Combination treatment of dupilumab with bortezomib in a patient with IgG kappa gammopathy of renal significance, uremic pruritus and chronic lichenoid dermatitis.

Chronic pruritus (CP) is one of the most frequent symptoms among dermatological conditions, capable of reducing the quality of life (QoL). CP may be induced by atopic dermatitis or other dermatological and/or non-dermatological conditions. In this article, we report the case of a patient affected by generalized CP, characterised by multiple papulo-nodular and escoriatic lesions, developed after the onset of an immunoglobulin G (IgG) kappa monoclonal gammopathy of renal significance (MGRS), associated with renal insufficiency. Therefore, a combined treatment with dupilumab for CP and bortezomib

[When coitus produce pain: an exploration of female sexuality in northwest Mexico].

OBJECTIVE: To determine the prevalence of dyspareunia among women aged 25-54 and its associated risk factors. MATERIAL AND METHODS: A cross-sectional population-based study was carried out in the city of Hermosillo, Sonora and data from 1,183 sexually active women were analyzed. A multiple logistic regression was computed to analyze the association between dyspareunia and sociodemographic characteristics, medical conditions and sexual violence. RESULTS: The 12-month prevalence of dyspareunia was estimated to be 12.3% (95% CI 10.5, 14.4). After adjustment for working conditions, dyspareunia w

Global ageing: successes, challenges and opportunities.

The world's population is ageing rapidly, with significant increases in the numbers of the oldest old. This places great pressure on societies to adapt to this changing demography. Pertinent issues include provision of education and resource for long-term conditions. The priorities older people hold need to be fully understood and their contributions to society, often diverse and far-reaching, recognised with sincerity. Currently, health systems for older people can often feel reactive, fragmented and disjointed. These systems can harbour inequity and ageism, and leave both patients and health

Stroke and spinal infarct caused by percutaneous coronary intervention.

Acute ischaemic stroke is a known risk of percutaneous coronary intervention (PCI). The incidence of such complications has increased in frequency over the last decade due to higher comorbidity burden and increased complexity of PCI procedures. The overall incidence of post-PCI ischaemic stroke remains low at 0.56%, but some groups of patients have significantly higher risk. Risk factors include atherosclerotic plaques, atrial fibrillation, cardiogenic shock, older age and arterial disease. Although the overall incidence of acute ischaemic stroke following PCI is low, it can result in lifelong

High-Risk Comorbidity Combinations in Older Patients Undergoing Emergency General Surgery.

BACKGROUND/OBJECTIVES: Over a million older patients in the United States are admitted yearly for emergency general surgery (EGS) conditions. Seven procedure types dominate: colon, small bowel, gallbladder, ulcer disease, adhesiolysis, appendix, and laparotomy operations. A higher comorbidity burden is known to increase mortality in this population, but the impact of specific comorbidity combinations is unknown. Our objectives were to (1) characterize the distribution of procedures, comorbidities, and outcomes for older patients undergoing EGS; and (2) apply a data-driven approach (association

Descriptive analysis of platelet-rich plasma injection therapy in chronic musculoskeletal pain.

INTRODUCTION: Platelet-rich plasma (PRP) injections have been used to manage various chronic pain conditions. However, evidence remains limited due to poor standardisation across practices. In this descriptive study, we aimed to characterise current PRP practice patterns at a university-affiliated private pain clinic in Hong Kong, focusing on case mix and treatment outcomes in patients with chronic musculoskeletal pain. METHODS: This retrospective descriptive study included patients with diverse chronic musculoskeletal pain conditions aged 18 years or older who attended the CUHK Medical Centr

Determination of uromodulin in human urine: influence of storage and processing.

BACKGROUND: Uromodulin (Tamm-Horsfall protein) is the most abundant protein excreted in the urine under physiological conditions. It is exclusively produced in the kidney and secreted into the urine via proteolytic cleavage. The involvement of UMOD, the gene that encodes uromodulin, in rare autosomal dominant diseases, and its robust genome-wide association with the risk of chronic kidney disease suggest that the level of uromodulin in urine could represent a critical biomarker for kidney function. The structure of uromodulin is complex, with multiple disulfide bonds and typical domains of ext

Socioeconomic inequalities in health-related quality of life during the COVID-19 pandemic: a six-country comparison using the EQ-5D-5 L.

PURPOSE: Despite the growing attention to health inequalities, there is no global consensus on how to measure socio-economic status. This study examined inequalities in health-related quality of life (HRQoL) during the early phase of the COVID-19 pandemic across six countries-China, Italy, the Netherlands, Sweden, the United Kingdom (UK), and the United States (US)-using three SES indicators: education level, income, and work status. METHODS: Between April and June 2020, individuals aged 18-75 years old completed a web-based survey. HRQoL was measured using the EQ-5D-5L Level Sum Score (LSS),

Psychological flexibility and health status as predictors of perceived hearing aid benefit in adults with hearing loss.

PURPOSE: This study investigated the relationship between psychological acceptance of hearing loss and perceived hearing aid benefit in older adults and examined whether psychological flexibility remains a significant predictor of benefit after accounting for demographic, audiological, and clinical factors. METHOD: In this study, 159 hearing aid users (mean age = 62.66 ± 15.66 years) completed the Turkish version of the Acceptance and Action Questionnaire-Adult Hearing Loss (AAQ-AHL) and the Abbreviated Profile of Hearing Aid Benefit (APHAB). Group comparisons, analyses of covariance (ANCOVA)

Evidence that Dry Eye Represents a Chronic Overlapping Pain Condition.

Recent data suggest that corneal somatosensory dysfunction may be the underlying cause of severe dry eye symptoms in the absence of ocular surface pathology seen in a subset of patients diagnosed with “dry eye syndrome.” This subset of patients tends to demonstrate a unique constellation of symptoms that are persistent, more severe, and generally respond poorly to current dry eye therapies targeting inadequate or dysfunctional tears. A growing body of literature suggests that symptoms in these patients may be better characterized as neuropathic ocular pain rather than dry eye. In these patient

Ocular inflammation in neurorheumatic disease.

Neuroimmunologic and systemic rheumatic diseases are frequently accompanied by inflammation of the eye, ocular adnexa, and orbital tissues. An understanding of the diverse forms of ophthalmic pathology in these conditions aids the clinician in making appropriate preventative, diagnostic, therapeutic, and prognostic decisions. In this review, the authors address ocular inflammation in neurorheumatic disease in three sections: first, they highlight current perspectives on immune mechanisms in the development of these disorders; next, they provide a framework for the recognition and evaluation of

Central sensitization syndromes as a neuropsychiatric risk state for dementia: Clinical and epidemiologic perspectives.

Dementia prevention increasingly targets modifiable midlife risk states contributing to neurodegenerative vulnerability. While vascular and metabolic factors are established, a substantial proportion of risk remains unexplained. Central sensitization syndromes (CSS)-chronic conditions characterized by neural amplification-are highly prevalent, co-occur with depression and sleep disturbance, and involve neuroimmune and network-level mechanisms implicated in early Alzheimer's disease. To synthesize epidemiologic evidence linking CSS with incident dementia and evaluate competing causal explanatio

Sex Hormones, Neurosteroids, and Glutamatergic Neurotransmission: A Review of the Literature.

Glutamatergic dysfunction has been implicated in the pathophysiology of multiple conditions including epilepsy, chronic pain, post-traumatic stress disorder (PTSD), and premenstrual dysphoric disorder (PMDD), raising interest in potential ways of modifying glutamate in the nervous system. Emerging research has suggested an interactive effect between sex hormones and glutamatergic neurotransmission. The objective of this paper was to review existing literature on the mechanism of interaction between sex hormones and glutamatergic neurotransmission, as well as to explore what is known about thes

Musculoskeletal Treatments: Physical Modalities.

Chronic musculoskeletal pain disorders account for 70% to 80% of chronic pain diagnoses in the United States. T he most common conditions are low back pain, neck pain, headache disorders, and knee and hip osteoarthritis. Most evidence suggests that treatments for chronic pain are best used in combination, such as in multidisciplinary rehabilitation programs. Evaluation of individual treatments in isolation is limited by small sample sizes, high risk of bias, lack of standardization in clinician delivery and treatment dosage or intensity, and inconsistent categorization of treatment techniques.

Not All Microbiomes Reflect Chronic Pain: Evidence from the Urinary Tract in a Case-Control Study.

Background/Objectives: Chronic pain is increasingly conceptualized as a systemic condition characterized by central sensitization, autonomic dysregulation, and persistent neuroimmune and neuroendocrine alterations. These systemic changes have been linked to microbial dysbiosis, most prominently within the gut microbiome. In contrast, the relevance of the urinary microbiome outside primary urological disease remains poorly understood, particularly in non-urological chronic pain conditions. The objective of this study was to determine whether patients with chronic low back pain exhibit differenc

Fatal Legionella pneumophila serogroup 1 pleural empyema: A case report.

BACKGROUND: Legionella pneumophila (L. pneumophila) is a gram-negative intracellular bacillus composed of sixteen different serogroups. It is mostly known to cause pneumonia in individuals with known risk factors as immunocompromised status, tobacco use, chronic organ failure or age older than 50 years. Although parapneumonic pleural effusion is frequent in legionellosis, pleural empyema is very uncommon. In this study, we report a case of fatal pleural empyema caused by L. pneumophila serogroup 1 in an 81-year-old man with multiple risk factors. CASE SUMMARY: An 81-year-old man presented to
