Fanconi syndrome, ichthyosis, dysmorphism, jaundice and diarrhoea--a new syndrome.

We describe six infants, from consanguineous marriages, with a new syndrome comprising the Fanconi syndrome, ichthyosis, musculoskeletal abnormalities, jaundice and diarrhoea. In addition two of the infants were found to have abnormal platelet morphology--the grey platelet syndrome. No evidence of a recognised metabolic disorder was found in any of the six infants, nor did they appear to be typical of any previously described syndromes. Their progress was poor: they required high fluid and bicarbonate intakes and all died by the age of 6 months of dehydration, acidosis and sepsis.

Atypical Chest Pain: A Diagnostic Challenge for Takotsubo Cardiomyopathy.

Takotsubo cardiomyopathy (TTC) typically presents in post-menopausal females with chest pain, dyspnea, or palpitations and often mimics acute coronary syndrome. This case report describes a unique presentation of TTC in an elderly patient who exhibited gastrointestinal and musculoskeletal symptoms without cardiopulmonary complaints. This constellation of symptoms has not been previously described in the literature. The patient was ultimately diagnosed with TTC with evidence of ST-T wave abnormalities on ECG, elevated serum troponin levels, a reduced left ventricular ejection fraction, the abse

Homozygous deletion of exons 2-7 within TGFB3 gene in a child with severe Loeys-Dietz syndrome and Marfan-like features.

We describe a patient with palatal abnormalities-cleft palate and bifid uvula; distinctive facial features-long and triangular face, large ears and nose, thin lips and dental crowding; musculoskeletal abnormalities-severe scoliosis, joint laxity, long digits, flat feet, decreased muscle mass, and diminished muscle strength; and cardiac features-a dilatated ascending aorta at the level of Valsalva sinuses and a patent foramen ovale. Sequence analysis and deletion/duplication testing for a panel of genes involved in connective tissue disorders revealed the presence of a novel homozygous deletion…

Congenital longitudinal deficiency.

Radial and ulnar longitudinal deficiencies are the 2 most common types of congenital longitudinal deficiencies of the arm, with radial deficiency being 3 to 4 times more common. They are a spectrum of abnormalities, ranging from mild deficiency of the digits to complete loss of one-half the forearm, wrist, and fingers. Radial longitudinal deficiency is associated with a number of medical syndromes that require a comprehensive medical evaluation, while ulnar longitudinal deficiency (ULD) is associated with other musculoskeletal anomalies. Both conditions have a high incidence of ipsilateral thu

Congenital spine deformities. A review of 47 cases.

Forty-two cases of congenital scoliosis, four cases of congenital kyphosis and one case of congenital lordosis were reviewed retrospectively from 1975 through 1982, with an average follow-up of 5.09 years. Associated anomalies occurred in 62.5% of the cases, with Sprengel's deformity and Klippel-Feil syndrome being the most common musculoskeletal anomalies. The rate of abnormal intravenous pyelography was 21.8%, and absent kidneys or duplication of collecting systems accounted for the majority of renal abnormalities. Myelographic abnormalities were noted in eight of twenty-five studies and inc

Joubert syndrome with nephronophthisis in neurofibromatosis type 1.

Joubert syndrome (JS) is a rare developmental disorder of the central nervous system, characterised by brainstem and cerebellar malformations, hypotonia, episodic hyperapnea and apnea and mental retardation. It may be associated other systemic abnormalities like ocular (e.g., retinal dysplasia, etc.), oculomotor, musculoskeletal and renal (e.g., cystic dysplasia, nephronophthisis), with renal failure. We describe a case of JS with nephronophthisis in neurofibromatosis Type 1 leading to end-stage renal disease, a association that has never been described earlier in the medical literature to the

Hypomelanosis of Ito and hemimegalencephaly.

Hypomelanosis of Ito is a congenital neurocutaneous syndrome with a particular pattern of swirling hypopigmentation. Multiple extracutaneous abnormalities involving the central nervous system, the eyes, and musculoskeletal structures occur in over two-thirds of the cases. This report describes two patients with typical unilateral cutaneous lesions associated with extracutaneous features, including hypertrophy of the cerebral hemisphere contralateral to the cutaneous hypopigmentation. Magnetic resonance imaging and EEG findings support the diagnosis of hemimegalencephaly, as has recently been r

Unilateral athelia with a subcutaneous dermoid cyst.

A case of unilateral athelia with a subcutaneous dermoid cyst in the area in question is reported. The histologic features of the cyst wall suggest that it is derived from a base in the nipple-areola complex. Dozens of cases of congenital absence of the breast have been reported, some of them associated with congenital anhidrotic ectodermal dysplasia, others with additional musculoskeletal abnormalities, and still others normal in other respects. But to our knowledge, the combination of athelia and corresponding subcutaneous dermoid cyst has not been reported.

Resolution Biology in Soft Tissue Joint Disease.

Injured joint soft tissues, including tendons and ligaments, repair through an inflammatory process, which can result in fibrosis and persistent tissue abnormality, increasing the likelihood of recurrent injury. The aetiology of disease affecting tendons and ligaments is complex and multifactorial, including interplay between the effects of repetitive wear and tear, daily exercise and ageing. This chapter summarises new knowledge on how inflammation contributes to the onset and progression of soft tissue joint disease, highlighting fibroblasts and macrophages as important tissue-resident cell

The Effects of Microplastics on Musculoskeletal Disorder; A Narrative Review.

PURPOSE OF REVIEW: The physical health impacts of microplastics have received increasing attention in recent years. However, limited data impedes a full understanding of the internal exposure to microplastics, especially concerning the musculoskeletal system. The purpose of this review is to summarize the recent literature regarding the effects of microplastics on the musculoskeletal system. RECENT FINDINGS: Microplastics have been shown to cause abnormal endochondral ossification and disrupt the normal function of pre-osteoblasts, osteocyte-like cells, and pre-osteoclasts through gene mutati

An unusual case of coccidioidomycosis.

Dissemination of coccidioidomycosis is rare. The skin, musculoskeletal system, and central nervous system have been described as the most common sites of extrapulmonary disease. We present a case of an asymptomatic patient in whom the diagnosis of coccidioidomycosis was made on a lymph node biopsy. The biopsy was performed because of an abnormal mammogram and physical findings suggestive of malignancy.

Musculoskeletal Complications of CancerA Survey of 50 Patients.

A variety of complications associated with cancer may adversely affect the spine and musculoskeletal system, resulting in physical impairment, pain, or both. Dysfunction may arise because of tissue injury at the primary site of disease; from metastatic lesions and paraneoplastic syndromes; or as the result of treatment and bed rest. This study evaluated 50 patients seen in consultation by the Rehabilitation Service at Memorial Sloan-Kettering Cancer Center (28 retrospective, 22 prospective). Patients were evaluated for functional problems resulting from their disease or therapy. Patients were

Klippel-Feil Syndrome with Sprengel Deformity and Extensive Upper Extremity Deformity: A Case Report and Literature Review.

INTRODUCTION: Klippel-Feil syndrome (KFS) is a congenital anomaly resulting from fusion of cervical vertebral bodies secondary to the dysregulation of signaling pathways during somite development. It is commonly associated with scoliosis and Sprengel deformity. We present a case of KFS with commonly associated abnormalities as well as deformities that have not yet been reported in the literature. CASE PRESENTATION: A 3-year-old girl presented for further evaluation of a left upper extremity deformity following a negative genetic workup. Upon physical exam and radiographic imaging, the patient

Congenital Knee Dislocation: A Case Report.

Congenital knee dislocation (CKD) is a rare musculoskeletal condition that may occur in isolation or be associated with other congenital abnormalities. Prognosis depends on the severity of the deformity and the timing of diagnosis and treatment. Comprehensive assessment and close follow‑up are essential to optimize recovery and to exclude associated anomalies. We report the case of a female newborn with CKD, diagnosed postnatally, associated with developmental dysplasia of the hip (DDH), generalized ligamentous laxity, congenital heart disease, and a chromosomal abnormality. Early conservative

Shoe and Bracing Considerations for the Insensate Foot: Shoe considerations for diabetic foot.

Diabetic individuals with peripheral neuropathy are at risk for the development of foot ulcers due to musculoskeletal abnormalities and abnormal loading in the gait cycle leading to elevated plantar pressures. To prevent diabetic foot ulcers, practitioners should regularly screen patients for the presence of neuropathy as well as neuroarthropathies and prescribe the appropriate shoes and orthotics based on the best available clinical evidence. Although not widely available, there is potential for data-driven customization of orthotics and shoe wear based on plantar pressure data to prevent the…
