Mechanisms of action of intravenous immune globulin in immune-mediated diseases.

Intravenous immune globulin (IVIG) exhibits a number of immunomodulatory properties that are mediated by the Fe portion of IgG and by the spectrum of variable (V) regions contained in the immune globulin preparations. Five predominant and non-exclusive mechanisms of action have been proposed to account for the immunomodulatory effects of IVIG in immune-mediated diseases: (i) functional blockade of Fc receptors on splenic macrophages; (ii) inhibition of complement-mediated damage, an effect that is dependent on the ability of IgG to bind C3b and C4b and thus reduce the number of activated compl

Immune profiling in orthopaedic diseases using mass cytometry : a narrative review of applications and future directions.

AIMS: Musculoskeletal (MSK) diseases involve complex immune mechanisms. Conventional flow cytometry is restricted by spectral overlap, limiting multidimensional immune characterization. Mass cytometry by time-of-flight (CyTOF) can measure over 40 proteins per cell using metal-tagged antibodies and time-of-flight detection, providing deep immune phenotyping. The aim of this narrative review was to summarize the applications of CyTOF in the study of human orthopaedic and rheumatic diseases, highlight key immune findings including shared markers, and identify gaps to guide future research and pan

The multiple functions of CD5 in diseases related to immune disorders.

CD5 is a critical regulator of the immune system and a transmembrane glycoprotein expressed on T cells and the surface of certain B cells. Over the years, numerous studies have demonstrated that CD5 is involved in the regulation of immune responses, maintenance of immune tolerance, and modulation of cell survival and apoptosis. Beyond these functions, CD5 also plays a role in immunotherapy, recognition of pathogen-associated molecular patterns, regulation of inflammatory processes, prediction of poor tumour prognosis, and clearance of macromolecules. Immune aberrant disorders-such as infection

The intestinal neuro-immune axis: crosstalk between neurons, immune cells, and microbes.

The gastrointestinal tract is densely innervated by a complex network of neurons that coordinate critical physiological functions. Here, we summarize recent studies investigating the crosstalk between gut-innervating neurons, resident immune cells, and epithelial cells at homeostasis and during infection, food allergy, and inflammatory bowel disease. We introduce the neuroanatomy of the gastrointestinal tract, detailing gut-extrinsic neuron populations from the spinal cord and brain stem, and neurons of the intrinsic enteric nervous system. We highlight the roles these neurons play in regulati

The shifting balance: innate lymphoid cell plasticity in chronic gut diseases.

Innate lymphoid cells (ILCs), as core components of innate immunity, coordinate mucosal immune defense and homeostasis through their functional plasticity. They exhibit functional diversity in the intestine and display a complex dual role in intestinal diseases, which is closely associated with their phenotypic transformation and functional reprogramming. Current studies have demonstrated that ILCs play critical roles in various chronic intestinal diseases, including autoimmune intestinal disorders, chronic infectious intestinal inflammation, and colorectal cancer. However, the specific functi

Association between Guillain-Barré syndrome and 7 autoimmune diseases: a mendelian randomization study.

BACKGROUND: The genetic and environmental factors influencing an individual's susceptibility to develop Guillain-Barré syndrome (GBS) remain largely unidentified. This study aims to evaluate the genetically predicted associations between GBS and candidate autoimmune diseases using a Mendelian randomization (MR) framework. METHODS: Genetic instruments for autoimmune diseases were extracted from large-scale genome-wide association study (GWAS) datasets. Following stringent quality control, seven autoimmune diseases were ultimately retained for the primary analysis. Summary statistics for GBS we

Novel insights into the role of microRNAs in atopic dermatitis and psoriasis: diagnostic and therapeutic aspects.

Atopic dermatitis (AD) and psoriasis are prevalent chronic inflammatory skin diseases with distinct clinical features and significant impacts on patients' quality of life. AD typically presents as recurrent eczema-like lesions accompanied by intense itching, whereas psoriasis is characterized by well-defined erythematous, scaly epidermal plaques. AD is associated with skin barrier dysfunction, allergen sensitization, and a type 2 immune response, while psoriasis involves hyperproliferation, abnormal differentiation of epidermal keratinocytes, and immune cell infiltration. However, the precise

Phenotypic diversity and emerging new tools to study macrophage activation in bacterial infectious diseases.

Macrophage polarization is a concept that has been useful to describe the different features of macrophage activation related to specific functions. Macrophage polarization is responsible for a dichotomic approach (killing vs. repair) of the host response to bacteria; M1-type conditions are protective, whereas M2-type conditions are associated with bacterial persistence. The use of the polarization concept to classify the features of macrophage activation in infected patients using transcriptional and/or molecular data and to provide biomarkers for diagnosis and prognosis has most often been u

CD3+ T-cell: CD14+monocyte complexes are dynamic and increased with HIV and glucose intolerance.

An increased risk of cardiometabolic disease accompanies persistent systemic inflammation. Yet, the innate and adaptive immune system features in persons who develop these conditions remain poorly defined. Doublets, or cell-cell complexes, are routinely eliminated from flow cytometric and other immune phenotyping analyses, which limits our understanding of their relationship to disease states. Using well-characterized clinical cohorts, including participants with controlled HIV as a model for chronic inflammation and increased immune cell interactions, we show that circulating CD14+ monocytes

Localised immune complexes and slipped upper femoral epiphysis.

Slipped upper femoral epiphysis remains a disease of unknown aetiology. Recent evidence has bolstered speculation that the immune system may play a role in the aetiology or pathogenesis of slipped epiphysis or of one of its complications, chondrolysis. This study reports the finding of immune complexes in the synovial fluid of all but one hip affected with slipped epiphysis in a consecutive series. In seven patients, immune complexes were detected by both the Raji cell assay and C1q-binding assay; in two, by the C1q-assay only; and in one, by the Raji cell assay only. No patients had immune co

Visceral leishmaniasis: host-parasite interactions and clinical presentation in the immunocompetent and in the immunocompromised host.

Visceral leishmaniases are vector-borne parasitic diseases caused by protozoa belonging to the genus Leishmania. The heterogeneity of clinical manifestations and epidemiological characteristics of the disease reflect the complex interplay between the infecting Leishmania species and the genetic and immunologic characteristics of the infected host. The clinical presentation of visceral leishmaniasis depends strictly on the immunocompetency of the host and ranges from asymptomatic to severe forms. Conditions of depression of the immune system, such as HIV infection or immunosuppressive treatment

[Mixed cryoglobulinemia].

Cryoglobulins are immunoglobulins that persist in serum, precipitate with cold temperature, and resolve when rewarmed. Cryoglobulinemia are part of systemic vasculitis, and they are in the subgroup of immune complex type vasculitis. Mixed cryoglobulinemia are associated with malignant B lymphoproliferative disorders, auto-immune and chronic infectious diseases, particularly hepatitis C virus infection. Mixed cryoglobulinaemia has a variable prognosis, depending mainly on renal involvement, systemic extension, and severity of hypertension. In pilot studies, alpha interferon seemed promising to

A study of possible ill effects from prolonged immunotherapy in treatment of allergic diseases.

It has been claimed that immunotherapy in allergic diseases may result in morbidity from chronic antigenic stimulation by the formation of immune complexes and by contributing to development of arteritis, amyloidosis and paraproteinaemia. We examined 35 older patients who received injections of allergen extracts for between two and 30 years and compared them with an age-matched control group. Treated patients had significantly higher serum IgG and lower total IgE than controls but no increased incidence of paraproteins or evidence of immune complex disease such as urinary abnormalities, increa

Detection of circulating immune complexes with a modified Raji cell technique.

A modification of the Raji cell technique for the detection of circulating immune complexes is described. Raji lymphoid cells bind immune complexes via the C 3 receptor. Prior to incubation with the test serum it is necessary to block the IgG Fc receptor which would interfere with the assay. In the present modification the IgG Fc receptor is blocked with monoclonal IgG myeloma proteins thereby facilitating the detection of C 3 bound immune complexes with fluoresecein-conjugated anti-light chain serum. This modification is presently used in a survey of immune complexes in a number of autoimmune

HIV-related thrombocytopenia.

About 5-10% of HIV seropositive individuals, in all risk groups, develop a syndrome of immunological thrombocytopenic purpura (ITP). Despite the clear association between HIV infection and thrombocytopenia, the exact immune mechanism leading to the peripheral platelet destruction remains unclear. Whereas some data support the direct effect of autoantibodies to platelet constituents, other findings argue in favour of the deposition of immune complexes containing anti-HIV antibodies. Although viral components could not be detected in the immune complexes or on the platelet membrane, megakaryocyt

[Secondary nephrotic syndrome due to collagen disease and vasculitis].

The most frequent and representative nephrotic syndrome associated with collagen disease is encountered in patients suffering from lupus nephritis. Lupus nephritis is a glomerulonephritis, which discloses various localizations of immune complexes in the endothelium, mesangium and subepithelium. In addition, vasculitides complicated by nephrotic syndrome also show the deposition of immune complexes in their glomeruli, such as Henoch-Schönlein nephritis and cryoglobulinemic nephritis. The pathogenetic mechanisms of these nephrotic syndromes are explained as follows. The depositions of immune com
