We report two cases of symptomatic Meckel's diverticulum in adults with recurrent abdominal pain and episodes of minor lower gastrointestinal bleeding. In case 1, the diagnosis was suggested by (99m)Tc pertechnetate scan and confirmed by laparoscopy; whereas in case 2, only diagnostic laparoscopy was performed because of suspected appendicitis. A segmental small bowel resection with attached diverticulum was performed extracorporeally after exteriorization through the umbilical port site in both cases.

There are few reports of the sonographic appearance of Meckel's diverticulum. We present a case of torsion of a Meckel's diverticulum that was suggested by sonography and confirmed pathologically. We discuss the sonographic differential diagnosis, which includes acute appendicitis, enteric duplication cyst and intestinal volvulus.

Seventy-two cases of Meckel's diverticulum were treated surgically in recent four years. There were 61 boys and 11 girls with a mean age of 5.2 years. Of the 65 symptomatic patients, 31(43%) had rectal bleeding, 16(23%) suffered from bowel obstruction and 17(24%) peritonitis. Patients with diverticulitis(8 cases) had acute abdominal pain in the periumbilical region or in the right lower quadrant compatible with appendicitis. Ectopic mucosa was found in 47 cases(65.3%). Laparoscopy in the emergency setting is a logical extension of this technique. Open laparoscopy is particularly useful in the management of the acute abdomen. In our institution, after a sequential work-out that includes physical examination, laboratory data, plain abdominal roentgenograms and ultrasonography, diagnostic laparoscopy is advocated. We present the laparoscopic treatment of an intestinal obstruction caused by a volvulus around Meckel's diverticulum. The efficacy and safety of the diagnostic and/or therapeutic laparoscopic procedures in the emergency setting are discussed.

Meckel's diverticulum is a pathology not rarely found everyday clinical medicine especially when it present with one of its complications. The specific diagnosis is a bit difficult to put because of the low sensibility and specificity of symptoms of diagnostic and instrumental techniques used. The reported case shows a rare type of complication constituted by the phlogosis of Meckel's diverticulum, itself due to an extraneous thing ingested by the patient. The authors discuss the entity of the manifestations of this pathology and highlight every aspects.

The authors sum up an experience with treatment of 87 patients with Meckel's diverticulum which was the cause of an acute surgical pathology in 66 patients, chronic disease in 5 cases and 16 patients had no alterations. Special attention is called to differential diagnostic problems in the preoperative diagnosing. Meckel's diverticulum is one of the primary concerns in the differential diagnosis of the pediatric patient with massive, acute gastrointestinal bleeding, intussusception, or abdominal pain of uncertain cause. The hospital course of two children with Meckel's diverticulum, successfully treated by laparoscopic excision, is presented, along with details of the operative procedure.
