Learning to Interpret Pediatric Vocal Fold Mobility: A Laryngeal Ultrasound Training Module.

OBJECTIVES/HYPOTHESIS: Vocal fold movement impairment (VFMI) in infants and children is most commonly evaluated by flexible nasolaryngoscopy (FNL). FNL in this population can be challenging due to movement, floppy supraglottic structures, or secretions. Laryngeal ultrasound (LUS) may be an alternative, less invasive means of evaluating VFMI that also decreases aerosolization during the COVID-19 pandemic. The primary objective was to examine LUS interpretation proficiency for VFMI via an educational module. A secondary outcome was to determine whether quantitative measurements increase interpre

Pediatric appendiceal ultrasound: maintaining accuracy, increasing determinacy and improving clinical outcomes following the introduction of a standardized reporting template.

BACKGROUND: Pediatric patients who underwent appendiceal US and received an equivocal interpretation had poorer clinical outcomes and higher medical costs compared to those to whom a definitive interpretation was given, either positive or negative. In an effort to reduce equivocal interpretations, we educated our group on the importance of increasing determinacy and encouraged the use of a reporting template with a definitive impression. OBJECTIVE: We hypothesized that educational sessions and implementation of an optional reporting template with only a definitive impression would reduce equi

[How to utilize JSML2009 guideline of laboratory examination].

As a resident, I am rotating various departments. I am called for to acquire knowledge, skills, and attitudes for primary care. Although I mainly perform medical examinations and treatments of inpatients, I also have some opportunities to treat outpatients who come to an emergency room. There we see various patients, from whose medical background we know nothing, to who are coming to our hospital regularly for years and have huge medical records. We need to choose suitable examinations and to interpret examination results. In the process, I utilize JSML2009 guideline of laboratory examination.

Chest pain, palpitations and panic.

Chest pain and palpitations are commonly described in the general population and are frequent presenting symptoms in all medical settings. Although often transient, they can be persistent and are associated with considerable disability, distress, and concern about heart disease and use of medical resources. Both symptoms are associated with panic but also with other psychiatric disorders. It is most useful to consider etiology as multicausal and interactive; psychological factors affect interpretation of bodily perceptions and have deleterious effects on quality of life and use of medical reso

Rapid Detection of Significant Traumatic Brain Injury Requiring Emergency Intervention.

Moderate and severe traumatic brain injuries (TBI) are a major cause of severe morbidity and mortality; rapid diagnosis and management allow secondary injury to be minimized. Traumatic brain injury is only one of many potential causes of altered mental status; head computed tomography (HCT) is used to definitively diagnose TBI. Despite its widespread use and obvious importance, interpretation of HCT images is rarely covered by formal didactics during general surgery or even acute care surgery training. The schema illustrated here may be applied in a rapid and reliable fashion to HCT images, ex

Snake venoms: A brief treatise on etymology, origins of terminology, and definitions.

The ancient perceptions of "venomous" and "poisonous snakes", as well as the Indo-European (IE) etymological origins of the term "venom" specifically associated with snakes are considered. Although several ancient cultures perceived snakes as symbols of fecundity and renewal, concurrent beliefs also associated venomous snakes with undesirable human characteristics or as portending non-propitious events. The respective IE roots of the terms "venom" and "poison", "wen" and "poi" refer to desire or the act of ingesting liquids. The origin of the term, "venom", is associated with polytheistic cult

Galen's anecdote of the fallen sophist: on the certainty of science through anatomy.

In this paper the author recounts an anecdote presented by Galen of Pergamum (circa 130-200 A.D.) about a sophist named Pausanias, who fell from his mount and struck his back against a rock. The patient developed a subsequent loss of sensation in the fingers of his left hand with complete sparing of motor function. Numerous medications were applied to his hand but to no avail. Galen stated that he applied the same medications to the original point of dorsal tenderness, resulting in the patient's dramatic and full recovery. Galen attributed the healing to local drug action at the site of a pres

Research on intellectual property rights and legal liability definition of artificial intelligence-assisted pediatric disease diagnosis system.

BACKGROUND: Children's physiological development is immature, their symptoms are hidden and their diagnostic fault tolerance window is low, so they are the key protected population of public health. Although pediatric AI can alleviate the shortage of medical resources, the unclear definition of ownership and responsibility hinders its standardized development. METHODS: This study adopts a mixed-method paradigm, separating exploratory analysis and hypothesis testing modules. Based on 89 hospital AI diagnosis documents, 17 medical dispute cases and relevant laws issued between 2021 and 2025, we

Overreliance on cultural doulas: the paradox of entrusting the communication and care of high-risk migrant women to cultural doulas.

BACKGROUND: It is widely recognized that migrant women from low-income countries are considered to be a group with increased obstetric challenges. To address these challenges, cultural doulas were introduced to provide continuous emotional and practical support during childbirth in Sweden. Leveraging their shared cultural background, language skills, and understanding, the idea behind these doulas was supposed to facilitate effective communication between the woman, her partner, and healthcare staff, with the assumption that this would lead to better maternity care for migrants. The aim of thi

Enhanced CoAtNet based hybrid deep learning architecture for automated tuberculosis detection in human chest X-rays.

Tuberculosis (TB) is a serious infectious disease that remains a global health challenge. While chest X-rays (CXRs) are widely used for TB detection, manual interpretation can be subjective and time-consuming. Automated classification of CXRs into TB and non-TB cases can significantly support healthcare professionals in timely and accurate diagnosis. This paper introduces a hybrid deep learning approach for classifying CXR images. The solution is based on the CoAtNet framework, which combines the strengths of Convolutional Neural Networks (CNNs) and Vision Transformers (ViTs). The model is pre

A Case Series of Familial ARID1B Variants Illustrating Variable Expression and Suggestions to Update the ACMG Criteria.

ARID1B is one of the most frequently mutated genes in intellectual disability (~1%). Most variants are readily classified, since they are de novo and are predicted to lead to loss of function, and therefore classified as pathogenic according to the American College of Medical Genetics and Genomics (ACMG) guidelines for the interpretation of sequence variants. However, familial loss-of-function variants can also occur and can be challenging to interpret. Such variants may be pathogenic with variable expression, causing only a mild phenotype in a parent. Alternatively, since some regions of the

Expert specification of the ACMG/AMP variant interpretation guidelines for genetic hearing loss.

Due to the high genetic heterogeneity of hearing loss (HL), current clinical testing includes sequencing large numbers of genes, which often yields a significant number of novel variants. Therefore, the standardization of variant interpretation is crucial to provide consistent and accurate diagnoses. The Hearing Loss Variant Curation Expert Panel was created within the Clinical Genome Resource to provide expert guidance for standardized genomic interpretation in the context of HL. As one of its major tasks, our Expert Panel has adapted the American College of Medical Genetics and Genomics/Asso

[OCT patterns as a source of errors in the diagnosis of intraocular tumors].

Despite the abundance of publications, the interpretation of imaging techniques such as optical coherence tomography (OCT) remains challenging in the differential diagnosis of intraocular tumors (IOTs), particularly - early choroidal melanomas. This article analyzes OCT findings in 15 patients whose initial diagnosis of "choroidal hemangioma" was revised to "choroidal melanoma" in 11 cases and to "progressive nevus" in 4 cases. The mean tumor prominence was 2.22±0.82 mm, and the mean diameter was 8.01±2.72 mm. The analysis identified OCT patterns frequently responsible for diagnostic errors, s

Ocular bioanalysis: challenges and advancements in recent years for these rare matrices.

There are many ocular diseases still presenting unmet medical needs. Therefore, new ophthalmologic drugs are being developed. Bioanalysis of eye compartments (along with plasma and other tissues) is important to determine exposure of the target organ to the drug and to help interpret local pharmacological or toxic effects. This review article identifies several challenges that occur within ocular bioanalysis. They include sample collection and preparation, analytical issues, sourcing control matrix, data interpretation and regulatory requirements. It summarizes how these challenges have been r

"Imagine if I'm not here, what they're going to do?"-Health-care access and culturally and linguistically diverse women in prison.

BACKGROUND: Women in prison have complex medical needs and poorer health status than the general population. Culturally and linguistically diverse (CALD) women in prison, particularly those with limited English proficiency (LEP), have distinct needs and risk additional isolation, discrimination and marginalization when they are in prison. OBJECTIVE: We sought to examine how cultural and linguistic diversity, particularly LEP, affects the health-care experiences of women in prison. DESIGN, SETTING AND PARTICIPANTS: We conducted focus groups and semi-structured qualitative interviews with CALD

A new computer-aided diagnosis tool based on deep learning methods for automatic detection of retinal disorders from OCT images.

PURPOSE: Early detection of retinal disorders using optical coherence tomography (OCT) images can prevent vision loss. Since manual screening can be time-consuming, tedious, and fallible, we present a reliable computer-aided diagnosis (CAD) software based on deep learning. Also, we made efforts to increase the interpretability of the deep learning methods, overcome their vague and black box nature, and also understand their behavior in the diagnosis. METHODS: We propose a novel method to improve the interpretability of the used deep neural network by embedding the rich semantic information of

Putting back respiration into respiratory sinus arrhythmia or high-frequency heart rate variability: Implications for interpretation, respiratory rhythmicity, and health.

Research on respiratory sinus arrhythmia, or high-frequency heart rate variability (its frequency-domain equivalent), has been popular in psychology and the behavioral sciences for some time. It is typically interpreted as an indicator of cardiac vagal activity. However, as research has shown for decades, the respiratory pattern can influence the amplitude of these noninvasive measures substantially, without necessarily reflecting changes in tonic cardiac vagal activity. Although changes in respiration are systematically associated with experiential and behavioral states, this potential confou

Radiology in family practice: experience in community practice.

This study examined the utilization and interpretation of office x-rays in community practice. Analysis of physician and practice descriptive data obtained by survey from the Minnesota Academy of Family Physicians Research Network, a representative sample of family physicians in Minnesota, leads to these conclusions: (1) Most (87.3%) of the study physicians have on-site x-ray facilities. (2) An extensive variety of x-rays is obtained in the office. (3) Most (54.2%) family physicians interpret their own x-rays and self-select those they want re-read by a radiologist; 43.8% referred all x-rays t

Family medicine residents' skill levels in emergency chest X-ray interpretation.

BACKGROUND: Family medicine physicians may encounter a wide variety of conditions, including acute and urgent cases. Considering the limited access to diagnostic investigations in primary care practice, chest X-ray remains the imaging modality of choice. The current study assessed the competency of family medicine residents in the interpretation of chest X-rays for emergency conditions and to compare it with that of diagnostic radiology residents, general practitioners, and medical interns. METHODS: An online survey was distributed to 600 physicians, including family medicine residents, medic
