Development and Reliability Testing of the Sinonasal Assessment of Reepithelialization, a Novel Wound Healing Scale to Assess Postoperative Healing.

BACKGROUND: Sinus and skull base surgery create areas of demucosalized bone and cartilage in the sinonasal cavity, yet no standardized system exists to describe the progressive healing of these wounds. The goal of this study was to develop the novel Sinonasal Assessment of Reepithelialization (SNARE) and perform parametric testing including inter-rater and intra-rater reliability analyses. METHODS: Using a modified Delphi method, an 11-member expert panel comprising 10 rhinologists and one PhD otolaryngology research scientist developed a four-stage wound healing scale over three iterative ro

An automated end-to-end system for schistosome viability assessment to accelerate anti-schistosomal drug discovery.

Schistosomiasis, a neglected tropical disease affecting millions globally, urgently requires new therapies. Current treatments, like praziquantel (PZQ), face challenges such as drug resistance and ineffectiveness against juvenile parasites. This study develops an automated, high-throughput computational platform to quantify schistosome viability from video data. We introduced an end-to-end system that leverages foundation models in computer vision and fine-tuning to assess schistosome viability from videos. By fine-tuning advanced image segmentation and spatiotemporal feature representation mo

Are head and neck versus abdominal paragangliomas driven by different single nucleotide events?

Paragangliomas (PGLs) are a heterogeneous group of tumours of the nonepithelial neuroendocrine type, with a significant percentage being genetically determined. They can develop from cells of the parasympathetic as well as the sympathetic nervous system. Tumours located in the head and neck usually have a parasympathetic origin, whereas those in the abdomen have a sympathetic origin. The aim of this study was to determine whether the development of PGLs at both locations is associated with specific variants of genes with proven relevance for the formation of these tumours. Thirty-one patients

Evaluation of blue-green-grey drainage systems for urban flooding based on resilience components.

This study proposes a practical evaluation framework to assess the pre-flood defense performance of urban drainage systems. This framework integrates the four components of resilience (rapidity, redundancy, resourcefulness, and robustness) with a Flood Defense Index (FDI), a metric designed to quantify the system's hydraulic capacity utilization. The proposed framework was applied to two urban watersheds in Seoul, South Korea, i.e., the Gunja and Banpocheon watersheds, under three blue-green-grey drainage system scenarios i.e., (1) sewer pipes only (SP), (2) SPs with detention basins (SP-DB),

Concurrent examination of gamma-stimulated variability in heart rate and auditory steady-state response in patients with major depressive, bipolar, and schizophrenia spectrum disorders.

BACKGROUND: Heart rate variability (HRV), a well-established indicator of autonomic nervous system function, has been proposed as a clinical biomarker for psychiatric disorders. However, its potential is limited compared to electroencephalography (EEG) markers, possibly due to the influence of confounding factors such as cardiovascular functions. This suggests a need for autonomic metrics more specific to the central nervous system. This study investigated central autonomic control by applying 40 Hz auditory stimulation, which can transmit information beyond the auditory pathway to deep brain

Enhancing multiple myeloma staging: a novel cell death risk model approach.

The prognostication of survival trajectories in multiple myeloma (MM) patients presents a substantial clinical challenge. Leveraging transcriptomic and clinical profiles from an expansive cohort of 2,088 MM patients, sourced from the Gene Expression Omnibus and The Cancer Genome Atlas repositories, we applied a sophisticated nested lasso regression technique to construct a prognostic model predicated on 28 gene pairings intrinsic to cell death pathways, thereby deriving a quantifiable risk stratification metric. Employing a threshold of 0.15, we dichotomized the MM samples into discrete high-r

Scale dependence of fractal dimension in deterministic and stochastic Lorenz-63 systems.

Many natural systems show emergent phenomena at different scales, leading to scaling regimes with signatures of deterministic chaos at large scales and an apparently random behavior at small scales. These features are usually investigated quantitatively by studying the properties of the underlying attractor, the compact object asymptotically hosting the trajectories of the system with their invariant density in the phase space. This multi-scale nature of natural systems makes it practically impossible to get a clear picture of the attracting set. Indeed, it spans over a wide range of spatial s

Does Time to Imaging and Surgery for Distal Radius Fractures Vary based on Geographic Socioeconomic Disadvantage?

BACKGROUND: While disparities in aspects of distal radius fracture (DRF) management and orthopedics at large have been studied, disparities in time to DRF evaluation and treatment are unknown. We sought to determine if geographic socioeconomic disadvantage is associated with time to imaging in the emergency department (ED) and time to surgery for DRFs. METHODS: We performed a time-to-event analysis of 105 patients undergoing DRF surgery after ED triage within our hospital system between January 1, 2015, and January 1, 2020. Area Deprivation Index (ADI) national percentile was used as the metr

Plus Disease in Retinopathy of Prematurity: Convolutional Neural Network Performance Using a Combined Neural Network and Feature Extraction Approach.

PURPOSE: Retinopathy of prematurity (ROP), a leading cause of childhood blindness, is diagnosed by clinical ophthalmoscopic examinations or reading retinal images. Plus disease, defined as abnormal tortuosity and dilation of the posterior retinal blood vessels, is the most important feature to determine treatment-requiring ROP. We aimed to create a complete, publicly available and feature-extraction-based pipeline, I-ROP ASSIST, that achieves convolutional neural network (CNN)-like performance when diagnosing plus disease from retinal images. METHODS: We developed two datasets containing 100

Promoting public bike-sharing: A lesson from the unsuccessful Pronto system.

In 2014, Seattle implemented its own bike-sharing system, Pronto. However, the system ultimately ceased operation three years later on March 17th, 2017. To learn from this failure, this paper seeks to understand factors that encourage, or discourage, bike-sharing trip generation and attraction at the station level. This paper investigates the effects of land use, roadway design, elevation, bus trips, weather, and temporal factors on three-hour long bike pickups and returns at each docking station. To address temporal autocorrelations and the nonlinear seasonality, the paper implements a genera

An in Silico, Biomarker-Based Method for the Evaluation of Virtual Neuropsychiatric Drug Effects.

The recent explosion in neuroscience research has markedly increased our understanding of the neurobiological correlates of many psychiatric illnesses, but this has unfortunately not translated into more effective pharmacologic treatments for these conditions. At the same time, researchers have increasingly sought out biological markers, or biomarkers, as a way to categorize psychiatric illness, as these are felt to be closer to underlying genetic and neurobiological vulnerabilities. While biomarker-based drug discovery approaches have tended to employ in vivo (e.g., rodent) or in vitro test s

Simulation for assessing statistical methods of biologic terrorism surveillance.

INTRODUCTION: Multiple systems have been developed that use surveillance of health-care encounters to provide early warning of a terrorist attack. Limited practical experience and the absence of adequate theoretical assessments have precluded determining which alarm-generating algorithms should be preferred. In the absence of practical and theoretical results, choosing a particular statistical algorithm can be difficult. One way to evaluate algorithms is through simulation. OBJECTIVES: This report describes conceptual features of an example simulation based on the dispersal of anthrax spores

Assessing the role of plan complexity and target geometry through multi-institutional gel based end-to-end QA in multi-focal single isocenter stereotactic radiosurgery.

BACKGROUND: Single-isocenter stereotactic radiosurgery enables efficient treatment of multiple brain metastases (SI-MBM SRS), but demands high geometric accuracy. Plan complexity metrics are increasingly used as indicators for quality assurance (QA) performance, however their applicability to SRS remains uncertain, particularly in the context of multi-institutional variability. PURPOSE: This study evaluated the relationship between plan complexity, target geometry, and end-to-end dosimetric QA outcomes for SI-MBM SRS across multiple institutions. METHODS: Forty-two SI-MBM SRS plans from diff

Disability risk in pediatric motor vehicle crash occupants.

BACKGROUND: Mortality rates among children in motor vehicle crashes (MVCs) are typically low; however, nonfatal injuries can vary in severity by imposing differing levels of short- and long-term disability. To better discriminate the severity of nonfatal MVC injuries, a pediatric-specific disability risk (DR) metric was created. METHODS: The National Automotive Sampling System 2000 to 2011 was used to define the top 95% most common Abbreviated Injury Scale (AIS) 2+ injuries among pediatric MVC occupants. Functional Independence Measure scores were abstracted from the National Trauma Data Bank

Experimental quantification of balance using whole-body stability regions from postural sway exercises.

BACKGROUND: Comprehensive balance assessment is essential for evaluating balance deficiencies, particularly in individuals with limited mobility and increased fall risk. However, clinical balance assessments often rely on subjective scoring and simplified models that may not capture the full dynamics of human postural control. Additionally, center of pressure (COP)-based metrics and inverted pendulum models offer limited insight into whole-body balance ability. Balance regions (BRs) offer a more holistic approach by quantifying balanced center of mass (COM) states. Previously, BRs were used to

When to Consider Invasive Lymph Node Staging in Non-Small-Cell Lung Cancer? A Novel Scoring System Utilising Metabolic Parameters in 18F-FDG PET/CT.

OBJECTIVE: The maximum standardised uptake value (SUVmax) is a widely utilised metric in positron emission tomography/computed tomography for clinically staging non-small-cell lung cancer (NSCLC), yet the reliability of SUVmax remains controversial. We herein aimed to assess the effectiveness of semi-quantitative parameters, encompassing size, SUVmax, metabolic tumour volume (MTV), total lesion glycolysis (TLG) and heterogeneity factor (HF), in evaluating both primary tumours and lymph nodes (LNs) on positron emission tomography/computed tomography. A novel scoring system was devised to apprai

Impact of hybrid solar and wind distributed generators on transient stability of captive power systems.

This paper investigates the impact of solar and wind distributed generators on the transient stability of a 39-bus industrial power system. The system is analyzed under different configurations, including wind-only, solar-only, and hybrid DG integration. Critical Clearing Time (CCT), active power, and reactive power are used as performance metrics. The results show that DG integration improves system stability compared to the baseline case. Specifically, the CCT increases from 0.018 s (without DG) to 0.0248 s with wind DG, and up to 0.3168 s with solar DG. The hybrid configuration results in a
