Incisional hernia after major pancreatic resection: long term risk assessment from two distinct sources - A large multi-institutional network and a single high-volume center.

BACKGROUND: Post-operative incisional hernia (IH) is a common complication following abdominal surgery. Data regarding IH after major pancre … rates for PD and DP (OR = 1.87, and OR = 1.86, respectively, P < 0.0001 each), as did postoperative intraabdominal infections (P < 0.0001). Patient-based survey of 104 patients, revealed that 16 patients (15%) reported post-operative IH during the follow-up period. BMI≥30, SSI and intra-abdominal abscesses were associated with increased IH risk (P < 0.05). CONCLUSION: Imp …

Simultaneous pancreas-kidney transplantation. Experience of the Doce de Octubre Hospital.

INTRODUCTION: Simultaneous pancreas-kidney transplantation (SPKT) constitutes the therapy of choice for diabetes type1 or type2 associated with end-stage renal disease, because is the only proven method to restore normo-glicemic control in the diabetic patient. METHODS: Retrospective and descriptive study of a series of 175 patients who underwent SPKT from March 1995 to April 2016. We analyze donor and recipient characteristics, perioperative variables and immunosuppression, post-transplant morbi-mortality, patient and graft survival, and risk factors related with patient and graft survival.

Three cases report of gastrointestinal leaks after gastric suturing treated with self-made water-drip negative-pressure dual-lumen tube combined with enteral nutrition.

RATIONALE: Postoperative gastrointestinal leaks complicated by severe intra-abdominal infection remain a significant clinical challenge. This report describes a conservative management approach using a self-made, water-drip, negative-pressure, double-lumen tube combined with enteral nutrition in three cases, highlighting its potential as a technical option when immediate surgical closure is not feasible. PATIENT CONCERNS: Three patients developed gastrointestinal leaks with severe intra-abdominal infection following gastric suturing. They presented with signs of peritonitis, sepsis, and persi

Enhanced Outcomes in Bacteremia Through Rapid Phenotype Identification of Carbapenemase-Producing Organisms.

OBJECTIVE: The treatment of carbapenemase-producing organisms (CPOs) causing bacteremia in patients with intra-abdominal infections remains challenging. The early detection of CPOs from blood specimens is critical for guaranteeing the timely selection of appropriate antibiotics and achieving favorable outcomes. Our aim was to evaluate whether the rapid identification of CPOs can improve clinical outcomes. PATIENTS AND METHODS: Between January 2019 and January 2024, we conducted a retrospective study of all bacteremia patients admitted to the Intestinal Fistulae and Intra-abdominal Infection C

The impact of age and comorbidity on the postoperative outcomes after emergency surgical management of complicated intra-abdominal infections.

Age-adjusted Charlson Comorbidity Index (a-CCI) score has been used to weight comorbid conditions in predicting adverse outcomes. A retrospective cohort study on adult patients diagnosed with complicated intra-abdominal infections (cIAI) requiring emergency surgery was conducted in order to elucidate the role of age and comorbidity in this scenario. Two main outcomes were evaluated: 90-day severe postoperative complications (grade ≥ 3 of Dindo-Clavien Classification), and 90-day all-cause mortality. 358 patients were analyzed. a-CCI score for each patient was calculated and then divided in two

Can the Cecal Ligation and Puncture Model Be Repurposed To Better Inform Therapy in Human Sepsis?

A recent report by the National Institutes of Health on sepsis research has implied there is a trend to move away from mouse models of sepsis. The most commonly used animal model to study the pathogenesis of human sepsis is cecal ligation and puncture (CLP) in mice. The model has been the mainstay of sepsis research for decades and continues to be considered the gold standard to inform novel pathways of sepsis physiology and its therapeutic direction. As there have been many criticisms of the model, particularly regarding its relevance to human disease, how this model might be repurposed to be

The 1-2-3 approach to abdominal packing.

BACKGROUND: Abdominal packing (AP) in damage-control laparotomy (DCL) is a lifesaving technique that controls coagulopathic hemorrhage in severely injured trauma patients. However, the impact of the duration of AP on the incidence of re-bleeding and on intra-abdominal infections in penetrating abdominal trauma is not clear. The objective of the present study was to evaluate the complications related to the duration of AP and to determine the optimal time for AP removal. METHODS: Prospectively collected/retrospectively analyzed data at an urban level I trauma center from January 2003 to Decemb

Urachal Cyst Causing Small Bowel Obstruction in an Adult with a Virgin Abdomen.

Introduction. A patent urachus is a rare congenital or acquired pathology, which can lead to complications later in life. We describe a case of urachal cystitis as the etiology of small bowel obstruction in an adult without prior intra-abdominal surgery. Case Report. A 64-year-old male presented to the acute care surgery team with a 5-day history of right lower quadrant abdominal pain, distention, nausea, and vomiting. He had a two-month history of urinary retention and his past medical history was significant for benign prostate hyperplasia. On exam, he had evidence of small bowel obstruction

[Serum iron level in the patients with inflammatory abdominal diseases].

In our study we investigated serum iron levels in the patients with localized or generalized peritonitis. These values were compared in group of 52 patients with acute peritonitis against group of 39 patients without inflammation within peritoneal cavity. The serum iron levels and Total Iron Binding Capacity (TIBC) was indicated in both group. Acute intraabdominal inflammation induced in all patients a state of hypoferremia. However, medium decline of iron level in patients with acute appendicitis or cholecystitis was smaller compare with that observed among the patients with generalized perit

Laparoscopic management of surgical complications after a recent laparotomy.

BACKGROUND: The use of laparoscopy in the scarred abdomen is now well established. However, recent laparotomy and the presence of a fresh abdominal wound usually preclude laparoscopic intervention. Thus, early postlaparotomy complications, which mandate surgical interventions, are usually treated by a second laparotomy. We report our experience with the use of laparoscopy for the treatment of postoperative complications, after open abdominal procedures. METHODS: Fourteen patients were operated for a variety of conditions, and postoperative complications, such as bowel obstruction, intraabdomi

[Enterovesical fistulas in Crohn disease: diagnosis and treatment].

Internal fistula is a complication of Crohn's disease. Among 589 patients operated upon at Hôpital Saint-Antoine between 1970 and 1992, 17 (2.9%) had entero-vesical fistula. Ileovesical fistulas were twice as frequent as sigmoidovesical fistulas. Typical symptoms were pneumaturia, fecaluria, and recurrent urinary tract infection. Cystoscopy was performed in 8 patients and determined the site of the fistula in each case. The indication for operation was the fistula itself (35%) or another complication of Crohn's disease (65%). Associated lesions were as follows: 6 entero-enteral, 2 ileogenital,

Group-C beta-hemolytic streptococcal bacteremia.

Group-C beta-hemolytic streptococci (GCBHS) is an uncommon cause of bacteremia. In a 5-year period, GCBHS accounted for 0.28% of positive blood cultures and 0.35% of bacteremias documented at our hospital. The incidence of GCBHS bacteremia was 0.05 episodes per 1000 admissions. We were able to analyze clinical data of 10 of the 13 patients with GCBHS bacteremias. All but one were adults with significant underlying diseases, and seven episodes were community acquired. The skin was the portal of entry in only one case. Clinical syndromes included primary bacteremia (four cases), pneumonia (two c

Klebsiella pneumoniae infection in solid organ transplant recipients: epidemiology and antibiotic resistance.

BACKGROUND: Klebsiella pneumoniae is a well recognized source of nosocomial infection in solid-organ transplant (SOT) recipients. It is also the most common species capable of producing extended-spectrum β-lactamases (ESBL). Its treatment can therefore be a challenge owing to antibiotic resistance. METHODS: Prospective study of all transplant recipients from July 2003 to December 2007 at our center. Klebsiellla pneumoniae infectious events were recorded. RESULTS: A total of 1,057 patients were enrolled, 509 (48%) renal, 360 (34%) liver, 78 (7%) heart, and 110 (10%) double transplants. We dia

Infectious complications after multivisceral transplantation in adults.

UNLABELLED: It is thought that multivisceral transplantation requires high levels of immunosuppression and therefore, patients run an increased risk of infection. We retrospectively reviewed our center's experience with clinically relevant infectious complications. PATIENTS: Between 2000 and 2005, 10 adult patients underwent multivisceral transplantation. Two immunosuppression protocols were used: between 2000 and 2003, a high immunosupression protocol (six patients; daclizumab induction, tacrolimus trough levels >20 ng/mL and steroids) and an immunomodulatory, low imunosuppression scheme fro
