Nuclear bodies in nerve cells of two patients with normal-pressure hydrocephalus.

Biopsy specimen fragments from two patients with normal-pressure hydrocephalus were studied using ultrastructural methods. Simple nuclear bodies were found in all layers of the cortex and white matter, whereas complex ones were only seen in the fifth and sixth layers and in the underlying white matter. There may be a relationship of these nuclear bodies, either to the neuroglial proliferation observed in both patients or to the existence of a viral infection of the CNS.

Normal pressure hydrocephalus, parkinsonism, and primary empty sella--coincidence or cause-effect?

The triad of normal-pressure hydrocephalus, parkinsonism, and primary empty sella appeared in a 70-year-old woman. There was dramatic resolution of the extrapyramidal signs and symptoms on treatment with levodopa-carbidopa and ventriculojugular shunt. The syndrome may be the result of several mechanisms, but an alteration of cerebrospinal fluid (CSF) pressure dynamics may be the sole etiology for this triad.

Normal pressure hydrocephalus. Recognition and relationship to neurological abnormalities in Cockayne's syndrome.

Normal pressure hydrocephalus (NPH) in adults is a well-known cause of dementia. We describe NPH in children having the recessively inherited Cockayne's syndrome (CS). Cockayne's syndrome is characterized by cachectic dwarfism, neurological dysfunction, and cutaneous sunlight sensitivity. We noted that the NPH-associated triad of dementia, gait disturbance, and incontinence developed in CS patients. Computerized tomography of the brain in our four CS patients showed hydrocephalic enlargement of the brain ventricles greatest in the older patients. There was no evidence of cortical atrophy excep

Syndrome of normal pressure hydrocephalus: possible relation to hypertensive and arteriosclerotic vasculopathy.

A patient with clinical features of idiopathic normal pressure hydrocephalus, who responded dramatically to shunting, was found a necropsy to have a severe hypertensive and arteriosclerotic vasculopathy with multiple lacunar infarcts. There was no pathological evidence of thickened leptomeninges, fibrosis of the arachnoid villi, or Alzheimer's disease. An abnormal absorption mechanism was demonstrated with cisternography and by an increase in the concentration of homovanillic acid in the cerebrospinal fluid. It is suggested that vascular changes may play an important role in the pathophysiolog

"Normal" pressure hydrocephalus in chronic inflammatory diseases: a clinical and laboratory study.

A review of the 21 cases of "normal" pressure hydrocephalus (N.P.H.) identified by means of radioisotope cisternography, seems to support the basic views of Adams et al. (1965) that a chronic inflammatory disease is an important cause of N.P.H. The clinical and laboratory date made it possible to identify a substantial number of cases affected by N.P.H. in the course of a chronic inflammatory disease; of particular interest were those affected by syphilis. In this connection it seems possible that the symptoms of classical neurosyphilis may be very similar to those often seen in patients with

[Cerebral cortex in normal pressure hydrocephalus: an electron microscopy study (author's transl)].

A cortical biopsy was taken at the time when ventricular pressure was measured, or a ventriculo-atrial shunt placed, in 11 patients with Normal Pressure Hydrocephalus (N.P.H.), and was studied with the electron microscope. In 9 cases, a rather typical electron microscopic picture with marked increase of extracellular space in an otherwise normal cortical neuropil was observed. 2 patients did not exhibit this pattern and, coincendentally, did not benefit from the placing of the shunt. As a control, about 600 cortical biopsies that had been taken for other reasons were reviewed. In the group, th

Disproportionately enlarged subarachnoid-space hydrocephalus (DESH) in normal pressure hydrocephalus misinterpreted as atrophy: autopsy and radiological evidence.

Disproportionately enlarged subarachnoid-space hydrocephalus (DESH), a feature of normal pressure hydrocephalus (NPH), is often misinterpreted as cortical atrophy. We report a 67-year-old man with features of NPH but not diagnosed because radiographic findings were interpreted as cortical atrophy. Autopsy showed findings consistent with NPH and no neurodegenerative disease. The second patient with DESH underwent shunt surgery. Entrapped fluid diminished after the surgery, confirming this is not atrophy, but due to a CSF dynamic process. Patients with DESH have tight sulci adjacent to the entra

From the past to the present: evolving theories in the pathophysiology of normal pressure hydrocephalus.

Over recent decades, various hypotheses and theoretical frameworks have been advanced to elucidate the aetiology of normal pressure hydrocephalus (NPH). This reversible neurological condition, characterised by the classical clinical triad of gait disturbance, urinary incontinence and cognitive impairment, represents a multifactorial interplay of pathophysiological processes that co-occur, rather than originating from a single, defined cause. Despite extensive research efforts, the precise aetiology and underlying pathophysiological pathways remain indeterminate. Contributory factors such as dy

Time to Reconsider the Value of Shunting Procedures for "Idiopathic Normal Pressure Hydrocephalus".

The concept of "normal pressure hydrocephalus" dates back to the description by Hakim and Adams in 1965 of 2 series of patients with enlarged lateral ventricles, high normal cerebral spinal fluid (CSF) pressure and a triad of cognitive impairment, urinary incontinence, and gait apraxia. The validity of this concept is based upon the reversal of its symptoms by a shunting procedure. However, all of these patients had secondary communicating hydrocephalus after previous episodes of meningitis, brain trauma, or subarachnoid hemorrhage. Nevertheless, shunting procedures for "idiopathic normal pres

Technological Advances for Gait and Balance in Normal Pressure Hydrocephalus: A Systematic Review.

Normal pressure hydrocephalus (NPH) is a recognized cause of reversible cognitive and motor decline, with gait and balance impairments often emerging early. Technologies providing gait and balance measures can aid in early detection, diagnosis, and prognosis of the disease. This systematic review comprehensively discusses previous studies on the instrumental assessment of gait and balance in NPH. A PubMed search following PRISMA guidelines identified studies published between 2000 and 2024 that used laboratory instruments to assess gait and balance in NPH. Studies underwent quality assessment

Value of biomarkers in the prediction of shunt responsivity in patients with normal pressure hydrocephalus.

Preoperative differentiation between responders and non-responders to ventriculoperitoneal (VP) shunting in the treatment of normal pressure hydrocephalus (NPH) remains a significant challenge. Identifying biomarkers in presurgical assessment represents a promising approach to reducing the need for invasive cerebrospinal fluid CSF testing. In this prospective observational study, thirty adult patients were classified into Group A (responders to VP shunting) and Group B (non-responders) based on their responsiveness to invasive CSF testing. The overall clinical condition and Idiopathic NPH (iNP

A Soft Robotic Actuator System for In Vivo Modeling of Normal Pressure Hydrocephalus.

OBJECTIVE: The intracranial pressure (ICP) affects the dynamics of cerebrospinal fluid (CSF) and its waveform contains information that is of clinical importance in medical conditions such as hydrocephalus. Active manipulation of the ICP waveform could enable the investigation of pathophysiological processes altering CSF dynamics and driving hydrocephalus. METHODS: A soft robotic actuator system for intracranial pulse pressure amplification was developed to model normal pressure hydrocephalus in vivo. Different end actuators were designed for intraventricular implantation and manufactured by

Utility of Machine Learning in the Management of Normal Pressure Hydrocephalus: A Systematic Review.

BACKGROUND: In the past decade, many machine learning (ML) models have been used in the management of normal pressure hydrocephalus (NPH). This study aims at systematically reviewing those ML models. METHODS: The PubMed, Embase, and Web of Science databases were searched for studies reporting applications of ML in NPH. Quality assessment was performed using Prediction model Risk Of Bias ASsessment Tool (PROBAST) and Transparent Reporting of a multivariable predication model for Individual Prognosis Or Diagnosis (TRIPOD) adherence reporting guidelines, and statistical analysis was performed wi

The Causal Effect of Gut Microbiota on the Development of Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND: Older people are particularly at risk for idiopathic normal pressure hydrocephalus (iNPH), a rare neurological condition without known risk factors. Several recent observational studies have reported that gut microbiota composition is associated with iNPH. However, the causal impact of gut microbiota on iNPH is unknown. METHODS: Single nucleotide polymorphisms strongly associated with 196 gut microbiota (18,340 individuals) were selected as instrumental factors, and the results were iNPH data (322 cases of iNPH and 21,043 controls). Random effects, inverse variance weighting, weig

Parkinsonism in idiopathic normal pressure hydrocephalus: is it time for defining a clinical tetrad?

BACKGROUND: Association between parkinsonism and idiopathic normal pressure hydrocephalus (iNPH) still remains debated. There is already plenty of evidences in the literature suggesting that this clinical sign can be considered as an integral part of the clinical spectrum of iNPH patients. METHODS: We reviewed the possible pitfalls in the core clinical definition of iNPH based on available international diagnostic criteria, phenomenology of parkinsonism in iNPH, and neuroimaging supporting the presence of parkinsonism in iNPH. CONCLUSIONS: We argue that the diagnostic definition of the iNPH

Can the hummingbird sign contribute to the diagnosis of idiopathic normal pressure hydrocephalus?

BACKGROUND: We read with great interest the article by Atalay et al. in which they illustrate the frequent presence of hummingbird signs in their patient group with idiopathic normal pressure hydrocephalus (iNPH) [1]. The results are substantially interesting; however, we believe that some points may be further deliberated for a better understanding of this crucial study. DISCUSSION: The authors declare that there are no commercial, financial, and other relationships in any way related to the subject of this article that might create any potential conflict of interest.
