FinePruner: Unbiased Attention-Head-Level Fine-Grained Token Reduction for Efficient Inference of Large Vision-Language Models.

Large Vision-Language Models (LVLMs) suffer from the high computational cost of the attention mechanism caused by the large number of visual tokens. Token reduction has emerged as a promising approach to reduce the complexity by eliminating redundant visual tokens. However, existing token reduction methods struggle to preserve task-relevant tokens and eliminate irrelevant ones. This is due to the attention biases of LVLMs, where tokens with high attention scores are not always the critical ones. Such biases force existing methods into a dilemma: they face either high performance degradation or

Psychological Therapy in the Age of Large Language Models: Framework for Therapist-Delivered and AI-Supported Functions.

Large language models are increasingly used within and alongside therapy. As large language models perform more therapy functions, questions arise about what the future might hold for therapists and what they will do. We argue that the enduring therapist role in the age of AI-assisted care is currently best understood through relational, adaptive, and accountability functions. These functions include therapeutic challenge, use of the therapeutic relationship as a mechanism of change, rupture detection and repair, bearing witness to suffering, calibration of pace and treatment burden, and clini…

Large Language Models for Clinical Note Simplification: A Systematic Review and Experimental Evaluation of Medical Text Readability.

Since the introduction of the Patient Rights Act, patients in Germany have gained legal access to their medical records, including clinical notes. However, these documents are typically written for healthcare professionals and are often difficult for patients to understand due to specialized terminology, abbreviations, and complex sentence structures. Large Language Models (LLMs) offer new opportunities to automatically simplify such texts while preserving medically relevant information. This study investigates the potential of LLMs to improve the readability of German clinical notes by combin…

Potential of large language models for rapid clinical information support: evidence from acute kidney injury knowledge testing.

Large language models (LLMs) are increasingly applied in clinical contexts; however, to our knowledge, their performance in acute kidney injury (AKI) has not been directly compared with physician performance on standardized clinical knowledge assessments. We conducted a cross-sectional study to evaluate the performance of 13 publicly available LLMs and 123 volunteer participants at the 131st Annual Congress of the German Society of Internal Medicine in Wiesbaden, Germany. Both groups completed an identical AKI knowledge assessment consisting of two case vignettes and 15 single-best-answer mult…

Clinical utility of large language models in metastatic prostate cancer: A multicenter expert validation for decision support.

BACKGROUND: Initial systemic treatment planning for metastatic prostate cancer (mPCa) requires rapid synthesis of heterogeneous clinical and biomarker information. Large language models (LLMs) could assist clinicians, but their safety and acceptability in this high-stakes setting remain uncertain. METHODS: We conducted a multicenter retrospective evaluation of 238 consecutive mPCa cases from three tertiary centers (2018-2025). Five contemporary LLMs were tested via publicly available web interfaces under a locked, zero-shot prompting protocol to generate a clinical summary, a first-line syste…

Large language models for late-life depression: a blinded benchmark of clinical safety, geriatric appropriateness, and triage.

BACKGROUND: General-purpose large language models are increasingly used by patients and caregivers to obtain mental health information and guidance about when professional care is required. In late-life depression, broadly accurate information may nevertheless be unsafe when cognitive change, multimorbidity, frailty, polypharmacy, self-neglect, caregiver dependence, or suicide risk is not adequately recognized. This study compared the clinical accuracy, safety, geriatric-specific appropriateness, triage performance, and response consistency of three large language models when answering patient…

Large language models and artificial intelligence for generating, simplifying, and enhancing outpatient clinic letters: A systematic review.

Outpatient clinic letters are a cornerstone of clinical communication but frequently exceed recommended reading levels, limiting patient comprehension and engagement. Large Language Models (LLMs) and Artificial Intelligence (AI) have been proposed as scalable tools for generating and simplifying these documents, but the evidence regarding their clinical accuracy, safety, and effectiveness remains fragmented. We conducted a systematic review of studies evaluating AI or LLMs for generating, simplifying, or enhancing outpatient clinic letters. Five databases (PubMed, EMBASE, Web of Science, CENTR…

Knowledge augmented causal discovery through large language models and knowledge graphs: application in chronic low back pain.

Causal discovery algorithms are often leveraged for inferring causal relationships and recovering a causal model from data. However, causal discovery from data alone is limited by the structural constraints of the used dataset, the lack of causal logic, and the lack of external knowledge. Thus, data-driven causal discovery can only suggest possible causal relationships at best. To overcome these limitations, Large Language Models (LLMs) and knowledge systems, such as Retrieval-Augmented Generation (RAG), have been proposed as alternatives to data-driven causal discovery and as a method to augm…

AI in Hand and Wrist Radiography: Multimodal Large Language Models for Distal Radius Fracture Detection and Characterization.

Background/Objectives: Multimodal large language models (MLLMs) are increasingly evaluated for diagnostic tasks in medical imaging, including radiographic interpretation. However, most studies focus primarily on binary fracture detection and rarely assess clinically relevant fracture characteristics such as displacement or intra-articular extension, which influence treatment decisions. In addition, most evaluations rely on single-run inference designs that do not assess response reproducibility. This study evaluated the diagnostic performance and inter-run reliability of five MLLMs for radiogr…

Zero-shot multimodal large language models underperform a domain-trained CNN baseline in pediatric wrist fracture detection.

Multimodal large language models (LLMs) that process text and images are increasingly discussed for medical imaging, yet their diagnostic performance on radiographs remains poorly characterized. We evaluated three commercially available multimodal LLMs (GPT-4o, Claude 3.5 Sonnet, Gemini 1.5 Pro) in a strict zero-shot setting for pediatric wrist fracture detection and compared them with a domain-trained Inception v3 convolutional neural network (CNN) on the same GRAZPEDWRI-DX dataset. We constructed a balanced patient-level test cohort of 1,000 children (2,298 radiographs; 500 fracture, 500 non…

Benchmarking of large language models to determine candidacy for spine surgery and comparison with conventional machine learning.

BACKGROUND: Symptomatic lumbar spinal stenosis (LSS) is a disabling condition with a substantial economic impact. The determination of surgical candidacy for LSS relies on a subjective assessment of multiple clinical and imaging factors, leading to variability in recommendations. Artificial intelligence (AI), including traditional machine learning (ML) and emerging large language models (LLMs), holds promise for enhancing the accuracy and consistency of surgical decision-making. However, the comparative performance of LLMs against established ML methods for this specific task remains unclear. …

Can Large Language Models Answer Questions about Spinal Cord Injury? Risks, Challenges, and Opportunities-A Narrative Review.

Access to high-quality health information (HI) is critical for everyone involved in the research and management of medical conditions such as spinal cord injury (SCI). Recently, the use of Large Language Models (LLMs) through AI-based chatbots like ChatGPT has become increasingly integral to how people seek and consume HI. While LLMs have been evaluated in various clinical and health domains, there remains a notable gap in the literature regarding their use for SCI-specific questions. We conducted a narrative synthesis to identify the opportunities, challenges, and risks of using LLMs in SCI-r…

Bi-linguistic performance of large language models in multimodal analysis for differentiating jawbone-destroying malignancy from osteomyelitis.

BACKGROUND: Differentiating jawbone-destroying malignancy from osteomyelitis remains a major diagnostic challenge in oral and maxillofacial surgery because these entities share overlapping radiologic features but require fundamentally different management strategies. This study evaluated the bi-linguistic diagnostic performance of advanced multimodal large language models (LLMs) in distinguishing these conditions using stepwise multimodal inputs. METHODS: In this retrospective diagnostic accuracy study, 50 patients with histopathologically confirmed malignancy or osteomyelitis of the maxilla o…
