Malignant hyperthermia (MH) is an uncommon and potentially fatal pharmacogenetic complication of anesthesia. It is commonly triggered by administration of halothane and succinylcholine. The treatment of choice is to administer dantrolene sodium while simultaneously discontinuing the triggering agent. The continued treatment of the patient will reflect the hypermetabolic state. Anesthesia can be safely administered to MH-susceptible patients. Nontriggering agents are used, and meticulous monitoring of the patient is performed. All anesthesia providers should be familiar with MH.

UNLABELLED: High-frequency, low-amplitude signals detected in the terminal portion of the surface QRS, called late potentials (LP), have been shown to correlate with inducible sustained ventricular tachycardia and to be absent in normal subjects. Thirty patients with severe symptomatic coronary artery disease were studied with two 48-h periods of ambulatory ST Holter monitoring. In common with other groups, there was a much higher incidence of asymptomatic (341 or 66.2 per cent) than symptomatic (174 or 33.8 per cent) ST segment depressions. Blood pressure is an inherently variable phenomenon. In addition to this the level rises in the clinical setting; a response which is extremely variable. Problems therefore arise in the diagnosis and treatment of hypertension. Studies have shown that blood pressure can reliably be measured at home either with a stationary or ambulatory apparatus. A construction outline of lightweight portable equipment for VO2 measurements by the Douglas bag method is given. The equipment consists of three separate units: an inspiration-expiration valve, a switch directing the expiratory flow, and a disconnectable bag with stopcock. The ease of operation assures good reproducibility of the results. Experiences during maximal speed skating are described. Use of the equipment demands only simple operations by the subject, which do not interfere with the exercise.

An assessment was made of the usefulness of continuous dynamic ECG (CDE) in diagnosis of the sick sinus syndrome (SSS). It was found that CDE has the following advantages: ready repeatability; control of patients during such physiological activities as sleep; recording of the possibly fleeting and intermittent pathological episodes that characterise SSS. The conclusion is drawn that CDE is the soundest technique for the investigation of SSS, whereas electrophysiological examination, albeit complementary, could be limited to cases whose interpretation proves more difficult.

Prolonged frequency modulation (FM) radiotelemetry EEG and videotape monitoring (TEEG-VR) have been used together extensively to study patients with epileptic seizures. This technique was used to differentiate epileptic seizures from other episodic behavior in 45 children referred to our epilepsy center in whom the diagnosis could not be made by history, standard EEGs, or observation alone, in 40 children, the behavior in question was recorded during TEEG-VR.
