Secondarily reduced cytochrome c oxidase activity in various neuromuscular disorders.

Decreased cytochrome c oxidase (CCO) activity was found in various neuromuscular diseases, including infantile spinal muscular atrophy (SMA) and Fukuyama type congenital muscular dystrophy (FCMD), that was thought to result from a secondarily induced mitochondrial defect. To determine whether or not the enzyme activities in the mitochondrial electron transport system can be secondarily changed, we measured the enzyme activities in denervated rat muscles. The CCO activity decreased progressively to 38-47% of the control value in 3 weeks after denervation, the NADH-cytochrome c reductase and suc

MRI-measured tendon retraction distance is associated with EMG-confirmed neurotrauma in proximal hamstring avulsion.

PURPOSE: Complete proximal hamstring avulsion injuries are anatomically complex because of their proximity to the sciatic nerve. This study characterises neurological abnormalities following injury and surgical repair. Although denervation has been described, its severity, pattern and diagnostic thresholds in relation to magnetic resonance imaging (MRI) findings, anatomical innervation and tendon retraction distance remain poorly defined. METHODS: In this prospective longitudinal cohort study, 18 patients undergoing surgical repair of MRI-confirmed complete proximal hamstring avulsion were ev

Exercise-mediated reinnervation of skeletal muscle in elderly people: An update.

Sarcopenia is defined by the loss of muscle mass and function. In aging sarcopenia is due to mild chronic inflammation but also to fiber-intrinsic defects, such as mitochondrial dysfunction. Age-related sarcopenia is associated with physical disability and lowered quality of life. In addition to skeletal muscle, the nervous tissue is also affected in elderly people. With aging, type 2 fast fibers preferentially undergo denervation and are reinnervated by slow-twitch motor neurons. They spread forming new neuro-muscular junctions with the denervated fibers: the result is an increased proportion

Idiopathic peripheral neuropathy in the horse with knuckling: muscle and nerve lesions in additional cases.

We have previously reported a pathological investigation of peripheral neuropathy in a horse with knuckling. This report describes details of the muscle and peripheral nerve lesions in two additional cases of light horse yearlings with knuckling. The skeletal muscles showed neurogenic atrophy characterized by scattered single angular fibers, fiber grouping, and fiber-type grouping. The severity of muscle lesions increased distally; that is, both fore- and hindleg muscles were affected more severely than cervical and dorsal muscles. In the peripheral nervous system, a number of Renaut bodies ap

Remodeling of the neuromuscular junction in myasthenia gravis increases serum neurofilament heavy chain levels.

INTRODUCTION/AIMS: In myasthenia gravis, prolonged muscle denervation causes muscle atrophy. We re-visited this observation using a biomarker hypothesis. We tested if serum neurofilament heavy chain levels, a biomarker for axonal degeneration, were elevated in myasthenia gravis. METHODS: We enrolled 70 patients with isolated ocular myasthenia gravis and 74 controls recruited from patients in the emergency department. Demographic data were collected alongside serum samples. Serum samples were analyzed by enzyme-linked immunosorbent assay (ELISA) for the neurofilament heavy chain (NfH-SMI35). T…

Muscle involvement in a case of oculocutaneous albinism.

A 2-month-old girl with tyrosinase-positive oculocutaneous albinism (OCA) and severe muscle hypotonia is reported. She was admitted to our hospital because of poor sucking and poor weight gain. On physical examination she was found to have generalized muscle weakness and multiple anomalies including deafness, mental retardation, cataracta and a high-arched palate. A muscle biopsy showed marked variation in fiber size with bimodal distribution, suggesting a neuropathic process. Since electromyography showed a myopathic pattern, CK was definitely elevated and muscle histologic examination did no

Imaging of peripheral nerve lesions in the lower limb.

Lower limb peripheral neuropathy may have a variety of causes. This article focuses on focal neural lesions because of neural entrapment ass … aumatic neuroma in continuity) and intraneural ganglia. Imaging can also demonstrate the effects of muscle denervation. Focal areas of tenderness can be highlighted using skin markers for magnetic resonance imaging and by transducer palpation on ultrasound. Ultrasound can be particularly useful in assessing for intrinsic lesions in small peripheral nerves because of …

Driving fibrosis in neuromuscular diseases: Role and regulation of Connective tissue growth factor (CCN2/CTGF).

Connective tissue growth factor or cellular communication network 2 (CCN2/CTGF) is a matricellular protein member of the CCN family involved in several crucial biological processes. In skeletal muscle, CCN2/CTGF abundance is elevated in human muscle biopsies and/or animal models for diverse neuromuscular pathologies, including muscular dystrophies, neurodegenerative disorders, muscle denervation, and muscle overuse. In this context, CCN2/CTGF is deeply involved in extracellular matrix (ECM) modulation, acting as a strong pro-fibrotic factor that promotes excessive ECM accumulation. Reducing CC…

Botulinum toxin type A: implications in wound healing, facial cutaneous scarring, and cleft lip repair.

Botulinum toxin is a neurotoxin that has been utilized to induce chemo-denervation of muscles. Cutaneous wounds represent a special situation in which the tensile forces applied by these muscles on wound edges might have deleterious effects on the healing process. The aim of this review was to investigate such an effect and to review other mechanisms this toxin might have on the healing process. We also reviewed the role of botulinum toxin in the management of hypertrophic scars and cleft lip repair.

Postpartum neuralgic amyotrophy with involvement of the anterior interosseous nerve.

Neuralgic amyotrophy (NA) is a syndrome of unknown etiology associated with pain in the necl" and shoulder girdle followed by wasting and weakness in muscles supplied by the brachial plexus. Electromyographic studies typically show evidence of denervation in affected muscles. In this report, two cases of NA, both of which presented in the postpartum period, ace described. Both of the described cases demonstrated severe involvement of the anterior interosseous nerve. The course and electrophysiological assessment of postpartum NA is discussed.

Nerve implantation to the muscle after neuroma formation enables pain relief and motor function recovery with reinnervation in rats.

AIMS: Neuroma formation after peripheral nerve injury inhibits functional recovery and contributes to neuropathic pain. This study aimed to evaluate whether nerve implantation into denervated muscle, even after neuroma formation, could promote motor and sensory functional restoration. MATERIALS AND METHODS: A rat tibial nerve injury model was used. The tibial nerve was ligated and transected to induce neuroma, followed by nerve implantation into the gastrocnemius muscle at 12 days post-injury. Mechanical hypersensitivity was assessed by von Frey testing, and motor function was evaluated using

Central core myopathy with RYR1 mutation masks 5q spinal muscular atrophy.

We report the case of a male who presented in infancy with motor delay and muscle weakness. Typical muscle biopsy features and heterozygous RYR1 mutation confirmed a diagnosis of central core disease. Family studies showed this to be a de-novo mutation. Some years later, his two older teenage brothers presented with proximal muscle weakness. Neurophysiology, muscle biopsy and DNA studies confirmed spinal muscular atrophy. Subsequent genetic studies in the index case also confirmed homozygous deletions of exon 7 and 8 in the SMN gene. Review of the original muscle biopsy showed classical featur

Improving nerve and muscle function: an exploration of targeted nerve function replacement following differential delay periods in a rat model.

BACKGROUND: Targeted Muscle Reinnervation (TMR) improves real-time control of EMG-based prostheses by connecting severed nerves to adjacent muscles, creating new EMG signals. However, TMR requires cutting original nerve connections, which can cause denervation atrophy and limit functional recovery. As an alternative, Targeted Nerve Function Replacement (TNFR) offers a fundamentally different approach by establishing a direct end-to-end anastomosis between an intact donor nerve and the original nerve of a target muscle, preserving existing neural pathways while providing supplementary neural in

Direct injection of liposome-encapsulated doxorubicin optimizes chemomyectomy in rabbit eyelid.

PURPOSE: Doxorubicin chemomyectomy presently represents the only permanent, nonsurgical treatment for blepharospasm and hemifacial spasm. The major deterrent to an otherwise extremely effective treatment protocol is the development in patients of localized inflammation, discomfort, and skin injury over the injection site. As a potential alternative therapy, Doxil (Sequus, Menlo Park, CA), a liposome-encapsulated form of doxorubicin that displays tissue-selective therapeutic effects compared with free doxorubicin, was examined. These effects have been related to its increased retention in tissu

Transformation of contraction speed in muscle following cross-reinnervation; dependence on muscle size.

The characteristics of isometric contractions and the force-velocity relation were studied in flexor digitorum longus, flexor hallucis longus and soleus muscles of the cat, in situ, at 37 degrees C and with nerve stimulation. The two flexors were identified as typical fast twitch muscles and the soleus as a typical slow twitch muscle. Following self-reinnervation, both fast and slow muscles retained, to a large extent, their basic contraction characteristics. The soleus muscle, when cross-reinnervated with the nerve of either flexor hallucis longus muscle or extensor digitorum longus muscle ex

Acetylcholine contractures of skeletal muscles: inhibition by chlorpromazine and diltiazem.

The maximum contractures evoked by 100 microM ACh of mouse soleus muscles denervated for 3-7 days are completely inhibited by a 10 min exposure to 100 microM chlorpromazine (CP). Recovery on washout of CP takes more than 1 hr to complete. ACh evoked contractures are also inhibited by diltiazem (D); washout of D is immediately followed by recovery. Electrically evoked twitches and K evoked contractures are largely unaffected by CP, caffeine evoked contractures are decreased but not abolished. Fast mouse and (non-denervated) frog tonic muscles behave similarly. Depolarization by ACh and ACh-evok

Muscular atrophy and weakness in the lower extremities in Behçet's disease: A case report and review of literature.

BACKGROUND: In Behçet's disease (BD), very few cases of muscular involvement have been reported previously. The natural history and therapeutic protocol for muscular involvement in BD are obscure due to the low incidence of peripheral neuropathy or myopathy in BD. The purpose of our study was to report a rare case of BD with chronic, focal forms of neuromyopathy and review the relevant literature. CASE SUMMARY: We herein report the case of a 54-year-old man who presented with progressive muscular atrophy and weakness of both thighs 2 years after the presentation of the cardinal symptoms of BD

[A case of distal spinal muscular atrophy].

A 13-year-old boy with distal spinal muscular atrophy was reported. He noted atrophy of the bilateral legs since 11 years old. Ankle jerk was brisk and pes cavus was observed. Needle EMG of the leg showed denervation pattern. Muscle biopsy of the gastrocnemius muscle revealed large group atrophy and fiber type grouping. MCV, SCV and results of sural nerve biopsy were normal. We diagnosed his disorder as distal muscular atrophy. The essential lesion of this disease was supposed to be in the anterior horn cells.

Intestinal giardiasis, steatorrhoea and peripheral nerve dysfunction.

Two patients presented with similar peripheral neuropathic symptoms. Muscle denervation was shown by electromyography and muscle biopsy. Motor and sensory nerve conduction studies were normal. Both patients had intestinal giardiasis: 1 had asymptomatic steatorrhoea, and the other had diarrhoea and abdominal pain. Treatment with metronidazole abolished the gastrointestinal symptoms. After a delay of several months the neurological symptoms also gradually improved. No other cause of peripheral neuropathy was apparent on investigation, and the relationship between the intestinal giardiasis and th…
