Pressure pain and isometric strength of neck flexors are related in chronic tension-type headache.

BACKGROUND: In patients with chronic tension-type headache (CTTH) changes in pressure pain in the cervical region are associated with peripheral or central sensitization. It is hypothesized that an increase of isometric strength of neck flexors would lead to a decrease of pressure pain in CTTH, as an expression of reduced peripheral or central sensitization OBJECTIVE: In this study we aimed to analyze the correlation between change in isometric strength of the neck flexors and change in pressure pain scores (PPS) in patients with CTTH. STUDY DESIGN: Comparative analysis of data from previous

A rare case of Parkinson's disease with severe neck pain owing to crowned dens syndrome.

BACKGROUND: Pain is regarded as one of the most common nonmotor symptoms in Parkinson's disease (PD). In particular, musculoskeletal pain has been reported as the most common type of PD-associated pain. Crowned dens syndrome (CDS), related to microcrystalline deposition in the periodontoid process, is the main cause of acute or chronic cervical pain. CASE PRESENTATION: This report describes the case of an 87-year-old woman who had severe bradykinesia, muscle rigidity, gait disturbance and neck pain. Laboratory examination revealed marked elevations of white blood cells (10,100/µl) and C-react…

The use of transdermal buprenorphine to relieve radiotherapy-related pain in head and neck cancer patients.

Many head and neck cancer (HNC) patients experience painful therapy-related mucositis and dermatitis. This prospective observational study evaluated transdermal buprenorphine use in HNC patients to relieve treatment-related pain. During treatment with paracetamol or tramadol, visual analogue scale (VAS)-pain scores >30/100 occurred in 26/45 patients 4 weeks after starting cancer therapy, persisting for ≥2 weeks after treatment. These patients subsequently received transdermal buprenorphine. Pain therapy should be more accurately up-titrated to the maximum recommended dose (140 μg/hr) where nec

Effect of cycling on oxygenation of relaxed neck/shoulder muscles in women with and without chronic pain.

Work-related neck/shoulder muscle pain has been associated with increased anaerobic muscle metabolism. Thus, interventions to enhance oxygenation of painful muscles seem relevant. While cycling with relaxed shoulders has been shown to result in acute neck/shoulder muscle pain reduction, the effect on tissue oxygenation remains unknown. The aim of the present study was to investigate tissue oxygenation of the passive trapezius muscle during and after cycling in female workers with (MYA) and without (CON) trapezius myalgia. Eligible participants (n = 17 MYA, n = 8 CON) performed 20 min sub-maxim

Infusional 5-fluorouracil can be a pain in the neck: A case for repositioning displaced Hickman lines.

Increasing numbers of patients receive infusional chemotherapy or total parenteral nutrition via Hickman or Grochong lines. Although the insertion of these indwelling catheters is generally performed under radiological guidance and their positions verified by chest radiography, it is still feasible for them to become displaced at a later date. This possibility should be excluded in patients who develop unusual symptoms during the course of their infusional therapy. We review the reported complications associated with Hickman lines, and present a case history demonstrating that interventional r

Is there an association between diabetes and neck and back pain? An updated systematic review with meta-analyses.

BACKGROUND: Spinal pain and disorders are a major burden on our population and health systems. Previous research showed people with diabetes were 1.35 times and 1.24 times more likely to report low back pain and neck pain than people without diabetes respectively, but diabetes was not a predictor of future spinal pain. Here we update a previous systematic review to determine whether in an adult population, diabetes is associated with low back, neck, or spinal pain. METHODS: The search was performed in PUBMED, Medline, CINAHL and EMBASE. Studies between 2017-2025, of cross-sectional, case-cont…

Red flags for extracranial vertebral artery dissections in patients with neck pain: a scoping review.

OBJECTIVES: Neck pain is common and generally benign, but in rare cases it may be the early symptom of vertebral artery dissection (VAD). To date, no published secondary analysis addressing red flags for VADs in patients whose primary complaint is neck pain, nor any describing the typical clinical presentation, demographic characteristics, and common risk factors of VAD, has been published. Therefore, this scoping review addresses that gap by systematically mapping available evidence on red flags, predisposing risk factors, clinical features at presentation, and demographic profiles in patient…
