The safety and efficacy of primary duct closure after laparoscopic common bile duct exploration in patients with mild-to-moderate calculus-associated acute cholangitis: a retrospective cohort study.

While laparoscopic common bile duct exploration with primary duct closure (LCBDE + PDC) has been considered a feasible and safe treatment for cholecystocholedocholithiasis, uncertainties remain regarding its effectiveness and safety in patients with mild-to-moderate calculus-associated acute cholangitis. Therefore, this study aims to investigate the safety and efficacy of LCBDE + PDC specifically in patients with mild-to-moderate acute cholangitis (AC). Patients with cholecystocholedocholithiasis who underwent LCBDE + PDC treatment at our hospital between July 2020 and September 2022 were incl

How Endoscopic Guidance Augments Nonvascular Image-guided Interventions.

Endoscopy can improve guidance in nonvascular procedures performed by interventional radiologists (IRs). Historically, the major limiting factors preventing the widespread use of endoscopic tools by IRs were the large diameter (>20F) and length of the endoscopes. IRs had to significantly upsize their access into vascular organs such as the kidney and liver to allow endoscope placement. With the advent of newer endoscopes with sizes smaller than 11F (approximately 4 mm in diameter), percutaneous endoscopy has become more feasible than before. IRs routinely place percutaneous drains (eg, abscess

Placement of ureteral stent after uncomplicated ureteroscopy: do we really need it?

OBJECTIVE: To investigate the role of stenting after uncomplicated ureteroscopy. METHODS: This was a systematic review and meta-analysis of randomized controlled trials from MEDLINE, the Cochrane Central Search Library, and the EMBASE database. All of the studies reported various outcomes with or without stenting after ureteroscopy. RESULTS: Fourteen trials were identified. As there was significant heterogeneity in pain assessment, difference in postoperative pain between the stent and nonstent groups was not clear. The incidence of dysuria, frequency, and hematuria was statistically signifi

[ROBOT-ASSISTED PYELOLITHOTOMY IN PELVIC KIDNEY].

INTRODUCTION: Pelvic kidney is a congenital anomaly whereby the kidney fails to ascend to its normal anatomical location during early embryonic development. This, in turn, makes traditional surgical approaches for renal calculi challenging. AIMS: To summarize our experience with robotic-assisted pyelolithotomy (RPPK) for pelvic kidney stones. METHODS: A retrospective review of patients who underwent RPPK between 2014 and 2023 was conducted. Demographic data, stone characteristics (size and density), operative details, and postoperative outcomes were collected and analyzed. RESULTS: Four pat

Diagnostic Accuracy and Therapeutic Efficacy of Digital Single-Operator Cholangioscopy for Biliary Lesions and Stenosis.

BACKGROUND/AIMS: Digital single-operator cholangioscopy (dSOC) has revolutionized bile duct visualization. Interventions like electrohydraulic or laser lithotripsy, inspection of suspicious areas, and targeted biopsies have become possible quick and easy. One main indication for dSOC remains the evaluation of indeterminate biliary strictures. OBJECTIVE AND METHODS: We analyzed 180 consecutive dSOCs procedures performed in a high-volume tertiary center to evaluate sensitivity, specificity as well as positive and negative predictive values (PPV and NPV) for indeterminate strictures. Furthermore

Ureteral Obstruction Secondary to an Appendiceal Mucocele: A Case Report and Literature Review.

Background: Ureteral obstruction is rarely caused by extrinsic compression from the appendix. In addition, mucinous neoplasms of the appendix are rare, found incidentally in 0.2%-0.7% of appendectomy specimens. Case Presentation: We present an unusual case of ureteral obstruction caused by a large appendiceal mucocele. An asymptomatic 53-year-old caucasian male patient, known for recurrent nephrolithiasis, was referred for management of bilateral nephrolithiasis. A noncontrast CT scan found an atrophic kidney with an obstructive 1.8 cm right midureteral stone above a 9 × 4.3 cm appendiceal muc

Retrograde intrarenal surgery for the treatment of renal stones in children: factors influencing stone clearance and complications.

BACKGROUND: Retrograde intrarenal surgery (RIRS) is a known option for the treatment of upper tract calculi with an excellent success. However, the reports of RIRS in prepubertal children are limited. In this study, we evaluated the factors which affected the success rate and the complications of RIRS at renal stone treatment in childhood. METHODS: We retrospectively reviewed the records of children under 14 years old who underwent RIRS for renal stone disease between January 2009 and December 2012. Patients' age, gender, body mass index (BMI), stone size, stone location, stone number, intrao

Efficacy and safety of an improved Xiao-Xu-Ming-Tang traditional Chinese medicine hot compress for preventing catheter-related bladder discomfort: a multicenter, randomized, double-blind, controlled trial.

OBJECTIVES: Catheter-related bladder discomfort (CRBD) occurs in 47-95% of patients recovering from general anesthesia, causing discomfort and increasing the risk of postoperative delirium. Xiao-Xu-Ming-Tang is a traditional Chinese medicine (TCM) concoction that has demonstrated efficacy in relieving urinary symptoms. Thus, we propose the use of this improved traditional Chinese medicine concoction in the form of a hot compress as an inexpensive, effective, and simple intervention for preventing postoperative CRBD. METHODS: This was a multicenter, randomized, double-blind, controlled study.

Massive retroperitoneal hematoma as an acute complication of retrograde intrarenal surgery: A case report.

BACKGROUND: Retrograde intrarenal surgery (RIRS) has been proven to be a safe and effective treatment modality in large-scale quantitative studies. However, although its safety profile has been established, it also has a potential risk of life-threatening complications. We here describe our experience with a patient who developed a huge periureteral hematoma after RIRS with holmium laser lithotripsy. CASE SUMMARY: A 73-year-old woman visited our center with a complaint of gross hematuria. An enhanced computed tomography (CT) scan revealed a 1.5-cm left renal pelvis stone with hydronephrosis.

The Application of Retrograde Intrarenal Surgery to Remove a Single Large Kidney Stone During Pregnancy.

PURPOSE: To report the application and outcome of retrograde intrarenal surgery (RIRS) to remove a large kidney stone during pregnancy. PATIENT AND METHODS: A 30-year-old woman presented with an infected kidney stone (3 cm in size) at 4 weeks of pregnancy. We decided to remove the stone due to the possibility of obstruction and infection and chose to carry out this procedure by RIRS. In order to avoid complications associated with anesthetic, the surgery was carried out after the infection had cleared and when the patient had entered the second trimester of pregnancy. First, we used an ureter
