Immediate hypersensitivity phenomena in bullous pemphigoid: critical concepts.

The immediate hypersensitivity phenomena in bullous pemphigoid (BP) will be discussed in this review. There is prominent involvement of eosinophils in the pathogenesis of human BP. Activation of these eosinophils in BP may lead not only to tissue damage, but also to regulation of the immune response. Stimulation of Th2 cells is responsible for the production of IgG4 and IgE autoantibodies to the hemidesmosomal proteins, 180-kDa BP antigen and 230-kDa BP antigen.

Role of food hypersensitivity in irritable bowel syndrome.

A significant proportion of IBS patients attribute their symptoms to adverse food reactions. Dietary elimination and re-challenge studies support the role of diet in the pathogenesis of IBS. The aetiopathogenesis of IBS is thought to be multifactorial involving an interaction between diet, infection, antibiotics and psychosocial factors. Serum IgE and IgG4 antibodies are elevated in food hypersensitivity induced atopic conditions and a similar mechanism has been postulated in IBS. Increased number of mast cells is present in the ileocaecal region of IBS patients. Once sensitized, they are capa…

Haemorrhagic hypersensitivity pneumonitis due to naphthylene-1,5-diisocyanate.

Symptoms of hypersensitivity pneumonitis and massive pulmonary haemorrhage occurred in a 24-yr-old male shortly after occupational exposure to naphthylene-1,5-diisocyanate (NDI). The present examination was performed approximately 1-yr after the initial life-threatening haemoptysis and following an uneventful recovery after resection of the middle lobe, which had been identified bronchoscopically as the bleeding source. Histological re-examination of the lung was compatible with hypersensitivity pneumonitis. After a chamber challenge with NDI (5 parts per billion (ppb) for 10 min, 10 ppb for 1…

A case report of olanzapine-induced hypersensitivity syndrome.

Hypersensitivity syndrome is defined as a drug-induced complex of symptoms consisting of fever, rash, and internal organ involvement. The hypersensitivity syndrome is well recognized as being caused by anticonvulsants. Olanzapine is an atypical antipsychotic agent whose side effects include sedation, weight gain, and increased creatinine kinase and transaminase levels. To date, there have been no reports of hypersensitivity syndrome related to this drug. A 34-year-old man developed a severe generalized pruritic skin eruption, fever, eosinophilia, and toxic hepatitis 60 days after ingestion of …

Neurophysiological study of patients with perceived 'electrical hypersensitivity'.

The aim of the present study was to investigate baseline neurophysiological characteristics of the central and autonomous regulation and their reactivity to different tests in a group of persons with so-called 'electrical hypersensitivity', which is often considered as a form of psychosomatic disorders. Twenty patients with combinations of neuroasthenic symptoms (general fatigue, weakness, dizziness, headache) and facial skin (itching, tingling, redness) have been investigated. An equal number of symptom-free persons served as a control group. The examination comprised self-reported measures, …

Central hypersensitivity in chronic pain after whiplash injury.

OBJECTIVE: The mechanisms underlying chronic pain after whiplash injury are usually unclear. Injuries may cause sensitization of spinal cord neurons in animals (central hypersensitivity), which results in increased responsiveness to peripheral stimuli. In humans, the responsiveness of the central nervous system to peripheral stimulation may be explored by applying sensory tests to healthy tissues. The hypotheses of this study were: (1) chronic whiplash pain is associated with central hypersensitivity; (2) central hypersensitivity is maintained by nociception arising from the painful or tender …

Immunoglobulin E-mediated hypersensitivity in food-producing animals.

Type I hypersensitivity has been described as a cause of allergic reactivity to inhalants, injectables, endoparasites, and ectoparasites in food animal species. In addition, IgE is credited with showing some host-sparing effect when produced in response to certain gastrointestinal and other parasites. Recently, the sophistication of diagnostic procedures has increased with the elucidation of epsilon heavy chain sequences, expressed protein, development of chimeric IgE antibodies, and production of species-specific anti-IgE reagents. Application of ELISA and Western blotting has replaced the pa…

Latex hypersensitivity in spinal cord injured adult patients.

Introduction  Tinnitus is experienced by a significant part of the patients suffering from otosclerosis. Objective  To assess the prevalence of tinnitus in otosclerosis, its main features, and the impact on the daily life. Methods  Patients diagnosed with otosclerosis in 2019 in a tertiary hospital were enrolled in the study. Demographic data were retrieved and, besides a regular audiometric evaluation, the patients underwent acuphenometry to assess the psychoacoustic measurements (pitch and loudness), and the Tinnitus Handicap Inventory (THI). Results  In total, 66 patients fulfilled the incl

Anticonvulsant hypersensitivity syndrome associated with reactivation of cytomegalovirus.

Recently, it has been proposed that viral infection is involved in the pathogenesis of hypersensitivity syndrome. Cytomegalovirus (CMV), one of the aetiological agents of infectious mononucleosis, has never been reported as an organism associated with hypersensitivity syndrome. We describe a 64-year-old man with severe phenytoin-induced hypersensitivity syndrome associated with CMV infection. Twenty-five days after the patient was started on phenytoin, he developed high fever and a generalized erythematous rash followed by jaundice, renal failure and disseminated intravascular coagulopathy (DI…

[Hypersensitivity to Anisakis simplex: apropos of 36 cases].

The authors report a group of 36 cases of hypersensitivity to Anisakis simplex, a fish parasite. They examined 11 boys and 25 girls who attended the Allergy service of the Central Hospital Marques of Valdecilla in Santander (Spain), between 12th May 1997 and 26th June 1998, for history of acute urticaria, most often after an aggravation of an allergic pathology. All were given an immunological examination that was composed of: food investigation, prick test, measurement of total and specific IgE and blood eosinophilia. In conclusion, the authors emphasize an increase in hypersensitivity to Ani…

Modulation of hypersensitivity reaction by lipids given orally.

The effect of lipids administration by gavage (0.4% body weight) given daily during four weeks on the hypersensitivity reaction in trachea, upper and lower bronchi, liver, kidney, mesentery, and pancreas was investigated in male rats. The plasma exudation was assessed by using Evans blue (EB) dye extravasation method. There was a significant difference in the permeability of the organs in nonimmunized rats. The immunization increased the vascular permeability and the response with the organs varied greatly. The effect of lipids on anaphylactic reaction was compared to those of untreated rats (…

Food hypersensitivity and allergic disease: a selective review.

Food intolerance can be caused by immunologic, pharmacologic, toxic, infectious, idiosyncratic, metabolic, and neuropsychologic processes. Food allergy denotes an adverse reaction to food or food additives in which an underlying immunologic mechanism can be shown. Its incidence in young children is approximately 1.3% and among adults is 0.3%. Parental history of atopy is a significant causal factor. In addition, exposure to common allergenic foods in infancy increases risk. For these reasons, exclusive breast-feeding and maternal avoidance of peanut, egg, fish, and dairy products during lactat

Hypersensitivity pneumonitis from residential composting: residential composter's lung.

Hypersensitivity pneumonitis results when a susceptible individual is exposed to sufficient airborne material capable of inducing a systemic and pulmonary immune response. We describe a man who had all the classic manifestations of hypersensitivity pneumonitis but in whom the circumstances of sensitization, residential yard work with composted yard clippings, has not previously been reported. We call this new entity residential composter's lung.

Aetiological, predisposing and environmental factors in dentine hypersensitivity.

If the hydrodynamic theory of stimulus transmission across dentine is valid, sensitive dentine has tubules open at the surface and patent to … or of sensitivity. An increase in our understanding of the aetiological factors involved in dentine hypersensitivity is essential if a sound approach to its management is to evolve. Such information could emanate from epidemiological studies, clinical investigations and laboratory experiments.
