The Role of Narrative Medicine and Lean Management in Umbilical Cord Blood Donation: A Story of Success.

Background/Objectives: Umbilical cord blood (UCB) is a valuable source of hematopoietic stem cells used in treating blood and immune disorders. Despite its potential and the availability of public banking systems in Italy, donation rates remain low due to patient misinformation, emotional barriers, and organizational inefficiencies. This study aimed to evaluate the impact of integrating Narrative Medicine (NM) and Lean Management (LM) on UCB donation rates and operational effectiveness at the University Hospital of Alessandria. Methods: This prospective, single-center pre-post study ran from J

The Centre for Evidence-Based Orthopaedics Tool for the Implementation of Evidence-Based Practice.

Introduction Updating practices can be challenging in evidence-based practice when integrating the evidence, clinical expertise, and patient values and preferences. Implementing evidence-based practice requires individual, collective, and organizational behavioral changes. Effective behavior change interventions are necessary to facilitate the implementation of new evidence. We propose a tool to facilitate the implementation of evidence in clinical decision-making by changing healthcare providers' behaviors. Methods The Centre for Evidence-Based Orthopaedics (CEBO) tool was developed to bridge

Reasons for attending support groups and organizational preferences: A replication study using the North American Scleroderma Support Group Survey.

Peer-facilitated support groups are an important source for receiving disease-related information and support for people with systemic sclerosis (or scleroderma). A recent survey explored reasons for attending systemic sclerosis support groups in Europe and Australia and used exploratory factor analysis to group reasons for attendance into three main themes: (1) interpersonal and social support, (2) disease treatment and symptom management strategies, and (3) other aspects of living with systemic sclerosis. The objective of the present study was to replicate this study in a sample of patients

Uptake and delivery of hepatitis C treatment in opiate substitution treatment: perceptions of clients and health professionals.

Uptake of treatment for hepatitis C virus (HCV) infection is very low particularly among people who have injected drugs. Opiate substitution treatment (OST) programs, with a high prevalence of people living with HCV, have been a site of growing interest in the delivery of hepatitis C treatment. There has been no exploration of OST clients' and health professionals' perceptions of the barriers and facilitators to uptake and delivery of HCV treatment in OST clinics from personal and organizational perspectives. This qualitative study involved interviews with 27 OST clients in New South Wales and

Psychosocial risk factors of pain among employees.

The study of psychosocial risk factors of pain among employees has typically focused on Karasek's job-demand-control model. The aim of the study was to examine the own and independent associations of job strain, organizational justice, workplace bullying, and work-home interface with pain. Data were collected through a postal survey to all 40, 45, 50, 55, and 60-year-old employees of the City of Helsinki in 2001 and 2002 (response rate 66%, N=5819, 80% women). Pain was measured with a three category outcome: no pain, acute pain or chronic pain. Adjustment was made for age, education, physical

A group randomized trial of critical incident stress debriefing provided to U.S. peacekeepers.

In a group randomized trial of critical incident stress debriefing (CISD) with platoons of 952 peacekeepers, CISD was compared with a stress management class (SMC) and survey-only (SO) condition. Multilevel growth curve modeling found that CISD did not differentially hasten recovery compared to the other two conditions. For those soldiers reporting the highest degree of exposure to mission stressors, CISD was minimally associated with lower reports of posttraumatic stress and aggression (vs. SMC), higher perceived organizational support (vs. SO), and more alcohol problems than SMC and SO. Creating the model of a clinical practice: the case of emergency medicine.

Medical schools and specialty societies have struggled to define a core content for medical students and practitioners but, to date, have been stymied by both political considerations and the sheer burden of the innumerable decisions that must be made to define the essence of a medical specialty. Six professional organizations representing the field of emergency medicine recently collaborated with the National Board of Medical Examiners to accomplish this objective by developing a Model of the Clinical Practice of Emergency Medicine. This document will provide support for the development of me

Influences on intentions for obstetric practice among family physicians and residents in Canada: an explorative qualitative inquiry.

BACKGROUND: Many family medicine residency graduates indicate a desire to provide obstetric care, but a low proportion of family physicians (FPs) provide obstetric care within their practice. This suggests personal preference alone may not account for the low proportion of FPs who ultimately provide full obstetric care. If decisionmakers plan to augment the number of FPs providing obstetric care, barriers to the provision of such care must first be identified. Within this paper, we explore the perspectives of both family practice residents and early-career FPs on the factors that shaped their

The early intervention safeguarding nurse pilot: an integrated model of working.

Some of the most vulnerable children will need co-ordinated help from health, children's services and other agencies. Co-ordinated and joint working hinges on effective communication at all levels. Evidence shows that direct verbal communication is a more effective way to share concerns and that this should be followed up by written information. Yet barriers persist that prevent this from happening. Integrated health and social care teams are purported to break down professional and communication barriers. This paper reports on the evaluation of a pilot integrated model of health and social ca

Staff Stability Through Service: Promoting a Person-Centered Culture for Work and Care in Long-Term Services and Supports Environments.

Nursing homes have struggled for years to secure and retain staff. Turnover of all staff, from administrators to direct care providers, continues to plague the industry. Although improvements in compensation, benefits, and opportunities for promotion may enhance retention, organizational culture may also play a major role. The current article discusses the SERVICE Model of Leadership©, which can improve the culture and promote staff retention through the establishment of systems and programs that meet the reported needs and desires of all staffing levels in nursing homes and other organization

Medical progress, psychological factors and global care of the patient: lessons from the treatment of childhood leukemia.

The history of treatment of childhood leukemia is a meaningful model of ethical, bioethical and organizational repercussions of medical progress. Specifically, it has provided precious indications and very useful tools to cope with several of the more important problems of modern medicine: the value of controlled randomized studies; the risks of intense medicalization impairing the quality of care; the importance of a valid doctor-patient relationship; the psycho-emotive involvement of the pediatric staff; and last but not least, the need of an unrelenting effort of humanization of the procedu

Resource allocation efficiency in Ethiopian hospitals: technology utilization as mediator and staff competency as moderator.

BACKGROUND: Efficient resource allocation remains a major challenge for hospitals in low-resource healthcare systems, where increasing service demand often exceeds the availability of financial, technological, and human resources. Understanding how organizational capabilities improve resource allocation efficiency is therefore essential. This study examined the effects of resource availability, process efficiency, and patient-centered care on resource allocation efficiency in Ethiopian hospitals, while investigating the mediating role of technology utilization and the moderating role of staff

Reconceptualizing de-escalation in mental health care: A qualitative study.

BACKGROUND: Patient aggression is a pervasive challenge in global mental healthcare. Although de-escalation is a cornerstone of non-coercive care, a significant theory-practice gap persists. This is often attributed to a research focus on fragmented techniques rather than on de-escalation as a continuous, relational process embedded in clinical practice. This gap is particularly pronounced in China's unique socio-cultural context, characterized by high-context communication and pronounced hierarchical power dynamics, which directly shape trust-building and authority navigation during de-escala

Associations of Emotional Intelligence and Ethical Climate With Job Involvement Among Operating Room Personnel: A Cross-Sectional Study.

BACKGROUND: Operating room practice requires personnel to remain actively committed to their professional roles while managing complex clinical tasks, close interdisciplinary coordination, and substantial responsibility for patient safety. Despite the importance of such work-related involvement, its association with personal emotional capabilities and the ethical characteristics of the workplace has received limited attention among operating room personnel. This study evaluated job involvement and examined its relationships with emotional intelligence and perceived ethical climate. METHODS: T

Perspectives and Experiences of Stakeholders on Self-Disclosure of Peers in Mental Health Services.

With the movement towards recovery-oriented mental health (MH) services, individuals with MH lived-experience are increasingly employed as peer providers (peers). Peers are unique in that they bring knowledge from experience and eye-level connection to service users that enhance the quality of services and humanize MH systems' culture. In Israel, hundreds of peers are employed in various roles and settings across the MH system. However, peer integration into MH services faces challenges. One issue involves the use of self-disclosure (SD) in MH services which varies with explicitness across rol

Defensive Medicine Attitudes and Associated Factors Among Physicians Working in Obstetrics and Gynecology Clinics in Istanbul, Türkiye: Cross-Sectional Analytical Study.

BACKGROUND: Defensive medicine (DM) is particularly relevant in obstetrics and gynecology (OB/GYN), where physicians face substantial medicolegal risks and malpractice-related pressures. Defensive practices may involve cost-increasing diagnostic or therapeutic behaviors and avoidance of high-risk patients or procedures. Identifying factors independently associated with DM may help inform organizational and medicolegal interventions. OBJECTIVE: This study aimed to evaluate DM attitudes among physicians working in OB/GYN clinics in Istanbul, Türkiye, and to identify professional, organizational
