11 women gave birth to 13 live-born infants after a mean period of 39.6 months following transplantation, whereby one of these women had two successful pregnancies and one patient had a twin pregnancy. The highly premature twins, born after a complicated pregnancy with impaired renal graft function in the mother, died of respiratory insufficiency on the 13th day. The course of 13 pregnancies in 12 women (mean age 28 [19-34] years) after renal transplantation was analysed retrospectively. On Surgical Clinic KBC and Medical faculty at Rijeka in the period from 30.01.1971 to 30.03.1988 403 kidney transplantations were performed. The authors deal with the problem of tuberculosis in patients after transplantation of the kidney. They give an account of eight cases of the disease in 647 patients where during the last 22 years transplantations where performed in the Institute for Clinical and Experimental Medicine. Morbidity of tuberculosis following kidney transplantation is about 1% (14/1719 kidney transplantations). All cases occurred within 1 year after transplantation; surprising was the relative high incidence in patients from outside Central Europe. To determine the feasibility of third kidney transplantation, the experience at the University of Iowa was evaluated. The success of 14 such transplantations was dependent on the outcome of both of the previous graft operations. Ectopic intrathoracic kidney is an extremely rare congenital disorder. It is generally asymptomatic, being discovered as incidental finding on a routine chest radiograph. Diagnosis is established by an intravenous pyelogram demonstrating an abnormally shaped excretory system with longer than usual ureters, which also helps to differentiate it from other posterior mediastinal tumors, such as neurogenic masses, including neuroblastoma, ganglioneuroma, neurofibroma, neuroenteric cysts, meningoceles and hernia of the foramen of Bochdalek.

Twenty-six children with ureteroceles--twenty-three with unilateral double collecting system and three with bilateral double kidney--are analyzed. The clinical and radiological features of these ureteroceles are presented. The different possibilities of surgical management and their results are discussed. A 56-year-old woman received a kidney transplant and presented subsequently with evidence of volume contraction, hyponatremia and hyperkalemia. Urinary sodium excretion was inappropriately high for the degree of volume contraction and urinary potassium excretion inappropriately low for the degree of hyperkalemia. Marked elevation of plasma renin activity and plasma aldosterone suggested that the renal tubules were unresponsive to mineralocorticoids. The defect was shown to be transient. The mechanisms leading to the defect are discussed.

During the period 1964--1978 700 renal transplants were given to 602 patients. Eight patients had a history of tumour before transplantation. After transplantation six cases with a de novo tumour were found. The treatment of the pretransplant tumours was completed on average 3 years before transplantation. The average interval from transplantation to discovery of de novo tumour was 2 years. The pretransplant tumours appeared 5 to 9 years before last control (Jan. 79) (mean 8 years). Two of the transplanted patients with previous tumours died later of their tumours. The others are still alive.
