PURPOSE: Derotational distal femoral osteotomy (DDFO) has good clinical outcomes for the treatment of the recurrent patellar dislocation combined with increased femoral anteversion angle (FAA). Currently, there is no uniform surgical technique. The purpose of this study was to evaluate the safety and efficacy of computer-aided design (CAD) combined 3D-printed osteotomy guide-assisted DDFO for treating these patients.
METHODS: In a retrospective study of 36 patients with recurrent patellar dislocation (RPD) from December 2017 to December 2020, all patients had increased FAA and underwent DDFO

We present the clinical and radiographic outcomes of the cementless low-contact-stress (LCS) rotating-platform total knee arthroplasty. Overall, 423 prostheses were implanted in 393 consecutive patients (30 patients had bilateral total knee replacement) for primary varus gonarthrosis (381 patients) and rheumatoid arthritis (12 patients). There were 81 men and 312 women with a mean age of 73 years (range, 58-85 years). Patella replacement was not performed in any case. Clinical and radiographic evaluation was performed using the Knee Society Score (KSS) and the Knee Society Assessment Form, res

Total dislocation of the talonavicular joint is rare. Detection of a major tarsal bone extrusion is simple. However, its management and treatment are not. We present a case of a total extrusion of the talus and navicular of an open dislocation and the reconstruction options that subsequently followed. In spite of a well-perfused foot, our patient decided to undergo a below the knee amputation after an infection.

The goal of this study was to evaluate the Elmslie-Trillat procedure for recurrent patellar dislocation, patellofemoral pain (with extensor mechanism malalignment), or a combination of both. Thirty-two patients underwent the modified Elmslie-Trillat procedure, consisting of a lateral retinacular release and medialization of the tibial tuberosity for recurrent patellar dislocation, patellofemoral pain, or both. Twenty-nine of 32 patients were available for follow-up. All patients were evaluated clinically and radiologically. Subjective scores were evaluated using the Cox grading system and obje

OBJECTIVE: Patellar dislocations are a common occurrence in orthopedic practice, often accompanied by osteochondral fractures of the retropatellar cartilage surface, known as flake fractures, in up to 58% of cases. The parachute technique represents a simple and cost-effective surgical option aimed at restoring osteochondral integration and preserving native cartilage.
INDICATIONS: Flake fracture of the patella with osteochondral fragments.
CONTRAINDICATIONS: Patella fracture.
SURGICAL TECHNIQUE: By utilizing transpatellar, absorbable sutures, a stable osteochondral interface is achieved wi

BACKGROUND: The association between patella alta and episodic patellar dislocation (EPD) has been well described, but its pathophysiology is not completely clear. Patella alta causes decreased contact between the patella and trochlea and decreased resistance to lateral translation of the patella. Additionally, increased patellar tendon length may allow pathologically increased coronal plane patellar motion. It may thus be desirable to address the length of the patellar tendon itself rather than just its insertion site.
HYPOTHESIS: Tenodesis of the patellar tendon in association with tibial tu

RATIONALE: Distal femoral derotational osteotomy (DFDO) is widely performed to correct excessive femoral anteversion in patients with patellofemoral instability. However, unintended changes in coronal alignment can occur depending on the orientation of the cutting plane. We present a case of unintentional femoral valgization after DFDO in a patient with a recurrent patellar dislocation.
PATIENT CONCERNS: A 23-year-old female presented with multiple episodes of recurrent patellar dislocation of her right knee.
DIAGNOSES: Physical examination revealed a positive apprehension test, positive J-s

Blau syndrome (BS) is a rare autosomal dominant familial granulomatous inflammatory disease presenting in early childhood with dermatitis, arthritis and uveitis. Early-onset sarcoidosis represents the sporadic form, and both are characterised by mutations in the CARD15/NOD2 gene on chromosome 16. We describe a 38-year-old man with known BS who presented for orthopaedic review following right-sided patellar dislocation. MRI of the injured knee demonstrated diffuse synovitis and prominent fatty tissue resembling lipoma arborescens with evidence of recent patellar dislocation. Synovectomy was per

INTRODUCTION: Total subtalar dislocation without fracture of the ankle is a rare clinical entity; it is usually due to a traumatic high-energy mechanism. Standard treatment is successful closed reduction under general anesthesia followed by non-weight bearing and ankle immobilization with a below-knee cast for 6 weeks.
CASE PRESENTATION: We present the case of a 30-year-old Moroccan woman who was involved in a road traffic accident. She subsequently received a radiological assessment that objectified a total subtalar dislocation without fracture of her ankle. She was immediately admitted to t

Bilateral congenital hyperextension of the knees occurring in dizygotic twins is reported. Twin A had the mild form of the disease, genu recurvatum, while twin B had a more severe form of the disease, congenital subluxation. Hyperextension of the knee may occur as a sporadic abnormality, in conjunction with multiple dislocations or as a feature of a syndrome. Early detection and diagnosis are important, especially in the more severe forms of the condition, subluxation and dislocation, which require aggressive immediate intervention to prevent long-term sequelae. Conservative treatment consisti

BACKGROUND: Isolated patellofemoral joint arthritis with patellar malalignment in young patients presents a unique challenge, as these patients typically wish to avoid early total knee arthroplasty. The purpose of this retrospective case series is to describe a standardized dual-surgeon technique for combined patellofemoral arthroplasty (PFA) with patellar realignment using medial patellofemoral ligament reconstruction or tibial tubercle osteotomy and to evaluate early clinical and patient-reported outcomes.
METHODS: A retrospective review was conducted on patients who underwent combined PFA

BACKGROUND: Congenital dislocation of the knee (CDK) is a very rare condition that comprises a spectrum of deformities from subluxation to complete dislocation. The incidence of CDK is estimated at 1 per 100,000 live births, which is 1% of the incidence of developmental dysplasia of the hip (DDH). Moreover, 40-100% of patients with CDK have additional musculoskeletal anomalies, the most common being DDH and clubfoot. In general, the diagnosis is established immediately after birth according to the position of the knee recurvatum. Treatment with conservative methods at an early stage is most li
