.

AIM/OBJECTIVES: To access the incidence and specificity of maternal red blood cells alloimmunisation and its relevant clinical impact in Greece. BACKGROUND: The rate of alloimmunisation in pregnant women in Greece is unknown. MATERIALS/METHODS: We performed a 4-year study in two tertiary hospitals in Greece. Demographics, transfusion and obstetric history were analysed. Maternal alloimmunisation was detected with indirect anti-globulin test. RESULTS: We investigated 4368 pregnant women. Of which 3292 (75·37%) were Greek and 1076 (24·63%) were migrants. In 39 alloimmunised women, 41 alloanti

Assessment of adherence problems in patients with serious and persistent mental illness: recommendations from the Expert Consensus Guidelines.

Poor adherence to medication treatment can have devastating consequences for patients with serious mental illness. The literature review and recommendations in this article concerning assessment of adherence are reprinted from The Expert Consensus Guideline Series: Adherence Problems in Patients with Serious and Persistent Mental Illness, published in 2009. The expert consensus survey contained 39 questions (521 options) that asked about defining nonadherence, extent of adherence problems in schizophrenia and bipolar disorder, risk factors for nonadherence, assessment methods, and intervention

Vesicovaginal fistula: do the patients know the cause?

BACKGROUND: So much has been written on vesicovaginal fistula (VVF) but there is little on the patients' perspective of the condition. The objectives of this study were to determine the knowledge of patients who have developed VVF on the causes of the fistula and their attitude toward measures that would prevent future occurrence. METHODS: The questionnaire-based survey was conducted on VVF patients on admission from June to August 2003 at Maryam Abacha Women and Children Welfare Hospital, Sokoto, Nigeria. The case notes of the patients were reviewed after the interview to match the responses

Improving minority blood donation: anthropologic approach in a migrant community.

BACKGROUND: As a result of blood group polymorphism, distinctive blood types have evolved in populations around the world. In countries with large migrant populations, finding rare blood types for transfusion can be challenging. This is especially true for sub-Saharan African immigrants living in countries with predominantly European populations. This problem is further compounded by hereditary disorders such as sickle cell disease and by traditional cultural values that discourage routine donation. STUDY DESIGN AND METHODS: The purpose of this report is to describe the drive to recruit more

Concepts of risk among young Swedes tested negative for HIV in primary care.

OBJECTIVE: To explore why young adults test for HIV, how they construct the HIV risk, and what implications testing has for them. DESIGN: Six tape-recorded focus-group interviews were transcribed verbatim and analysed according to a grounded theory approach. SUBJECTS AND SETTING: Women and men between 18 and 24 years of age were recruited from a youth clinic in northern Sweden in 2004-05 after being tested and found to be HIV negative. RESULTS: A core category--reconsidering risk and four categories--HIV: a distant threat; the risk zone; responsibility: a gendered issue; a green card-- emer

[Typical and avoidable doctor's errors in surgical gastroenterology].

Everyone makes mistakes, and even the greatest efforts may fail to eliminate them completely. In contrast, typical errors and miscalculations--which by the very fact of being typical are predictable--can and must be avoided. The reasons for such mistakes may be lack of experience, inadequate interdisciplinary cooperation, and ignorance of the current state of the art. This applies equally, and in particular, to the field of surgical gastroenterology. Intensive interdisciplinary discussions, a healthy surgical hierarchy, a knowledge of pertinent guidelines and the latest literature and, last, b

Taking health care back: the physician's role in quality improvement.

Physicians now enjoy a moment of tactical advantage in the evolution of the struggle for control over health care in the United States. The most effective way to capitalize on this-perhaps fleeting-position and to more permanently alter the balance of power in their favor is for physicians to establish strong and visionary leadership in health care quality improvement. Such an undertaking, if successful, could place the very essence of health care-defining, measuring, and improving its quality-in the hands of physicians. To succeed requires understanding the relationships between the different

Knowledge, attitudes, and practices of parents of asthmatic children in Cape Town.

To obtain information useful to asthma care in a relatively poor, high asthma prevalence population, a focus group study was undertaken with 72 parents of children with asthma or recurrent wheezing. There was a reasonable level of understanding of the causes of asthma, although variable acceptance of the diagnosis. Willingness to undertake home management of acute episodes and environmental control measures was high. Reported treatment was characterized by reliance on syrups, use of home remedies, resistance to inhaled therapy, and relatively low compliance with prescribed treatment. There was

Formative research for interventions among adolescents at high risk for gonorrhea and other STDs.

The gonorrhea infection rate among 15-19 year olds during 1993-1994 in an urban Upstate New York community was nearly twice the national norm. More than 50% of all the gonorrhea cases were concentrated in eight census tracts. This study was conducted to provide data regarding sexually transmitted diseases among high risk adolescents to increase successful public health interventions. The methodology consisted of six focus groups and a self-administered survey. The results determined the extent of accurate knowledge, their risk behaviors and beliefs, the barriers to treatment, and suggested int

Adolescents' communication styles and learning about birth control.

Adolescents have different communication styles for acquiring information about birth control from a mass medium--interactant or noninteractant, depending on whether they involve communication with others in the process of media use. Three interactant styles are identified: a home-oriented style, where communication is primarily with immediate members and friends; a peer-oriented style, where communication is primarily with own-age peers and personal friends; and multi-source user, where communication is with more than one cluster of sources and the majority of one's communication is not with

Gaining a comprehensive understanding of antimicrobial resistance in dental education: a mixed methods study protocol.

BACKGROUND: Antimicrobial resistance (AMR) is one of the most pressing global health challenges of the 21st century, driven in part by suboptimal prescription of antibiotics. Most antibiotic use worldwide occurs in outpatient settings, including dental practice, where dentists account for approximately 5-11% of all prescriptions. Educational environments play a critical role in shaping prescribing behaviors early in professional development, and interventions implemented during undergraduate training are more effective at changing clinical practice than those introduced later in clinical caree

Adapting global guidelines to local contexts: optimising community-acquired pneumonia (CAP) specific prescribing in Pakistan to counter antimicrobial resistance.

BACKGROUND/OBJECTIVES: Community-acquired pneumonia (CAP) imposes a significant health burden among low- and middle-income countries. The burden is exacerbated by antimicrobial resistance (AMR), often due to inappropriate antibiotic agent use and gaps in antimicrobial stewardship activities. This study aimed to explore physicians' perspectives on the diagnosis, treatment and prevention of CAP in Pakistan, with a focus on how international guidelines are interpreted and adapted to local clinical realities. METHODS: A qualitative study was conducted using semistructured interviews with 33 purpo

Why do general practitioners not screen and intervene regarding alcohol consumption in Slovenia? A focus group study.

AIM: To identify barriers influencing general practitioners' decisions regarding alcohol screening and brief intervention (SBI) in Slovenia. BACKGROUND: Slovenia occupies third place in a league of 51 European countries with respect to alcohol consumption. General practitioners in Slovenia have the majority of contacts with patients in primary healthcare but they rarely or never ask patients about their drinking habits. METHOD: Six focus groups with a total of 32 general practitioners from different parts of the country were set up. Participants discussed varied topics and the most significa

The prevalence of oral pathoses in a private dental practice: a 30 month survey.

The purpose of this study was to determine the prevalence of oral lesions in a private dental practice, which treats a broad spectrum of patients from both urban and rural areas. All oral lesions were documented over a 30 month period in a research programme, which formed part of the normal patient treatment plan. The lesions were analysed according to the prevalence of the lesions, race, age and gender distribution, and the ratio of lesions diagnosed by the practitioner compared with those presented for treatment by the patients. A total of 8418 patients were examined and 151 pathological les

An intervention to improve the assessment of alcoholism by practicing physicians.

OBJECTIVE: Alcoholism is estimated to affect at least 10% of American adults. Despite the fact that early diagnosis is possible and early treatment can prevent great suffering, American physicians typically diagnose and treat alcoholism less than half the time. Several authors have suggested additional physician education as a possible way to improve alcoholism assessment skills and increase the diagnosis rate. METHODS: A baseline audit, an educational intervention, and a post-intervention audit were done with physicians and nurse practitioners in an urban family practice group, using informa
