Denervation impacts muscle quality and knee bone mineral density after spinal cord injury.

STUDY DESIGN: Cross-sectional study. OBJECTIVES: To compare muscle size, body composition, bone mineral density (BMD), and metabolic profiles in denervated versus innervated individuals with spinal cord injury (SCI). SETTING: Hunter Holmes McGuire Veterans Affairs (VA) Medical Center. METHODS: Body composition, bone mineral density (BMD), muscle size, and metabolic parameters were collected in 16 persons with chronic SCI (n = 8 denervated, n = 8 innervated) using dual-energy x-ray absorptiometry (DXA), magnetic resonance imaging (MRI), and fasting blood samples. BMR was measured by indirect

Exosome-Laden Scaffolds for Treatment of Post-Traumatic Cartilage Injury and Osteoarthritis of the Knee: A Systematic Review.

Mesenchymal stem cell (MSC)-based exosomes have garnered attention as a viable therapeutic for post-traumatic cartilage injury and osteoarthritis of the knee; however, efforts for application have been limited due to issues with variable dosing and rapid clearance in vivo. Scaffolds laden with MSC-based exosomes have recently been investigated as a solution to these issues. Here, we review in vivo studies and highlight key strengths and potential clinical uses of exosome-scaffold therapeutics for treatment of post-traumatic cartilage injury and osteoarthritis. In vivo animal studies were gathe

Detection of knee wobbling as a screen to identify athletes who may be at high risk for ACL injury.

This study developed a method to detect knee wobbling (KW) at low knee flexion. KW consists of quick uncontrollable medio-lateral knee movements without knee flexion, which may indicate a risk of ACL injury. Ten female athletes were recorded while performing slow, single-leg squats. Using motion capture data, the ratio of the frontal angular velocity to sagittal angular velocity (F/S) was calculated. An 'F/S spike' was defined when the F/S ratio exceeded 100%. The number of F/S spikes was counted before and after low-pass filtering at different cut-off frequencies. Intraclass correlation coeff

Eccentric knee flexor strength of professional football players with and without hamstring injury in the prior season.

Both injury history and eccentric knee flexor strength have been associated with risk of football players sustaining hamstring strain injury (HSI). However, it remains unclear whether football players who sustained HSIs in the prior season present persistent eccentric strength deficits. Therefore, the aim of the present study was to verify the eccentric knee flexor strength of professional male football players with and without history of HSI in the prior season. This case-control study assessed 210 professional male football players from 10 Brazilian clubs: 182 included in the control group a

Avulsion of the femoral attachment of the medial collateral ligament in the setting of knee multiligament injury: A case report.

RATIONALE: Medial collateral ligament (MCL) injury is a common sports injury. The damage mainly occurs in ligament fibers, but MCL avulsion fracture is extremely rare and only a few reports have been published. PATIENT CONCERNS: Herein, we present a healthy 21-year-old man with an avulsion fracture of the MCL of the right knee sustained during snowboarding. DIAGNOSIS: Clinical and radiographic findings confirmed the presence of an avulsion fracture at the proximal attachment of the MCL, combined with complete anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) rupture. Variations in Knee Kinematics After ACL Injury and After Reconstruction Are Correlated With Bone Shape Differences.

BACKGROUND: The factors that contribute to the abnormal knee kinematics after anterior cruciate ligament (ACL) injury and ACL reconstruction remain unclear. Bone shape has been implicated in the development of hip and knee osteoarthritis, although there is little knowledge about the effects of bone shape on knee kinematics after ACL injury and after ACL reconstruction. QUESTIONS/QUESTIONS: (1) What is the relationship between bony morphology with alterations in knee kinematics after ACL injury? (2) Are baseline bone shape features related to abnormal knee kinematics at 12 months after ACL reco

Limb Symmetry Indexes Can Overestimate Knee Function After Anterior Cruciate Ligament Injury.

Study Design Prospective cohort. Background The high risk of second anterior cruciate ligament (ACL) injuries after return to sport highlights the importance of return-to-sport decision making. Objective return-to-sport criteria frequently use limb symmetry indexes (LSIs) to quantify quadriceps strength and hop scores. Whether using the uninvolved limb in LSIs is optimal is unknown. Objectives To evaluate the uninvolved limb as a reference standard for LSIs utilized in return-to-sport testing and its relationship with second ACL injury rates. Methods Seventy athletes completed quadriceps stren

Three-step sequential management for knee arthroplasty after severe ballistic injury: Two cases.

Management of knee bone loss after gunshot trauma requires a multidisciplinary approach. Two cases of knee arthroplasty after devastating ballistic trauma are reported. Treatment comprised several steps: sampling, bone resection, reinforced cement spacer, latent sepsis control, and prosthetic reconstruction. The patients showed no neurovascular disorder and had a functioning extensor mechanism. At follow-up of at least 2 years, results were satisfactory, with return to unaided walking and mean International Knee Society (IKS) score improved from 18 to 59 points. In light of these observations,

Contribution of mechanical unloading to trabecular bone loss following non-invasive knee injury in mice.

Development of osteoarthritis commonly involves degeneration of epiphyseal trabecular bone. In previous studies, we observed 30-44% loss of epiphyseal trabecular bone (BV/TV) from the distal femur within 1 week following non-invasive knee injury in mice. Mechanical unloading (disuse) may contribute to this bone loss; however, it is unclear to what extent the injured limb is unloaded following injury, and whether disuse can fully account for the observed magnitude of bone loss. In this study, we investigated the contribution of mechanical unloading to trabecular bone changes observed following

Early changes in the knee of healer and non-healer mice following non-invasive mechanical injury.

In this study, we examined early time-dependent changes in articular cartilage and synovium in response to tibial compression and sought the plausible origin of cells that respond to compression in the healer (LGXSM-6) and non-healer (LGXSM-33) recombinant inbred mouse strains. The right knee of 13-week old male mice was subjected to tibial compression using 9N axial loading. The contralateral left knee served as a control. Knees were harvested at 5, 9, and 14 days post-injury. Histological changes in cartilage and synovium, immunofluorescence pattern of CD44, aggrecan, type-II collagen, carti

Fatigue injury risk in anterior cruciate ligament of target side knee during golf swing.

A golf-related ACL injury can be linked with excessive golf play or practice because such over-use by repetitive golf swing motions can increase damage accumulation to the ACL bundles. In this study, joint angular rotations, forces, and moments, as well as the forces and strains on the ACL of the target-side knee joint, were investigated for ten professional golfers using the multi-body lower extremity model. The fatigue life of the ACL was also predicted by assuming the estimated ACL force as a cyclic load. The ACL force and strain reached their maximum values within a short time just after b

Knee morphometric risk factors for acute anterior cruciate ligament injury in skeletally immature patients.

STUDY DESIGN: Retrospective, case-control. PURPOSE: Knee morphometric risk factors for noncontact anterior cruciate ligament (ACL) injury have been a popular topic with skeletally mature patients. Little research has focused on the skeletally immature, with conflicting conclusions. This study performs a comprehensive analysis of identified parameters thought to predispose to ACL injury in a skeletally immature cohort. METHODS: A retrospective review of pediatric patients undergoing knee magnetic resonance imaging (MRI) was performed over a 4-year period. Inclusionary criteria included mid-su

Injury risk to extraosseous knee vasculature during osteotomies: a cadaveric study with CT and dissection analysis.

BACKGROUND: Realignment osteotomies about the knee may be performed as distal femoral or proximal tibial osteotomies; both may be performed either on the medial or lateral sides of the knee, in closing- or opening-wedge fashion. Although rare, injury to neurovascular structures may occur, and the proximity of the vascular structures to the osteotomy saw cuts has been incompletely characterized. QUESTIONS/PURPOSES: We performed a cadaver study to assess the risk of vascular injury in patients undergoing realignment osteotomies by (1) quantifying the distances between osteotomy saw cuts and blo

Stance controlled knee flexion improves stimulation driven walking after spinal cord injury.

BACKGROUND: Functional neuromuscular stimulation (FNS) restores walking function after paralysis from spinal cord injury via electrical activation of muscles in a coordinated fashion. Combining FNS with a controllable orthosis to create a hybrid neuroprosthesis (HNP) has the potential to extend walking distance and time by mechanically locking the knee joint during stance to allow knee extensor muscle to rest with stimulation turned off. Recent efforts have focused on creating advanced HNPs which couple joint motion (e.g., hip and knee or knee and ankle) to improve joint coordination during sw

Morphology of knee extension torque-time curves following anterior cruciate ligament injury and reconstruction.

BACKGROUND: Mechanical instability, neurosensory deficits, and/or modified coactivation strategies following anterior cruciate ligament injury and surgery might compromise the ability of the knee extensors to exert a precise force during isokinetic knee extension with maximal effort. The purpose of this study was to examine the effects of an anterior cruciate ligament deficiency and reconstruction on knee extensor torque-time curve smoothness and to elucidate its association with hamstring (antagonist) activation and physical performance.
