INTRODUCTION: Primary hyperparathyroidism is a relatively rare disease caused in 80-85% of cases by solitary adenoma of the parathyroid glands. The laboratory findings are hypersecretion of PTH and hypercalcaemia. We distinguish between asymptomatic and symptomatic primary hyperparathyroidism. 25 patients of our clinic who underwent surgery in 1996 and 1997 are presented to illustrate our surgical concept of therapy.
METHODS: 7 patients were asymptomatic and 18 symptomatic with regard to primary hyperparathyroidism. Preoperative localisation was facilitated by ultrasonography of the neck, whi…

The treatment of secondary hyperparathyroidism (HPT) in patients with chronic renal disease has improved markedly in recent years. The skeletal pain, disabling fractures, tendon ruptures, and myriad other symptoms associated with HPT can now be avoided, and the quality of life of patients with end-stage renal disease is improved. Control of hyperphosphatemia, maintenance of normocalcemia, and appropriate dosing of vitamin D analogues can prevent HPT in many cases. Palatable, nutritious diets should be followed; serum calcium, phosphorus, alkaline phosphatase, and parathyroid hormone should be …

Primary hyperparathyroidism is diagnosed with increasing incidence. Many patients are misinterpreted as being asymptomatic, but they do indeed suffer from a broad variety of non-specific symptoms. Investigations show that patients with primary hyperparathyroidism may be lethargic, apathic, demented and depressed. Many suffer from bone and joint pain. Surgical treatment can be done with few complications and a high cure rate, resulting in reversal of symptoms. Since symptoms do not correlate with S-Ca++ or S-parathyroid hormone and since the symptoms can be attributed to many other causes, we n…

Nutritional secondary hyperparathyroidism (NSH) was diagnosed in six cats during a three-year period, based on clinical, radiographic and laboratory findings. Clinical signs were attributable to severe osteopenia (n = 5) and hypocalcaemia (n = 4), which had resulted in spontaneous fractures of long bones, scapulae, pelvis, nasal bones, or spine, and in excitation, muscle twitching or seizures, respectively. Serum parathormone levels were markedly elevated, and 1,25(OH)2-vitamin D3 mildly elevated, whereas 25(OH)-vitamin D3 was mildly decreased compared to age-matched healthy cats. Treatment wa…

BACKGROUND: Primary hyperparathyroidism is a rare but important cause of ventricular arrhythmias. METHODS AND RESULTS: The medical records of a patient with primary hyperparathyroidism and polymorphic ventricular tachycardia was reviewed. The patient was serially interviewed and examined before and after parathyroidectomy. A literature search was performed. The association of primary hyperparathyroidism and polymorphic ventricular tachycardia is rare. Hypokalemia and hypomagnesemia occur infrequently in primary hyperparathyroidism. CONCLUSIONS: The potential for malignant ventricular arrhythmi…

Hyperparathyroidism (HPT) is a common finding in patients with end-stage renal disease (ESRD). Most chronic peritoneal dialysis (CPD) patients with HTP can be successfully managed with phosphate binders, calcium supplements, and calcitriol therapy. Noncompliance with diet, prescribed binder, or calcitriol therapy may also lead to HPT. We reviewed New Haven CAPD unit patients who failed medical therapy and required parathyroidectomy (PTX) for control of severe HPT [sustained immunoreactive parathyroid hormone (iPTH) level > 600 pg/mL]. From 1990 to 1997, 18 out of 620 patients (3.0%) required P…

BACKGROUND: Pancreatitis is associated with primary hyperparathyroidism (PHPT) in 1.5-7% of cases. The relationship of cause and effect between the two diseases has been debated.
METHODS: To evaluate this relationship, the clinical, biochemical and pathological data on 1435 patients operated on for hyperparathyroidism (HPT) over the past 30 years were retrospectively reviewed. A total of 1224 of these patients had biologically proven and cured PHPT and 211 patients had renal HPT (RHPT). The diagnosis of pancreatitis (PTS) was based on a high serum amylase level and/or abnormalities on ultraso…

BACKGROUND: The persistence of secondary hyperparathyroidism after renal transplantation is frequent and often complicated by overt hypercalcemia. Recent investigations have shown an effect of the different vitamin D receptor (VDR) genotypes on parathyroid hormone (PTH) secretion in both primary and secondary hyperparathyroidism. The aims of this study were (i) to assess whether persistent secondary hyperparathyroidism after renal transplantation is characterized by any change in calcium-controlled PTH secretion, and (ii) whether different VDR allelic distributions might play any role on this …

We report two cases of pseudogout after parathyroidectomy by primary hyperparathyroidism. The pseudogout in each instance developed within 48 hours after parathyroid adenoma resection. Calcium supplement therapy, nonsteroidal anti-inflammatory drugs and colchicine (1 mg/day) suppressed the acute attack. The chondrocalcinosis is often asymptomatic or undiagnosed, for that, preoperative radiological studies of the knees, wrists and pelvis are recommended to screen for chondrocalcinosis. We advocate also, therapy with colchicine (1 mg/day, oral) in the prophylaxis of postoperative pseudogout.

Within a prospective case control study from 1993 to 1996, in 85 first operations for primary hyperparathyroidism the neck exploration was ended unilaterally when an adenoma (also histologically proven) and one normally small parathyroid gland were found. Otherwise or in case of simultaneous surgery of the thyroid gland, a bilateral exploration was performed. The primary success rate with this strategy, additionally based on 69.4% correct results of cervical ultrasound, was 95.3%, with a significant saving of an average of 30 min. operating time.

There is a very high incidence of hyperparathyroidism secondary to chronic renal insufficiency in dialysed patients (67%). When medical therapy can no longer control the evolution of disease, an appropriate surgical strategy must be defined for the clinical case. Instrumental tests play an important role during the preoperative phase in the possible "localisation" of ectopic parathyroids (10% of cases) and the identification of those glands with a more pronounced hyperplastic appearance. The authors examine and describe the most useful tests for a preoperative study, namely ultrasonography, ec…

Primary hyperparathyroidism (PHP) is most common in adults in the fifth and sixth decades of life, but is seen infrequently in adolescence and is extremely rare in children under the age of 14 years. We report the case of a young female whom we suspect had PHP from the age of 6 years. This condition remained undiagnosed for the next 11 years in spite of repeated hospital admissions. The hypercalcaemia which was initially labelled as 'idiopathic' was eventually found to be secondary to a parathyroid adenoma, which was treated successfully by surgery. The case illustrates two important advances:…

Nonmedullary thyroid carcinoma is known to occur in association with primary hyperparathyroidism. A combination of secondary, uremic, hyperparathyroidism and non-medullary thyroid carcinoma is rare and was hitherto reported in only 12 cases. We report another three patients with this parathyroid/thyroid disease combination, suggesting that it may represent not merely a coincidence. A number of factors active in secondary hyperparathyroidism may play a role in the induction and/or promotion of the thyroid cancer. These include parathyroid endocrinopathy, goiterogenic effect of prolonged hyperca…

Primary hyperparathyroidism may be caused by one or two benign tumors in separate glands, adenoma, a malignant tumor of one gland, carcinoma or hyperplasia of all four glands. Pathologically, the main problem lies in distinguishing between primary chief cell hyperplasia and adenoma, it is impossible from pathological findings of only one gland. Parathyroid adenomas and chief cell hyperplasias contain large numbers of active chief cells. The cells contain aggregated arrays of rough endoplasmic reticulum and large, complex Golgi apparatus with numerous vacuoles and vesicles. Secretory granules a…

A 73-year-old woman was diagnosed as primary hyperparathyroidism. Hypercalcemia recurred after parathyroidectomy, though serum calcium concentration temporarily resumed to normal. Hypercalcemia was eventually shown to be due to multiple myeloma. In spite of the frequent association of hypercalcemia with malignant disease and the relative frequency of primary hyperparathyroidism as a cause of hypercalcemia, primary hyperparathyroidism accompanied by multiple myeloma has been considered in only a limited number of reports. Here, the cases in the literature are reviewed and discussed.

Hyperparathyroidism in association with carotid body tumor is a rare combination. A common embryologic cellular origin was postulated previously to explain this unusual occurrence. We report a case of left carotid body tumor and left inferior parathyroid adenoma. We believe this to be the sixth reported case in the literature with this combination.
