Helicopter Emergency Medical Service (HEMS) in Northern Ireland: An Analysis of the First 100 Cases.

INTRODUCTION: Northern Ireland (N.I) is the most recent region within the UK to establish a helicopter emergency medical service (HEMS) which became operational in July 2017.1 We present descriptive data and discussion about the first 100 cases managed by this new trauma service. Some call-outs involved multiple cases. The data covers a period of 130 days from late July 2018 to late November 2018. METHODS: Information from all HEMS cases was captured manually and records retained for governance purposes. For the purpose of this paper we conducted a hand trawl of records relating to the first …

The powerless or the empowered? Stakeholders' experiences of diagnosis and treatment for attention-deficit hyperactivity disorder in Ireland.

UNLABELLED: IntroductionIn Ireland, attention-deficit hyperactivity disorder (ADHD) is the most commonly diagnosed condition in Child and Adolescent Mental Health Services (CAMHS). However, little is known about the experiences of stakeholders affected by ADHD in their pathways through care, especially in Ireland. OBJECTIVES: The aim of this stakeholder voice study was to explore stress and coping among parents and Children and Young People (C/YP) affected by ADHD in an Irish context, in order to contribute to knowledge about what works and what needs to change in practice from a service user…

Population attributable fractions of psychopathology and suicidal behaviour associated with childhood adversities in Northern Ireland.

Childhood adversities are strong predictors of psychopathology and suicidality. However, specific adversities are associated with different outcomes, with cross-national variations reported. The current study examined rates of adversities reported in Northern Ireland (NI), and associations between adverse childhood experiences and psychopathology and suicidal behaviour were explored. Data was obtained from the Northern Ireland Study of Health and Stress (NISHS), conducted as part of the World Mental Health (WMH) survey initiative (2004-2008); response rate 68.4% (n = 1,986). The on-line survey…

70,489 primary care encounters: retrospective analysis of morbidity at a primary care centre in Ireland.

BACKGROUND: While considerable changes are happening in primary care in Ireland and considerable potential exists in intelligence derived from practice-based data to inform these changes, relatively few large-scale general morbidity surveys have been published. AIMS: To examine the most common reasons why people attend primary care, specifically 'reasons for encounter' (RFEs) among the general practice population and among specific demographic groups (i.e., young children and older adults). METHODS: We retrospectively examined clinical encounters (which had a diagnostic code) over a 4-year t…

Delayed diagnosis of developmental dysplasia of the hip in Northern Ireland: can we do better?

Developmental dysplasia of the hip (DDH) should be diagnosed as early as possible to optimise treatment. The current United Kingdom recommendations for the selective screening of DDH include a clinical examination at birth and at six weeks. In Northern Ireland babies continue to have an assessment by a health visitor at four months of age. As we continue to see late presentations of DDH, beyond one year of age, we hypothesised that a proportion had missed an opportunity for earlier diagnosis. We expect those who presented to our service with Tonnis grade III or IV hips and decreased abduction …

Impact of TB on the survival of people living with HIV infection in England, Wales and Northern Ireland.

INTRODUCTION: The impact of TB disease on survival in people living with HIV in high resource settings is not well documented in the antiretroviral treatment (ART) era. We calculated TB incidence rates and compared the mortality of persons with and without HIV-TB in a UK HIV cohort in the post-ART era, to determine the impact of HIV-TB on survival in the UK. METHODS: We linked the national cohort of persons (aged ≥15 years) diagnosed with HIV between 2000 and 2008 in England, Wales and Northern Ireland with the national TB register and deaths from the Office of National Statistics. We compare…

Monitoring occurrence and persistence of Listeria monocytogenes in foods and food processing environments in the Republic of Ireland.

Although rates of listeriosis are low in comparison to other foodborne pathogenic illness, listeriosis poses a significant risk to human health as the invasive form can have a mortality rate as high as 30%. Food processors, especially those who produce ready-to-eat (RTE) products, need to be vigilant against Listeria monocytogenes, the causative pathogen of listeriosis, and as such, the occurrence of L. monocytogenes in food and in the food processing environment needs to be carefully monitored. To examine the prevalence and patterns of contamination in food processing facilities in Ireland, 4…

Occurrence, antibiotic resistance and molecular characterization of Listeria monocytogenes in the beef chain in the Republic of Ireland.

This study investigated the occurrence, concentration and key characteristics of Listeria monocytogenes in beef chain samples (n=1100) over a 2-year period (July 2007-June 2009). Listeria monocytogenes was isolated from bovine hides (27%), pre-chill carcasses (14%) and ground beef (29%), but not from ready-to-eat (RTE) beef. The concentration of the pathogen in the majority (95%) of contaminated samples was low and detected by enrichment only. The highest concentrations recovered (100-200 CFU/g) were in ground beef samples. The most commonly isolated serotype group was 1/2a (58%) followed by 4

Mortality predictions for Ireland, 2001-2015: cancers of the breast, ovary, and cervix and Corpus uteri.

Linear and log-linear Poisson regression models of Irish breast, ovarian, and cervical and corpus uterine cancer mortality data for the years 1953-2000 were used to predict European age standardized mortality rates (EASMRs) per 100,000 person years and numbers of deaths for the period 2001-2015. Rates for the whole population and for those under 65 are expected to fall from their current levels for breast and corpus uterine cancers but not for ovarian and cervical uterine cancers. EASMRs for postmenopausal women aged between 55 and 69 years are predicted to fall for breast, ovarian, and cervic

Case-control study for risk factors for Q Fever in southwest England and Northern Ireland.

Q fever (Coxiella burnetti) is thought to account for 1% (700 cases) of community acquired pneumonia in the United Kingdom each year, and can result in serious complications such as endocarditis. Although outbreaks have frequently been reported worldwide, the causes are often not clearly identified and there have been few studies of risk factors in sporadic cases. We conducted a matched case-control study. Cases of acute Q fever in people aged over 15 years in southwest England and Northern Ireland were identified from January 2002 to December 2004. Controls were matched for age, sex and the g…
