The planning of therapeutic trials in leukemias presents special problems due to the very variable spontaneous courses. The chronic forms are better accessible to an individual comparison. With the multitude of objective criteria relatively few difficulties arise if long enough periods of therapy are planned and performed. The criteria for the composition of homogeneous collectives as the basis for the much more difficult group comparison are discussed. In the absence of a subdivision into stages the duration of the past history can be used as a substitute criterion. Non-homogeneity in this re…

OBJECTIVES: Recent revisions to acute leukemia classification systems increasingly emphasize genetic drivers of disease; however, critical immunophenotypic features remain essential for accurate diagnosis and risk assessment. CD56, an isoform of the neural cell adhesion molecule, is expressed on natural killer (NK) cells but is aberrantly present across multiple acute leukemias, where it may contribute to diagnostic complexity and adverse clinical outcomes. This review highlights the diagnostic and clinical significance of CD56 expression in acute leukemias.
METHODS: We review current classif…

Leukemias are a class of human cancers that originate from hematopoietic progenitors and are characterized by extensive remodeling of the immune microenvironment. Leukemic cells, on transformation, acquire the ability to evade immune recognition but, despite undergoing genetic and epigenetic changes, retain their characteristic immature immune signature. For this and other reasons, leukemias are often refractory to immune therapies. In the present Review, we cover these areas as a means of improving outcomes from a deeper understanding of immune rewiring, inflammatory signaling and the barrier…

A 9-year-old male castrated golden retriever cross was evaluated for an acute onset of ataxia. A complete blood count revealed marked leukocytosis, anemia, and thrombocytopenia. Leukocytosis was characterized by marked lymphocytosis, monocytosis, neutropenia, and eosinopenia. Flow cytometry identified CD34+ leukocytosis with cells that did not express lineage antigens, suggesting the most likely diagnosis of either acute lymphoid or myeloid leukemia. Leukoreduction via leukapheresis was elected to reduce the disease burden before initiating chemotherapy. Postleukoreduction, the dog had a 54% d…

ETS proto-oncogene 1 (ETS1) is a transcription factor (TF) critically involved in lymphoid cell development and function. ETS1 expression is tightly regulated throughout differentiation and activation in T-cells, natural killer (NK) cells, and B-cells. It has also been described as an oncogene in a range of solid and hematologic cancer types. Among hematologic malignancies, its role has been best studied in T-cell acute lymphoblastic leukemia (T-ALL), adult T-cell leukemia/lymphoma (ATLL), and diffuse large B-cell lymphoma (DLBCL). Aberrant expression of ETS1 in these malignancies is driven pr…

In the year 2021, there were three new Food and Drug Administration approvals for all leukemia types: asciminib (Scemblix) for chronic myeloid leukemia, brexucabtagene autoleucel (Tecartus) for relapsed/refractory B-cell acute lymphocytic leukemia, and asparaginase erwinia chrysanthemi (recombinant)-rywn (Rylaze) for acute lymphocytic leukemia. This is down from 2017-2018 when eight new therapies were approved for acute myeloid leukemia alone. However, this decrease from prior years does not imply that little progress was made in our understanding or treatment of leukemias in 2021. Asciminib a…

Leukemias diagnosed in <1-year-old infants generally have an aggressive clinical nature and unique biological characteristics. Acute lymphoblastic leukemia (ALL) in infants is still intractable and difficult to treat as compared with other pediatric ALLs, for which considerable progress in treatment outcomes has been recently achieved. Infant leukemia cells frequently carry chromosome translocations involving the 11q23 locus, resulting in the rearrangement and fusion of the KMT2A (MLL) gene. Among several KMT2A fusion genes, KMT2A-AFF1 (MLL-AF4) fusion is characteristically observed in neonata…

Leukemia comprises a group of hematological malignancies responsible for 8% of all cancers and is the most common cancer in children. Despite significant improvements in leukemia treatment, the efficacy of conventional chemotherapeutic agents is low and the disease carries a poor prognosis with frequent relapses and high mortality. Curcumin is a yellow polyphenol compound with diverse pharmacological actions including anticancer, antioxidant, antidiabetic, anti-inflammatory, immunomodulatory, hepatoprotective, lipid-regulating, antidepressant, and antiarthritic. Many cellular and experimental …

Pediatric leukemia is the single most common malignancy affecting children, representing up to 30% of all pediatric cancers. Dramatic improvements in survival for acute lymphoblastic leukemia (ALL) have taken place over the past 4 decades with outcomes approaching 90% in the latest studies. However, progress has been slower for myeloid leukemia and certain subgroups like infant ALL, adolescent/young adult ALL, and relapsed ALL. Recent advances include recognition of molecularly defined subgroups, which has ushered in precision medicine approaches. We discuss the current understanding of the bi…
