Applicability of the New Zealand-eating behavior questionnaire to predict weight loss responses to naltrexone/bupropion: a proof-of-concept trial.

BACKGROUND: Despite the availability of different obesity medications, individual responses vary widely. This pilot study evaluated whether the New Zealand Eating Behavior Questionnaire (NZ-EBQ) could predict weight loss response to naltrexone/bupropion in adults with clinical obesity. METHODS: We conducted a 24-week prospective, single-site, pilot, exploratory, phenotype-stratified trial evaluating utility of the NZ-EBQ for predicting weight reduction response to naltrexone/bupropion. Prior to enrolment, participants completed the NZ-EBQ, which classified their eating behavior phenotype and …

OPRM1 rs1799971 Polymorphism Predicts Differential Response to Extended-Release Naltrexone in Alcohol Use Disorder: The Interplay of Genetics and Motivation.

Alcohol use disorder (AUD) remains a major contributor to the global burden of disease. Extended-release naltrexone (XR-NTX), a μ-opioid receptor antagonist, is a key pharmacological option for maintaining abstinence, though individual treatment response varies substantially. Genetic variability in the μ-opioid receptor gene (OPRM1), particularly rs1799971, may underpin this heterogeneity. Study objective was to evaluate the association between OPRM1 rs1799971 polymorphism and clinical outcomes of XR-NTX therapy in patients with AUD while examining interactions with motivational profiles. In t…

Safety and Tolerability of Injectable Extended-Release Naltrexone for the Management of Alcohol Use Disorder in Advanced Alcohol-Associated Liver Disease.

BACKGROUND: Pharmacologic treatment of alcohol use disorder (AUD) in patients with advanced alcohol-associated liver disease (ALD) remains underutilised due to concerns regarding hepatotoxicity. Injectable extended-release naltrexone (XR-NTX) may offer a safer alternative by avoiding first-pass hepatic metabolism, but data on its safety and effectiveness in patients with advanced ALD are limited. AIM: To describe the clinical experience with XR-NTX in individuals with advanced ALD, evaluating its safety, tolerability and impact on liver function and alcohol use. METHODS: Retrospective case s…

A Randomized, Blinded, Placebo-Controlled Crossover Study of the Pharmacokinetics and Pharmacodynamics of Naloxone, Naltrexone, and Nalmefene in Methadone-Sedated Working Dogs.

A randomized, blinded, placebo-controlled crossover study was performed with eight professional working dogs to evaluate the pharmacokinetics and pharmacodynamics of three opioid reversal agents. Following sedation with 1 mg kg-1 methadone HCl IV, dogs were randomly assigned to receive naloxone, naltrexone, or nalmefene at 0.1 mg kg-1 IM, or saline (0.1 mL kg-1). Sedation scores and vital signs were obtained for 6 h, and blood samples were obtained for 72 h. Across all groups and phases, the mean sedation score prior to methadone was 0.93/14, and prior to reversal/placebo was 11.45/14. Mean se…

Development of a liquid chromatography-tandem mass spectrometry method for the analysis of plasma naltrexone and its active metabolite in patients with AUD.

Naltrexone, an FDA-approved mu-opioid receptor antagonist for alcohol dependence, requires therapeutic drug monitoring due to its pharmacokinetic variability. This study developed a liquid chromatography-electrospray ionization-tandem mass spectrometry method for accurately quantifying naltrexone and its active metabolite, 6β-naltrexol, using deuterated internal standards (naltrexone-d3 and 6β-naltrexol-d3). Sample preparation involved solid-phase extraction with a Strata-X cartridge followed by elution. Separation was performed on a C18 column with gradient elution at 0.3 mL/min. Detection in…

Carfentanil stabilizes µ opioid receptor conformations that are ultra-efficient in inhibiting cAMP, resistant to naloxone or nalmefene but sensitive to naltrexone.

The highly potent opioid carfentanil (CAR) represents a growing health risk. CAR acts via Gi/o-coupled µ opioid receptors (µOR) and exhibits … are rather resistant towards clinically used antidotes. Of note, when the long acting OR antagonist naltrexone was tested, it exhibited a ~ 65-times higher potency to inhibit CAR but not fentanyl compared to naloxone. Our data highlight the unique nature of CAR's interactions with µOR and provide first pharmacodynamic indication that naltrexone might be a su …

Diagnosis and Treatment of Presumed Naltrexone-XR-precipitated Opioid Withdrawal in a Patient Chronically Treated With Buprenorphine-XR: A Case Report.

Naltrexone, buprenorphine, and methadone are Food and Drug Administration-approved medications for the treatment of opioid use disorder in the United States. Naltrexone, an opioid antagonist, can precipitate opioid withdrawal if administered too quickly after the use of full or partial opioid agonists for those with either dependence or use disorder. We describe a case of severe precipitated opioid withdrawal syndrome after reported buprenorphine extended-release (XR) administration, despite the patient having been stable on buprenorphine-XR for several years, with no missed doses or recent op…

Estimating the impact of stimulant use on initiation of buprenorphine and extended-release naltrexone in two clinical trials and real-world populations.

BACKGROUND: Co-use of stimulants and opioids is rapidly increasing. Randomized clinical trials (RCTs) have established the efficacy of medications for opioid use disorder (MOUD), but stimulant use may decrease the likelihood of initiating MOUD treatment. Furthermore, trial participants may not represent "real-world" populations who would benefit from treatment. METHODS: We conducted a two-stage analysis. First, associations between stimulant use (time-varying urine drug screens for cocaine, methamphetamine, or amphetamines) and initiation of buprenorphine or extended-release naltrexone (XR-NT…

Availability of Extended-Release Naltrexone May Increase the Number of Opioid-Dependent Individuals in Treatment: Extension of a Randomized Clinical Trial.

BACKGROUND AND OBJECTIVE: Opioid maintenance treatment (OMT) is highly available in Norway, but only 50% of opioid-dependent individuals are enrolled in such programs. This study was aimed at examining if availability of extended-release naltrexone (XR-NTX) could attract individuals who for different reasons were not enrolled in an OMT program. METHODS: In a Norwegian clinical study, n = 117 opioid-dependent adults volunteered to receive XR-NTX in a 9-month period, as an extension of a previous randomized clinical trial. RESULTS: Before study inclusion, 40.2% (n = 47) of the study participan…

The effect of naltrexone and acamprosate on cue-induced craving, autonomic nervous system and neuroendocrine reactions to alcohol-related cues in alcoholics.

INTRODUCTION: Acamprosate and naltrexone have been shown to be effective in relapse prevention of alcoholism. It is hypothesized that naltrexone exerts its effects primarily on cue-induced craving and neuroendocrine cue reactivity, whereas acamprosate exerts its effect primarily on autonomic nervous system reactions to alcohol-related cues. EXPERIMENTAL PROCEDURES: In a randomized double-blind experiment, 131 abstinent alcoholics received either acamprosate (n=56), naltrexone (n=52) or placebo (n=23) for three weeks and participated in two cue-exposure sessions: the first the day before and t…

Reinstatement of ethanol-seeking behavior by drug cues following single versus multiple ethanol intoxication in the rat: effects of naltrexone.

RATIONALE: A positive relationship exists between chronic ethanol intoxication experiences and the severity of neural hyperactivity and withdrawal seizures. An important possibility is that withdrawal reactions also influence the motivation to obtain and consume ethanol. OBJECTIVES: To test this hypothesis, the effects of ethanol-cues on the recovery of extinguished ethanol-seeking and the reversal of this effect by naltrexone, were determined in non-dependent rats and in rats subjected to single versus repeated ethanol intoxications. METHODS: Rats were trained to self-administer and discrim…

Effects of naltrexone and fluoxetine on alcohol self-administration and reinstatement of alcohol seeking induced by priming injections of alcohol and exposure to stress.

We have recently shown that priming injections of alcohol and footshock stress reinstate alcohol seeking in drug-free rats. Here we tested whether naltrexone and fluoxetine, two drugs used in the treatment of alcohol dependence, would affect reinstatement of alcohol seeking induced by these events. We also determined the effects of these drugs on alcohol self-administration during the maintenance phase. Rats were trained to press a lever for a 12% w/v alcohol solution. After stable drug-taking behavior was obtained, lever pressing for alcohol was extinguished. Reinstatement of drug seeking was…

Prevention of late fentanyl-induced respiratory depression after the injection of opiate antagonists naltrexone and S-20682: comparison with naloxone.

Compared with naloxone, two opiate antagonists (naltrexone and S-20682) were tested in the intact dog in order to reverse respiratory depression, induced by fentanyl 50 micrograms kg-1 i.v. Respiratory rate and arterial blood-gases were measured at 5, 30, 60, 120, 180 and 240 min after the additional i.v. injection of the antagonist (1 microgram kg-1). Only S-20682, but not naloxone or naltrexone, prevented late (240 min) fentanyl-induced depression of respiratory drive. A late respiratory depression coincided with an increase in amplitudes and a reduction of frequency in electrical cortical a…
