From 'control to elimination': a strategic change to win the end game.

Strategies for elimination evolve from early use of available tools, to elaboration of control strategies, through to 'elimination.' Onchocerciasis control in Africa demonstrates this evolution. Early strategies used vector control but later used mass distribution of ivermectin. Elimination in Africa though was not thought to be possible; however, with excellent coverage of ivermectin distribution it was demonstrated that treatment could be stopped on the Senegal Mali border stimulating a new policy of elimination of transmission where possible. This new policy must not be business as usual bu…

Dracunculiasis eradication: neglected no longer.

This report summarizes the status of the global Dracunculiasis Eradication Program as of early 2008. By the end of 2007, dracunculiasis (Guinea worm disease) transmission had been eliminated from 15 of the 20 countries where the disease was endemic in 1986, only 9,585 cases were reported worldwide, and 2,016 villages still had indigenous cases of the disease. Two of the remaining affected countries (Nigeria and Niger) reported < 100 cases in 2007 and are on the verge of eliminating dracunculiasis if they have not stopped transmission already. Sudan, Ghana, and Mali are addressing their final c…

Hematospermia: a new etiology of clinical interest.

Ten Spanish male tourists developed hematospermia and ultrasonographic evidence of involvement of the prostate and/or seminal vesicles after recreational exposure in bodies of fresh water in the Dogon country of Mali. Schistosoma eggs were detected in the ejaculate of five men, in the others, eggs were observed in the urine or feces. Three different species were observed: S. intercalatum, S. haematobium, and S. mansoni. Hemospermia and clinical prostatitis may be frequently unrecognized clinical manifestations of the early stages of infection in previously nonexposed persons. Travelers to ende…

[Acute encephalitis concurrent with primary infection by Schistosoma mansoni].

A 24-year-old man returning from a trip to Mali was hospitalized for acute encephalitis and fever in association with acute primary infection by Schistosomiasis mansoni. Bilharziasis was suspected from the epidemiological context and presence of eosinophilia. Diagnosis was confirmed by serological testing. Specific treatment using praziquantel and corticotherapy was successful. Central nervous system involvement attributable to embolization of eggs or ectopic migration of adult worms has been reported in association with chronic Schistosomiasis by S. japonicum or S. mansoni. There have been fe…

Giardia Detection and Codetection With Other Enteric Pathogens in Young Children in the Vaccine Impact on Diarrhea in Africa (VIDA) Case-Control Study: 2015-2018.

BACKGROUND: Giardia has been associated with reduced risk of diarrhea in children in low-resource settings, but the mechanism underlying this association is unknown. To assess whether Giardia may shape colonization or infection with other enteric pathogens and impact associations with diarrhea, we examined Giardia and enteric pathogen codetection among children <5 years old in Kenya, The Gambia, and Mali as part of the Vaccine Impact on Diarrhea in Africa study. METHODS: We tested for Giardia and other enteric pathogens using enzyme-linked immunosorbent assays and real-time polymerase chain r…

[Haematological and biochemical outline of patients with sickle cell disease SS and SC in steady state in the National Hospital Center Yalgado Ouedraogo of Ouagadougou].

Burkina Faso belongs to the sicklemic's belt of LEHMAN and is the epicenter of haemoglobin C. This is the reason of this study on the haematological and biochemical parameters of homozygotes SS and double heterozygotes SC in stationary stage. These parameters will be use for comparison during crisis and to evaluate the therapy efficiency. So 20 homozygotes and 20 double heterozygotes were studied. The blood film showed anaemia with an haemoglobin rate average of 7.8 g/dl for SS against 9.8 g/dl for SC (p < 0.001). An serum's iron elevated in 26.3% of the SS with an average of 23 micromoles /l

Haemoglobinopathies C and S in the Dogons.

The distribution of haemoglobins C and S was studied in a population of caste and non caste Dogons living in villages located on the plateau and scree regions in the Sangha department of Mali. Results showed a 15.77% prevalence of haemoglobinopathy AC. Haemoglobin C was found in both plateau and scree villages and equally among caste and non caste Dogons, while the homozygous form CC was absent in non Dogons. The prevalence of haemoglobinopathy AS was extremely low with a calculated frequency of 3.05%, allele S being restricted to areas where one is likely to encounter populations other than t…

Case report of a brain abscess due to Nannizziopsis obscura species complex in a renal transplanted patient.

We here describe the case of a kidney transplant patient from Mali who presented with multiple complex seizures followed by a facial paresis revealing a brain abscess due to Nannizziopsis obscura species complex. This fungal species is usually implicated in reptile skin infections and rarely disseminated infections. Very rarely, it can infect human beings suffering from immunodepression, causing various kinds of complications ranging from soft tissue abscesses to solid organ abscesses. This case highlights the potential wide range of pathogens involved and underlines the need for greater vigil…

Pseudotumoral Bladder Schistosomiasis in a 16-Year-Old Adolescent: A Case Report.

We report the case of a 16-year-old adolescent from Mali who was admitted for acute macroscopic hematuria. CT urography revealed bladder wall calcifications associated with an endoluminal lesion, and cystoscopy identified a 16-mm polyp. Schistosomal serology was equivocal, while urine parasitology demonstrated nonviable eggs compatible with Schistosoma haematobium. A complete transurethral resection was performed, and histopathological examination revealed calcified eggs within the lamina propria without any signs of malignancy. This case highlights the importance of considering schistosomiasi…

A mycovirus-encoded homologue of plant viral movement proteins is not functional in movement complementation assays in plants.

Valsa mali negative strand RNA virus 1 (VmNSRV1), a fungal virus infecting Valsa mali and belonging to the Phenuiviridae family, was recently shown to be capable of naturally infecting plants. Its RNA2 encodes a homologue of a plant virus movement protein (MP) that can complement MP-deficient virus vectors and localizes to plasmodesmata, allowing natural cross-kingdom infection. To determine whether this property is a common feature among mycoviruses possessing distant MP homologues or an isolate-specific trait, we investigated Trichoderma gamsii cogu-like virus 1 (TgClV1), a Phenuiviridae mem…

Evaluation of the therapeutic efficacy of praziquantel against schistosomes in seven countries with ongoing large-scale deworming programs.

The World Health Organization (WHO) recommends periodic assessment of the therapeutic efficacy of praziquantel (PZQ) to detect reduced efficacy that may arise from drug resistance in schistosomes. In this multi-country study (2014), we assessed the therapeutic efficacy of a single oral dose of PZQ (40 mg/kg) against Schistosoma mansoni (Brazil, Cameroon, Ethiopia, Mali, Madagascar and Tanzania), S. haematobium (Cameroon, Ethiopia, Mali, Tanzania and Zanzibar) and S. japonicum (the Philippines) infections in school-aged children, across a total of 12 different trials. Each trial was performed a…

An unusual case of Dyke-Davidoff-Masson syndrome revealed by status epilepticus in a Malian patient.

The Duke-Davidoff-Masson syndrome (DDMS) is a rare neurological condition with unknown prevalence, globally. To date, <100 cases have been reported worldwide. We report the case of an 18-year-old patient admitted for status epilepticus seizure, and who presented a right hemiparesis, body asymmetry, joints ankylosis, and mental retardation. Brain CT-scan revealed left hemisphere atrophy, skull bone thickening, and hyperpneumatization of the frontal sinuses; all consistent with DDMS. Seizures improved remarkably on Levetiracetam and Valproate. This is the first report of an unusual DDMS in Mali,…

The power of markets: Impact of desert locust invasions on child health.

This paper investigates the consequences of the 2004 locust plague in Mali. We argue that in agricultural economies with a single harvest per year, this type of shock can affect households through two channels: first, a speculative/anticipatory effect that kicks in during the growing season, followed by a local crop failure effect after harvest. We document a substantial impact of the plague on crop price inflation before the harvest. Regarding health setbacks, children subject only to the speculative/anticipatory effect suffered as much as those exposed to the actual crop failure effect. The …
