Coccidioides immitis septic knee arthritis.

A 78-year-old man developed right knee pain and swelling without other systemic symptoms. He had travelled frequently to the Central Valley of California. He was diagnosed with coccidioidomycosis based on joint fluid culture. Coccidioidal complement fixation antibody titres were extremely elevated. Arthroscopic debridement and fluconazole therapy did not lead to satisfactory improvement. Subsequent open debridement and change to itraconazole was followed by resolution of clinical signs of infection.

[Chronic neuralgia after knee surgery].

BACKGROUND: Chronic knee symptoms after surgery around the knee may be neuropathic. These symptoms are often described after intramedullary osteosynthesis of the lower leg, placement of total knee prosthesis and arthroscopy. This neuropathic pain may be caused by partial damage to the infrapatellar nerve. CASE DESCRIPTION: An 18-year-old patient had chronic neuropathic symptoms after intramedullary osteosynthesis of the lower leg. The diagnosis was made through physical examination and injection of lidocaine 1%. After surgical removal of the infrapatellar nerve, the patient became symptom-fre…

[Congenital knee dislocation: case report].

Congenital knee dislocation is an infrequent condition with unknown etiology. In some cases it occurs as an isolated condition, while in others it coexists with associated conditions or syndromes. The treatment of congenital knee dislocation is driven by the severity and flexibility of the deformity. The literature includes from serial casting or the Pavlik harness to quadriceps tendon plasty or femoral osteotomies. We report herein the case of a congenital dislocation treated with serial casting with a good outcome.

Histopathological subgroups in knee osteoarthritis.

OBJECTIVE: Osteoarthritis (OA) is a heterogeneous, multi-tissue disease. We hypothesised that different histopathological features characterise different stages during knee OA progression, and that discrete subgroups can be defined based on validated measures of OA histopathological features. DESIGN: Medial tibial plateaux and synovium were from 343 post-mortem (PM) and 143 OA arthroplasty donations. A 'chondropathy/osteophyte' group (n = 217) was classified as PM cases with osteophytes or macroscopic medial tibiofemoral chondropathy lesions ≥grade 3 to represent pre-surgical (early) OA. 'Non…

Pediatric Knee Osteochondritis Dissecans Lesions.

Osteochondritis dissecans (OCD) can cause knee pain and dysfunction in children. The etiology of OCD remains unclear; theories on causes include inflammation, ischemia, ossification abnormalities, genetic factors, and repetitive microtrauma. Most OCD lesions in skeletally immature patients will heal with nonoperative treatment. The success of nonoperative treatment decreases once patients reach skeletal maturity. The goals of surgical treatment include maintenance of articular cartilage congruity, rigid fixation of unstable fragments, and repair of osteochondral defects with cells or tissues t…

Does arthroscopic knee surgery work?

A recent randomized trial from the Finnish Degenerative Meniscal Lesion Study Group was published in the New England Journal of Medicine and attempted to determine the efficacy of partial meniscectomy without osteoarthritis. Patients were randomized to either arthroscopic partial meniscectomy or sham surgery. The authors concluded that the clinical outcomes after arthroscopic partial meniscectomy were no better than those after the sham surgical procedure. However, there are several important limitations of this trial that make it difficult to generalize to the 700,000 arthroscopic partial men

Osteotomy for treating knee osteoarthritis.

BACKGROUND: Patients with unicompartmental osteoarthritis of the knee can be treated with an osteotomy. The goal of an osteotomy is to unload the diseased compartment of the knee. This is the second update of the original review published in The Cochrane Library, Issue 1, 2005. OBJECTIVES: To assess the benefits and harms of an osteotomy for treating patients with knee osteoarthritis, including the following main outcomes scores: treatment failure, pain and function scores, health-related quality of life, serious adverse events, mortality and reoperation rate. SEARCH METHODS: The Cochrane Ce…

[Imaging strategies for knee injuries].

Injuries of the knees are common. The Ottawa knee rule provides decisional support to determine whether radiographs are indicated or not. With the use of ultrasound it is possible to detect defects of the extensor ligaments and the anterior cruciate ligament. Furthermore, it is possible to detect indirect signs of an intra-articular fracture, e.g. lipohemarthrosis. In complex fractures, e.g. tibial plateau fractures, further diagnostic procedures with multislice computed tomography (CT) are needed for accurate classification and preoperative planning. Multislice CT with CT angiography enables …

MRI-arthroscopy correlation: the knee.

MRI findings can often be helpful in diagnosing and managing common knee injuries. The association between the normal and pathologic changes in the surgical anatomy of the knee commonly found on MRI and those found at arthroscopy can be demonstrated by examining a series of cases that correlate MRI and arthroscopy findings. Regular interactions are recommended between surgeons and musculoskeletal radiologists to improve MRI interpretation and achieve better surgical outcomes for patients.

Ultra-low-velocity knee dislocations.

BACKGROUND: Reports of low-velocity knee dislocations have focused primarily on dislocations occurring during athletic competition. The authors identified a subset of patients with low-velocity knee dislocations that occurred during activities of daily living, such as stepping off a curb, stepping off a stair, or simply falling while walking (ultra-low-velocity dislocations). HYPOTHESIS: Ultra-low-velocity knee dislocations are common in obese individuals and are associated with more complications than high-velocity knee dislocations. STUDY DESIGN: Case series; Level of evidence, 4. METHODS…

Atlas-based knee cartilage assessment.

Magnetic resonance imaging analysis of knee cartilage properties at corresponding anatomic locations could be a valuable tool in studies of knee osteoarthritis by enabling accurate comparisons at practically any region. A technique of this kind is presented in this study. The proposed technique is based on gray-level bone matching using affine transformations and free-form deformations thus eliminating the need of bone segmentations and landmark matching. Sixteen subjects of the osteoarthritis initiative with knee osteoarthritis (10 from baseline; 6 from 24-month follow-up) were included in th…

Swelling around a child's knee.

Swellings around the paediatric knee have a large differential diagnosis, although the majority can be diagnosed clinically. Some swellings merit further investigation by Magnetic Resonance Imaging (MRI).

The Stanmore knee arthrodesis prosthesis.

Knee arthrodesis is most commonly performed for failed total knee arthroplasty. Conventional arthrodesis techniques are associated with a high incidence of complications and are unsuitable in cases with extensive bone loss. We report our medium-term results using a custom-made cemented knee arthrodesis prosthesis in 10 patients with a mean follow-up of 56.4 months (range, 15-199 months). The prosthesis was implanted as a 1- or 2-stage procedure for infected revision knee arthroplasty or tumor endoprosthesis in 9 patients and as a primary procedure in 1 patient with angiosarcoma involving the k…

Future directions in knee replacement.

The use of artificial joints for the treatment of osteoarthritis is expected to expand considerably over the next decade. While newer technologies can offer yet further improvements in total knee systems, implementation will be strongly affected by the need to satisfy apparently competing requirements. Patients expect quicker rehabilitation, improved performance, and lifelong durability; on the other hand, economic constraints require a reduction in cost for each procedure, as well as early intervention and preventative measures, while there is increased pressure from health care systems to us…

Update management below knee intervention.

The application of percutaneous techniques for the treatment of peripheral arterial occlusive disease (PAOD) has gained widespread interest … suitable distal targets for revascularization. Therefore, CLI patients due to blockage of below-the-knee (BTK) arteries are in benefit of the endovascular approach: it offers the advantages of local anesthesia, potentially reduced costs (even anticipating the need for reintervention in many patients), shorter hospital stays The current article provides …
