Polyparasitism on the Kenya coast. 1. Prevalence, and association between parasitic infections.

A partially-stratified sample survey of people living in the catchment area of Kilifi General Hospital, Coast Province, Kenya, identified 19 species of parasites in blood and intestines. Age-prevalence curves of the common infections were of two distinct types, possibly reflecting the host's ability to mount a partially effective immune response. Although statistically-significant associations between pairs of infections were clearly demonstrable, these were almost wholly explained by environmental heterogeneity and epidemiological coincidence. There was no evidence of positive or negative inf…

The first ten years of heart surgery in Kenya--aspirations, dilemmas and possibilities.

This paper looks at the beginnings of cardiac surgery in Kenya with relevant clinical data as well as the overall historical background to various cardiovascular operations which preceded formal establishment of the Thoracic and Cardiovascular Surgical Unit at the Kenyatta National Hospital. Similar activities in other hospitals in Nairobi and elsewhere in Kenya have been excluded. The Unit separated from general surgical wards in January 1973. By the end of 1983, a total of 208 open heart operations had been done by various teams in the Unit. A number of other heart operations were also under…

Bacteriology of infections in a rural tropical area of Kenya: isolates and antibiotic susceptibility.

No microbial data are available for rural tropical areas. Yet most people in Africa live in rural areas where the burdens of infectious diseases remain the primary cause for morbidity and mortality. A pilot study was done by culturing midstream urine from patient with dysuria (100 with Gram-negative fermentative rods), blood from patients with FUO (55/307 positive) and swabs from patients with infected wounds (29 with Staphylococcus aureus). Bacteria were identified and susceptibility tests were performed according to protocols. Klebsiella was the most common isolate from urine (48%). The over

Clinical trial of halofuginone lactate for the treatment of East Coast fever in Kenya.

A field trial was undertaken to test the efficacy of halofuginone lactate in the treatment of East Coast fever under farming conditions in Kenya. The drug was administered orally at a dose of 1.2 mg/kg bodyweight and treatment was repeated after 48 hours. Of 293 cases treated 236 (80.5 per cent) recovered and 49 (16.7 per cent) died. Five animals were disposed of by the owners and three became chronically infected. No differences were recorded in recovery rate between uncomplicated cases and cases with concurrent anaplasmosis or babesiosis, nor were there any significant differences in recover…

Hepatosplenic schistosomiasis in Kenya: an assessment of the enzyme-linked immunosorbent assay (ELISA).

Sera from 124 adult Kenyan patients with chronic splenomegaly and from 93 geographically matched controls without splenomegaly were tested for evidence of Schistosoma mansoni infection by enzyme-linked immunosorbent assay (ELISA). Ova of S. mansoni were detected on stool or rectal snip examination in 23.4% of all patients, whereas 57.3% had a positive ELISA. All patients with parasitological or histological evidence of schistosomal infection had a positive ELISA, and a negative test reliably excluded schistosomiasis. On the basis of liver histology, 23 patients (18.5%) were considered to have …

Susceptibility of different breeds of goats in Kenya to experimental infection with Trypanosoma congolense.

To assess whether there was any evidence of genetic resistance to African trypanosomiasis, five breeds of goat (East African, Galla, and crossbreds between East African and Galla, Nubian or Toggenburg) were experimentally infected with Trypanosoma congolense either by needle inoculation or by tsetse-transmission. The goats had not been previously exposed to trypanosomiasis. With both methods of infection all breeds were found to be highly susceptible and suffered severe disease. Following tsetse-transmitted infection no significant differences were observed between breeds in the development, d

Taenia saginata (Cestoda) in western Kenya: the reliability of faecal examinations in diagnosis.

The prevalence of Taenia saginata (Cestoda) among 146 predominantly Pokot tribesmen in western Kenya was studied in an attempt to assess the reliability of detecting infections by faecal examinations. firstly, a single faecal sample was microscopically examined after being processed by a standard ether sedimentation technique. Secondly, all subjects were treated with niclosamide and a purgative and the tapeworms were recovered. In this way the efficiency of diagnosing infections by faecal examinations was estimated. T. saginata eggs were found in the faeces of 68% of the men who were eventuall…

Relationships between Ascaris infection and growth of malnourished preschool children in Kenya.

A longitudinal study in Ascaris-infected and noninfected children was conducted in two Kenyan villages. Anthropometric, clinical, and stool exams were performed three times at 14-week intervals. All children received an anthelmintic drug (levamisole) at the second examination. In the 14 weeks before deworming, children with Ascaris (n = 61) did not differ from controls (n = 125) in percentage expected weight gain. In the 14 weeks after deworming, previously infected children showed higher percentage expected weight gain than controls. Before deworming, there was a statistically significant (P …

The economic effects of trypanosomiasis in sheep and goats at a range research station in Kenya.

A study on the economic effects of naturally acquired trypanosomiasis in sheep and goats over a period of 35 weeks under range conditions in the Kiboko area of Kenya indicated that the financial loss from reduced weight gain and death amounted to 36.2 and 62.9 Kenya shillings per head for goats and sheep respectively. This was derived from the weight loss through death together with the reduced weight gain of the survivors compared with treated animals. The study also compared the efficacy of 3 drug regimes in goats and suggests that although quinapyramine (prophylactic) B.vet.C. provided the …

Investigations into yellow fever virus and other arboviruses in the northern regions of Kenya.

Previous studies having shown an appreciable level of yellow fever immunity to exist in northern Kenya, further epidemiological and serological surveys were carried out there in 1968 in an attempt to define more clearly the distribution of yellow fever and to locate possible vector and reservoir hosts of the disease; these surveys also provided information on a number of other arboviruses.Altogether 436 sera from 5 areas in northern Kenya were screened by haemagglutination-inhibition tests with 8 antigens, and 107 of these sera by neutralization tests for Group-B arboviruses. Small numbers of …

Advancing climate and health policies in Kenya: Insights from national policy stakeholders.

INTRODUCTION: Climate change is increasingly recognized as one of the most critical determinants of global public health. The intersection of climate and health requires urgent attention if the impacts of climate change on health are to be mitigated. Kenya has been scaling efforts to address the challenges of the climate-health nexus, but the question of how to integrate climate change and health policy remains unanswered. MATERIALS AND METHODS: This study aimed to answer this question by conducting interviews with 21 high-level stakeholders working on climate change, health, and related poli…

East African human-wildlife conflict: Policy challenges and proposed solutions from Kenya.

Kenya's persistent human-wildlife conflict (HWC) poses a threat both to regional conservation objectives and human livelihoods. This study integrates a historical review of Kenya's land, wildlife, and HWC legislation with stakeholder analysis, a synthesis of leading mitigation techniques by decade, and field interviews to identify critical gaps in the current compensation framework and propose a set of targeted reforms. First, we recommend eligibility should be confined to the most severe incidents such as human fatalities and major injuries, providing compensation only for high-damage species…

Elbow anatomy of fossil cercopithecids from Nakali, Kenya: Functional anatomy and taxonomy.

The locomotor behavior and substrate preferences of early colobines are a matter of debate. Some researchers advocate a partly terrestrial origin with the subsequent parallel evolution of arboreality in different clades during the Plio-Pleistocene, while others suggest that arboreality had already evolved earlier in the Miocene. Microcolobus, from the Late Miocene of Nakali (ca. 10 Ma, Kenya), is one of the earliest known and, for the most part, best-documented African Miocene fossil colobines. A large collection of postcranial remains was recovered at Nakali during the fieldwork of the Joint …
